We report the case of a 39 year old woman with a history of intravenous drug use who developed endocarditis with direct examination of the resected valve and vegetation showing the presence of mycelia. Cultures were positive for an Aspergillus of section Nigri, which was subsequently identified as Aspergillus tubingensis by sequencing. The clinical course was favorable following surgery and prolonged antifungal therapy (8 months in total). Antifungal susceptibility testing showed good in vitro activity of amphotericin B, voriconazole and echinocandins against planktonic cells of this A. tubingensis isolate. However, only amphotericin B displayed significant activity against biofilms. In vitro combinations of voriconazole or amphotericin B with echinocandins did not meet the criteria of synergism. Our review of the literature identified 17 other cases of endocarditis attributed to Aspergillus of section Nigri with an overall mortality rate of 57% (100% in the absence of surgery). Endocarditis caused by Aspergillus niger and related cryptic species are rare events, for which surgical management appears to be crucial for outcome. While amphotericin B was the only antifungal drug displaying significant anti-biofilm activity, the type and duration of antifungal therapy remain to be determined.

Chimeric antigen receptor (CAR) T-cell therapy is a relatively new, innovative treatment strategy to manage refractory hematological cancers, including some types of leukemia, lymphoma, and multiple myeloma. This article outlines the CAR T-cell therapy process, toxicity, and complications, along with an overview of the currently known short- and long-term physical and functional sequelae that will be helpful for general or oncology rehabilitation specialists caring for these patients.

There is a dearth of literature on the topic of rehabilitation of patients receiving CAR T-cell therapy. Rehabilitation practices can be extrapolated from the limited functional information on patients who have completed treatment for lymphoma and multiple myeloma. Patients present with cognitive impairment, muscle weakness, reduced exercise capacity, neuropathy, and cancer-related fatigue. Physical activity and rehabilitation programs may be beneficial to address fatigue, psychological symptoms, and quality of life. There in patients receiving CAR T-cell therapy. These patients may present with general deconditioning and neurological complications which translate to neuromuscular and cognitive impairment that benefit from multidisciplinary rehabilitation intervention prior to, during, and after treatment. Studies measuring the impairments at baseline and evaluation of the impact of rehabilitation practices are much needed to support this.

Few studies have evaluated the association between non-clinical determinants (socioeconomic status and geographic accessibility to healthcare) and the outcomes of bariatric surgery, with conflicting results. This study aimed to evaluate this association.

