Sodium-glucose cotransporter-2 (SGLT2) inhibitors are new antidiabetic drugs that reduce hyperglycemia by inhibiting the glucose reabsorption in renal proximal tubules. Clinical studies have shown that SGLT2 inhibitors not only improve glycemic control but also reduce major adverse cardiovascular events (MACE, cardiovascular and total mortality, fatal or nonfatal myocardial infarction or stroke) and hospitalization for heart failure (HF), and improve outcome in chronic kidney disease. These cardiovascular and renal benefits have now been confirmed in both diabetes and non-diabetes patients. The precise mechanism(s) responsible for the protective effects are under intensive investigation. This review examines current evidence on the cardiovascular benefits of SGLT2 inhibitors, with a special emphasis on the vascular actions and their potential mechanisms.

The objective of this study was to estimate the lifetime costs of patients receiving treatment for follicular lymphoma (FL) in the United States.

A Markov model was programmed in hēRo3 with a 6-month cycle length, 35-year time horizon (lifetime projection), and health states for line of treatment, response, receipt of maintenance therapy among responders, transformation to diffuse large B-cell lymphoma (DLBCL), development of second primary malignancy (SPM), and death. The model was used to estimate the expected lifetime costs of FL (in 2019 USD), including costs of drug acquisition and administration, transplant procedures, radiotherapy, adverse events, follow-up, DLBCL, SPM, end-of-life care, and indirect costs. Model inputs were based on published sources.

In the US, patients with FL receiving treatment have a life expectancy of approximately 14.5 years from initiation of treatment and expected lifetime direct and indirect costs of US$515,884. Costs of drugs for induction therapy represent the largesenefits that might be achieved by treatments for FL that prevent or delay disease progression.

The aim of this study was to evaluate the diagnostic accuracy of the 1mg dexamethasone suppression test (DST) for the prediction of autonomous cortisol secretion (ACS)-related comorbidities in patients with adrenal incidentalomas (AIs).

This was a retrospective multicenter study. We recruited patients with AI/s ≥ 1cm, excluding those who, during the study, were found during theextensionstudy of an extra-adrenal cancer, with a known diagnosis of hereditary syndromes characterized by adrenal tumors, those presenting with overt hormonal excess syndromes, and those in whom the DST results were missing.

