Metastatic small bowel adenocarcinoma (SBA) has a poor prognosis. Due to its rarity, high-quality data are lacking to guide treatment. In total, 437 patients who initially presented with or developed metastatic SBA between September 1977 and September 2019 were identified from the MD Anderson Tumor Registry. Clinical data were collected from review of the medical record. Overall response rates (ORR), time to progression (TTP), and overall survival (OS) were assessed across various treatments and treatment lines.
The median OS from diagnosis of metastatic disease was 15.9 months [95% confidence interval (CI) 14.3-17.9]. Seventy-five patients (17.1%) underwent metastasectomy, which was associated with a median OS of 34.5 versus 17.1 months among patients who received chemotherapy alone (P < 0.001). Fluoropyrimidine plus plativity of anti-EGFR therapy was limited.
In well-selected patients with SBA, metastasectomy appears to be associated with improved OS. This improvement was seen across metastasectomy sites, including liver, lung and peritoneal. Anti-programmed cell death protein 1 (PD-1) based immunotherapy was active for dMMR SBA but not pMMR SBA. While taxane-based chemotherapy demonstrates therapeutic activity, the activity of anti-EGFR therapy was limited.On 21 January 2021, the European Commission amended the marketing authorisation granted for pembrolizumab to include the first-line treatment of microsatellite instability-high (MSI-H) or mismatch repair-deficient (dMMR) metastatic colorectal cancer (mCRC) in adults. The recommended dose of pembrolizumab was either 200 mg every 3 weeks or 400 mg every 6 weeks by intravenous infusion. Pembrolizumab was evaluated in a phase III, open-label, multicentre, randomised trial versus standard of care (SOC FOLFOX6/FOLFIRI alone or in combination with bevacizumab/cetuximab) as first-line treatment of locally confirmed mismatch repair-deficient or microsatellite instability-high stage IV CRC. Subjects randomised to the SOC arm had the option to crossover and receive pembrolizumab once disease progressed. Both progression-free survival (PFS) and overall survival were primary endpoints. Pembrolizumab showed a statistically significant improvement in PFS compared with SOC, with a hazard ratio of 0.60 [95% confidence interval (CI) 0.45-0.80], P = 0.0002. Median PFS was 16.5 (95% CI 5.4-32.4) versus 8.2 (95% CI 6.1-10.2) months for the pembrolizumab versus SOC arms, respectively. The most frequent adverse events in patients receiving pembrolizumab were diarrhoea, fatigue, pruritus, nausea, increased aspartate aminotransferase, rash, arthralgia, and hypothyroidism. Having reviewed the data submitted, the European Medicines Agency's (EMA's) Committee for Medicinal Products for Human Use (CHMP) considered that the benefit-risk balance was positive. This is the first time the CHMP has issued an opinion for a target population defined by DNA repair deficiency biomarkers. The role of tumor mutational burden (TMB) is still debated for selecting advanced non-oncogene addicted non-small-cell lung cancer (NSCLC) patients who might benefit from immune checkpoint inhibitors (ICIs). Of note, TMB failed to predict a benefit in overall survival (OS) among such patients.
The purpose of this meta-analysis was to compare efficacy outcomes among first-line immune-oncology (IO) agents versus standard platinum-based chemotherapy (CT) within two subgroups (TMB-low and TMB-high on either tissue or blood). We collected hazard ratios (HRs) to evaluate the association for progression-free survival (PFS) and OS, with the relative 95% confidence intervals (CIs). Risk ratios (RRs) were used as an association measure for objective response rate (ORR).