The medical records of 1599 consecutive patients who underwent either laparoscopic Roux-en-Y gastric bypass or laparoscopic sleeve gastrectomy between June 2005 and December 2017 were retrieved. All relevant data, including patient characteristics, biometric values before and after surgery, related medical problems, surgical history, medications, and habitus, for each patient were prospectively collected in a database. Logistic regressions were used to assess the influence of non-clinical determinants on surgical indications and complications. Multilevel linear or logistic regression was used to evaluate the influence of non-clinical determinants on long-term %TWL and the probability to achieve adequate weight loss (defined as a %TWL > 20% at 12months)is associated with post-operative mortality and morbidity. This results suggests that bariatric surgery is a safe and effective tool for weight loss despite socioeconomic deprivation.Age-related macular degeneration (AMD) is a degenerative disorder in the macular region of the eye. AMD is the leading cause of irreversible vision loss in the elderly population. With the increase in aged population in the world, there is an urgent need to develop low-cost, hassle-free, and portable equipment diagnostic and analytical tools for early diagnosis. As AMD detection is done by examining the fundus images, its diagnosis is heavily dependent on medical personnel and their experience. To remove this issue, computer-aided algorithms may be used for AMD detection. The proposed work offers an effective solution to the AMD detection problem. It proposes a novel 13-layer deep convolutional neural network (DCNN) architecture to screen fundus images to spot direct signs of AMD. Five pairs of convolution and maxpool layers and three fully connected layers are utilized in the proposed network. Extensive simulations on original and augmented versions of two datasets (iChallenge-AMD and ARIA) consisting of healthy and diseased cases show a classification accuracy of 89.75%, 91.69%, and 99.45% on original and augmented versions of iChallenge-AMD and 90.00%, 93.03%, and 99.55% on ARIA, using a 10-fold cross-validation technique. It surpasses the best-known algorithm using DCNN by 2%.Human lung organoids (hLOs) are useful for disease modelling and drug screening. However, a lack of immune cells in hLOs limits the recapitulation of in vivo cellular physiology. Here, we generated hLOs containing alveolar macrophage (AMφ)-like cells derived from pluripotent stem cells (PSC). To bridge hLOs with advanced human lung high-resolution X-ray computed tomography (CT), we acquired quantitative micro-CT images. Three hLO types were observed during differentiation. Among them, alveolar hLOs highly expressed not only lung epithelial cell markers but also AMφ-specific markers. Furthermore, CD68+ AMφ-like cells were spatially organized on the luminal epithelial surface of alveolar hLOs. Bleomycin-treated alveolar hLOs showed upregulated expression of fibrosis-related markers and extracellular matrix deposits in the alveolar sacs. Alveolar hLOs also showed structural alterations such as excessive tissue fraction under bleomycin treatment. Therefore, we suggest that micro-CT analyzable PSC-derived alveolar hLOs are a promising in vitro model to predict lung toxicity manifestations, including fibrosis.Social impairments characteristic of autism spectrum disorder (ASD) are evident in early childhood and often worsen. There is a paucity of evidence-based interventions explicitly targeting social skill development for young children with ASD and few actively integrate caregivers. The PEERS® program, an evidence-based caregiver-assisted social skills program, was extended for young children with ASD (i.e., PEERS® for Preschoolers (P4P)). This pilot study expands upon initial results by examining the feasibility of a briefer intervention period and the effectiveness in improving child social skills among 15 children with ASD. Results suggest P4P recruitment, participant retention, and implementation are feasible across clinicians and sites. Further, P4P appears to improve social skills, maintained post-intervention. Future research might examine mechanisms that lead to results.The challenge of defining and cataloging the building blocks of the brain requires a standardized approach to naming neurons and organizing knowledge about their properties. The US Brain Initiative Cell Census Network, Human Cell Atlas, Blue Brain Project, and others are generating vast amounts of data and characterizing large numbers of neurons throughout the nervous system. The neuroscientific literature contains many neuron names (e.g. parvalbumin-positive interneuron or layer 5 pyramidal cell) that are commonly used and generally accepted. However, it is often unclear how such common usage types relate to many evidence-based types that are proposed based on the results of new techniques. Further, comparing different types across labs remains a significant challenge. Here, we propose an interoperable knowledge representation, the Neuron Phenotype Ontology (NPO), that provides a standardized and automatable approach for naming cell types and normalizing their constituent phenotypes using identifiers from community ontologies as a common language. The NPO provides a framework for systematically organizing knowledge about cellular properties and enables interoperability with existing neuron naming schemes. We evaluate the NPO by populating a knowledge base with three independent cortical neuron classifications derived from published data sets that describe neurons according to molecular, morphological, electrophysiological, and synaptic properties. Competency queries to this knowledge base demonstrate that the NPO knowledge model enables interoperability between the three test cases and neuron names commonly used in the literature.Revealing associations among various structural and functional patterns of the brain can yield highly informative results about the healthy and disordered brain. Studies using neuroimaging data have more recently begun to utilize the information within as well as across various functional and anatomical domains (i.e., groups of brain networks). However, most whole-brain approaches assume similar complexity of interactions throughout the brain. Here we investigate the hypothesis that interactions between brain networks capture varying amounts of complexity, and that we can better capture this information by varying the complexity of the model subspace structure based on available training data. To do this, we employ a Bayesian optimization-based framework known as the Tree Parzen Estimator (TPE) to identify, exploit and analyze patterns of variation in the information encoded by temporal information extracted from functional magnetic resonance imaging (fMRI) subdomains of the brain. Using a repeated cross-validation procedure on a schizophrenia classification task, we demonstrate evidence that interactions between specific functional subdomains are better characterized by more sophisticated model architectures compared to less complicated ones required by the others for optimally contributing towards classification and understanding the brain's functional interactions. We show that functional subdomains known to be involved in schizophrenia require more complex architectures to optimally unravel discriminatory information about the disorder. Our study points to the need for adaptive, hierarchical learning frameworks that cater differently to the features from different subdomains, not only for a better prediction but also for enabling the identification of features predicting the outcome of interest.

Return to work (RTW) after cancer can be modulated by psychosocial factors, including a reordering of one's life values, with more emphasis on private life than work-life. This change in patients' outlook on work-life is however poorly understood.

We used data from a French cohort (CANTO, NCT01993498) of women diagnosed with stage I-III primary breast cancer (BC) prospectively assessing life priorities between work and private life at diagnosis and 2years after diagnosis. We identified women who reported a shift in life values toward private life, and we investigated the clinical, demographic, work-related, and psychosocial determinants of this change using logistic regressions.

Overall, 46% (N = 1097) of the women had reordered their life priorities toward private life 2years after diagnosis. The factors positively associated with this shift included being diagnosed with stage III BC, perceiving one's job as not very interesting, being an employee/clerk (vs. executive occupation), perceiving no support from the supervisor at baseline, perceiving negative interferences of cancer in daily life, and perceiving a positive impact from experiencing cancer.