A total of 823 patients met the inclusion criteria. Based on the 1.8, 3.0, and 5.0µg/dl post-DST cortisol thresholds, the prevalence of ACS was 33.5%, 13.7%, and 5.6%, respectively. The prevalence of hypertension (OR = 1.8, 95% CI = 1.3-2.4), diabetes (OR = 1.6, 95% CI = 1.2-2.2), and dyslipidemia (OR = 1.4, 95% CI = 1.0-1.9) was higher with cortisol post-DST ≥ 1.8µg/dl; the prevalence of hypertension (OR = 2e DST. Other clinical, metabolic, or imaging markers showing a better performance for the prediction of the development and progression of cardiometabolic comorbidities in AIs need to be identified.Immunodeficient patients are susceptible to systemic fungal infections; however, these rarely cause secondary peritonitis. A 66-year-old man with multiple myeloma and diabetes mellitus on continuous ambulatory peritoneal dialysis (CAPD) presented with cloudy ascitic fluid. He had been treated with corticosteroids for 1 month for Tolosa-Hunt syndrome. We diagnosed peritoneal dialysis-related peritonitis caused by Enterococcus avium, removed the CAPD catheter, and initiated intravenous ampicillin. Computed tomography (CT) revealed an intramural gastric mass and a thinning ascending colon wall. Four days later, follow-up contrast-enhanced CT showed penetration of the ascending colon and rupture of the ileocolic artery. Emergency open surgery revealed hemorrhagic infarction with mucormycosis. We initiated intravenous liposomal amphotericin B 20 days after admission; however, he died 55 days later. Anatomical abnormalities, such as gastrointestinal perforation, should be considered for peritonitis in immunodeficient patients. Gastrointestinal mucormycosis is rare but fatal, resulting from a delay in diagnosis and consequent gastrointestinal perforation. For an early diagnosis and a favorable clinical outcome, it is important to consider the risk factors for mucormycosis, including corticosteroid use, diabetes, end-stage kidney diseases.Nitrate (NO3-) contamination has become a dominant international problem in the aquatic environment, so identifying the sources and transformations of NO3- is the basis for improving water quality. Since the Jing River is the largest tributary of the Wei River, to understand its water quality, this study collected surface water samples from the Shaanxi section of the Jing River during the dry season. The potential sources of NO3- were analyzed by hydrochemical and bi-isotopic methods, and the SIAR model was used to estimate the proportional contribution of each source. Results indicated that NO3--N was the main form of inorganic nitrogen in this area, and the average total nitrogen content was 10.23 mg·L-1, which showed that nitrogen pollution was highly serious; the transformation process of nitrogen in this study area was mainly nitrification; The results of Bayesian model showed that manure and sewage contributed to the most NO3- (64.39%) in the dry season, followed by soil nitrogen, which was 26.35%. These results help to adopt better nitrogen management measures to meet the national environmental quality standards for surface water.Pulmonary rehabilitation is essential in post-COVID subjects, reporting respiratory impairment after the discharge from the hospital. Because the number of patients with respiratory outcomes is high and there are few facilities available, we wonder if a spa setting could represent a valid out-of-hospital alternative. We aim to explore recent evidence related to respiratory rehabilitation in the spa environment to understand if it can represent an appropriate setting for respiratory rehabilitation interventions in post-COVID subjects. Studies were found by screening PubMed, MEDLINE, and Google Scholar databases from 2011 up to February 2021. Studies were eligible if they were reviews, randomized controlled trials (RCTs), or clinical trials, investigating respiratory interventions in the spa environment. Recent evidence has shown that inhalations and mineral-rich water immersions are effective in fighting and preventing multiple chronic respiratory tract diseases. Therefore, these treatments could also be applied to post-COVID patients with medium long-term respiratory outcomes.Animal studies suggested that exposure to ethylene oxide (EO) might induce hepatic lipid peroxidation and inflammatory lesions in various organs. However, the association between EO exposure and diabetes risk in humans is unknown. This study aimed to examine the association of EO exposure with the prevalence of diabetes mellitus in a general population of US adults. This study consisted of 3448 participants aged 20 years and older from the National Health and Nutrition Examination Survey (NHANES) 2013-2014 cycle and 2015-2016 cycle. Circulating levels of EO biomarker (hemoglobin adducts of EO (HbEO)) was measured by using high-performance liquid chromatography coupled with tandem mass spectrometry. Odds ratios (ORs) and 95% confidence intervals (CIs) were estimated using multivariate logistic regression. The weighted median level of HbEO was 29.9 (95% CI 21.8, 56.0) pmol/g Hb. Elevated levels of HbEO were associated with higher HbA1c and lower high-density lipoprotein cholesterol (both Ptrend less then 0.01). After multivariate adjustment including demographics, lifestyle factors, and body mass index (BMI), higher HbEO levels were significantly associated with an increased prevalence of diabetes mellitus. The OR (95% CI) of diabetes across increasing quartiles of HbEO was 1.00 (reference), 1.45 (1.08, 1.96), 1.76 (1.31, 2.36), and 1.77 (1.22, 2.57), respectively (Ptrend less then 0.001). Similar results were observed when analyses were stratified by smoking status, age, sex, race/ethnicity, and BMI. In a nationally representative sample of US adults, higher levels of HbEO were significantly associated with an increased prevalence of diabetes mellitus. Further prospective studies are needed to confirm these findings. This study investigated the effectiveness of bio-augmenting aerobic cell culture to mitigate ammonia and hydrogen sulfide emission in sewage sludge composting amended with reed straw (with the weight ratio of 10.3-0.4). During the 20-day aerated lab-scale composting, adding 200-mL culture (56.80 NTU) reduced ammonia and hydrogen sulfide emissions by 38.00% and 54.32%, and conserved total nitrogen and sulfate by 39.42% and 70.75%, respectively. Organic matters degradation was quick started 1 day ahead. Comparing to the control, nitrate content increased 38.75% at the end of the compost. Bioaugmentation evened the distributions of bacterial communities in the thermophilic phase. The shift was mainly due to 22.97% of relative abundance of Proteobacteria depressed and 157.16% of Bacteroidetes increased, which were beneficial for nitrogen conservation and glycan breakdown, respectively. In summary, the results demonstrated that bioaugmentation addition could be an effective strategy for enhanced sludge composting.Severe haze frequently hits the North China Plain (NCP), especially in winter during recent years. Linsitinib solubility dmso Meteorological factors affect aerosol formation and its optical properties, and accurate meteorological fields are imperative for accurate aerosol simulations. The impacts of Global Positioning System Zenith Total Delay (GPS-ZTD) data assimilation on meteorology and aerosol simulations were evaluated in this study using the WRF-CMAQ (the Weather Research and Forecasting model and Community Multiscale Air Quality) modelling system over the NCP during 01-31 December 2019. After bias correction, GSP-ZTD data were assimilated into the WRF model using the 3DVAR technique. Two sensitivity tests (CTR and ZTD) were conducted. The WRF model had generally acceptable performance for surface and upper air meteorological variables, PM2.5 and visibility. From the aspect of BIAS, STDE, RMSE, and R, the assimilation of ZTD data improved the underestimation of ground relative humidity (RH). The improvement was more pronounced inecific stations can be great. The improvement of PM2.5 troughs was in the range of 1-5 μg/m3, while the overestimation of PM2.5 peaks was reduced by few up to dozens μg/m3. This will contribute to the extreme value prediction during pollution episode. The TransMilenio (TM) is a transport system. Twenty-year-old TM is a fast, highly efficient, and self-sufficient mode of passenger transport. This work aims to evaluate the effects of changing current TM diesel buses by electricity-powered buses (battery, wire-based), on the PM2.5 concentrations at surface level. Emissions calculations considering combustions and resuspension of TM and Non-TM were performed. A CFD model was implemented to estimate current PM2.5 concentrations at the roadside level, and the CFD results were validated using the statistic parameters MB, RMSE, r, and IOA. Results from the emission calculations indicate that TM buses (30-50%) are one of the main sources of primary PM2.5 in all the considered urban sites in this study. Non-exhaust emissions from most vehicle categories were also identified as an important source of primary PM2.5 (40% of total emissions). The CFD model reproduced closely the trends and levels of PM2.5 concentrations measured at the roadside level in all the locations.
