Of these 37 patients, 8 (21.6%) had a complication by last follow-up (death 2; residual aneurysm 2; parent artery occlusion 4). This resulted in an incidence rate of 0.018 complications per patient at-risk month (95% CI 0.008-0.035). These data suggest that the use of flow-diverting stents for treatment of aneurysms in children may be safe and that FDA approval for this use should be evaluated. Although this analysis is comprised of case reports and case series, it represents the best attempt thus far to quantify the risk of using flow-diverting stents in children.

A 23-yr-old male presented with bitemporal hemianopia due to chaismal apoplexy. Imaging revealed a CM involving the OPH. The CM was resected by an extended endonasal approach. The patient had improvement in his visual field defects, and postoperative magnetic resonance imaging (MRI) revealed a gross total resection of the CM.

In the spine, schwannomas usually arise from spinal nerve roots and are therefore extramedullary in nature. Surgical resection-achieving a gross total resection, is the main treatment modality and is typically curative for patients with sporadic tumors. In this video, we present the case of a 38-yr-old male with worsening left leg radiculopathy, found to have a lumbar schwannoma. Preoperative imaging demonstrated that the tumor was at the level of L4-L5. A laminectomy at this level was performed with gross total resection of the tumor. The key points of the video include use of intraoperative fluoroscopy to confirm surgical level and help plan surgical exposure, use of ultrasound for intradural tumor localization, and advocating for maximum safe resection using neurostimulation. The patient tolerated the surgery well without any complications. He was discharged home with no additional therapy needed. Bottom-up speech analysis is accelerated by top-down predictions about upcoming words. In everyday communications, however, listeners are regularly presented with challenging input-fluctuations of speech rate or semantic content. In this study, we asked how reducing speech temporal regularity affects its processing-parsing, phonological analysis, and ability to generate context-based predictions. To ensure that spoken sentences were natural and approximated semantic constraints of spontaneous speech we built a neural network to select stimuli from large corpora. We analyzed brain activity recorded with magnetoencephalography during sentence listening using evoked responses, speech-to-brain synchronization and representational similarity analysis. For normal speech theta band (6.5-8 Hz) speech-to-brain synchronization was increased and the left fronto-temporal areas generated stronger contextual predictions. The reverse was true for temporally irregular speech-weaker theta synchronization and reduced top-down effects. Interestingly, delta-band (0.5 Hz) speech tracking was greater when contextual/semantic predictions were lower or if speech was temporally jittered. Engineering a GAG-binding surface on the proline-specific mutant enabled it to degrade type-I collagen in the presence of chondroitin-4-sulfate (C4-S). We also present the crystal structures of proline-specific (1.4 Å) and collagen-specific (1.8 Å) mutants.