Eight different cohorts of five randomized controlled phase III studies (3848 patients) were analyzed. In TMB-high patients, IO agents were associated with improved ORR (RRs 1.37, 95% CI 1.13-1.66), PFS (HR 0.69, 95% CI 0.61-0.79) and OS (HR 0.67,PD-L1) expression level, could represent the preferable option for future clinical research in the first-line management of advanced non-oncogene addicted NSCLC patients.Our paper describes and interprets grain microtexture and microstructure collected from periglacial sediments on the Abalakh High-Accumulation Plain (AHAP) in Central Yakutia. This territory occupies the Lena-Amga Rivers interfluve. In borehole 18/1, five sediment Complexes (I-V) of successive environments were recognized 1) alluvial in the base of the borehole-Complex I; 2) alluvial-lake-Complex II; 3) lake-complex-Complex III; 4) ice-complex (yedoma)-Complex IV; and finally 5) a Holocene cover-Complex V. Quartz sand-grain and silt-grain microtextural analysis was undertaken in a scanning electron microscope (SEM) and supplemented by mineralogical analyses to reconstruct the sedimentary-accumulation environment, discern the influence of periglacial conditions on the grains, and identify the sediment source(s) for each complex. Based on the results, a conclusion can be reached that the accumulation of Complex I took place as a result of multiple repetitive transportation events recycling the same material and introducing a limited supply of new material into the fluvial environment. Upward in the succession, fluvial-process activities decreased in favour of lake-deposit accumulation. Frozen syngenetic ice-rich silty deposits-yedoma or ice complex-of Complex IV are composed of grains with a precipitated surface, but differ from the underlying deposits in the degree of crusting and mineralogy. Across go/no-go repetition detection and semantic categorization tasks, targets in related pairs that shared handshape and location elicited smaller N400s than targets in unrelated pairs, indicative of facilitated processing. Handshape-related targets also elicited smaller N400s than unrelated targets, but only in the repetition task. The location priming effect reversed direction across tasks, with slightlylargeramplitude N400s for targets in related versus unrelated pairs in the semantic task, indicative of interference. These patterns imply that handshape and location play different roles during sign recognition and that there is a hierarchical organization for the sign lexicon. Similar to interactive-activation models of word recognition, we argue for differentiation between sublexical facilitation and lexical competition. Lexical competition is primarily driven by the location parameter and is more engaged when identification of single lexico-semantic entries is required.In recent years enterprise imaging (EI) solutions have become a core component of healthcare initiatives, while a simultaneous rise in big data has opened up a number of possibilities in how we can analyze and derive insights from large amounts of medical data. Together they afford us a range of opportunities that can transform healthcare in many fields. This paper provides a review of recent developments in EI and big data in the context of medical physics. It summarizes the key aspects of EI and big data in practice, with discussion and consideration of the steps necessary to implement an EI strategy. It examines the benefits that a healthcare service can achieve through the implementation of an EI solution by looking at it through the lenses of compliance, improving patient care, maximizing revenue, optimizing workflows, and applications of artificial intelligence that support enterprise imaging. It also addresses some of the key challenges in enterprise imaging, with discussion and examples presented for those in systems integration, governance, and data security and privacy.
Lockdowns amid the COVID-19 pandemic have offered a real-world opportunity to better understand air quality responses to previously unseen anthropogenic emission reductions.
This work examines the impact of Vienna's first lockdown on ground-level concentrations of nitrogen dioxide (NO
), ozone (O
) and total oxidant (O
). The analysis runs over January to September 2020 and considers business as usual scenarios created with machine learning models to provide a baseline for robustly diagnosing lockdown-related air quality changes. Models were also developed to normalise the air pollutant time series, enabling facilitated intervention assessment.
NO
concentrations were on average-20.1% [13.7-30.4%] lower during the lockdown. However, this benefit was offset by amplified O
pollution of+8.5% [3.7-11.0%] in the same period. The consistency in the direction of change indicates that the NO
reductions and O
increases were ubiquitous over Vienna. O
concentrations increased slightly by+4.3% [1.8-6.4%As heavy-duty vehicles can make up a large fraction of the fleet emissions of nitrogen oxides, the change in the volume of these vehicles on the roads may be the main driver to explain the change in NO
concentrations.
A probable future with emissions of volatile organic compounds (VOCs) dropping slower than emissions of nitrogen oxides could risk worsened urban O
pollution under a VOC-limited photochemical regime. More holistic policies will be needed to achieve improved air quality levels across different regions and criteria pollutants.
