All cases were performed in the office under local anesthesia (LA) without sedation. The majority received no prophylactic antibiotics. Materials and Methods We retrospectively reviewed the electronic medical records of 242 consecutive fTP-Bx cases using the PrecisionPoint™ Transperineal Access System (PPTAS) performed under LA without sedation by a single urologist (R.J.S.) at Kaiser Permanente Southern California. We compared complication rates of this series of cases to our initial series of 62 fTP-Bx cases without the PrecisionPoint as well as 133 transrectal prostate biopsy cases performed before we adopted the transperineal approach. Results Of the 242 consecutive free-hand transperineal biopsies performed between August 26, 2016 and December 31, 2018, 212 (88%) received no antibiotic prophylaxis. Medians for age, prostate-specific antigen, prostate volume, prostate-specific antigen density, and cores sampled were 63 years, 7.2 ng/mL, 50 mL, 0.15 ng/mL/cc, and 20 cores, respectively. In this study, we examined the potential of transferring a recombinant adeno-associated virus (rAAV) vector carrying a sequence for the highly chondroregenerative transforming growth factor beta (TGF-β), using poly(ɛ-caprolactone) (PCL) films functionalized by the grafting of poly(sodium styrene sulfonate) (pNaSS) in chondrogenically competent bone marrow aspirates as future targets for therapy in cartilage lesions. Effective overexpression of TGF-β in the aspirates by rAAV was achieved upon delivery using pNaSS-grafted and ungrafted PCL films for up to 21 days (the longest time point evaluated), with superior levels using the grafted films, compared with respective conditions without vector coating. The production of rAAV-mediated TGF-β by pNaSS-grafted and ungrafted PCL films significantly triggered the biological activities and chondrogenic processes in the samples (proteoglycan and type-II collagen deposition and cell proliferation), while containing premature mineralization and hypertrophy relative to the other conditions, with overall superior effects supported by the pNaSS-grafted films. These observations demonstrate the potential of PCL film-assisted rAAV TGF-β gene transfer as a convenient, off-the-shelf technique to enhance the reparative potential of the bone marrow in patients in future approaches for improved cartilage repair.Objective To investigate the efficiency of three advanced lithotripters during the treatment of artificial stones (hard and soft) in an experimental in vitro and in vivo setting. Materials and Methods An experimental configuration was created to treat artificial stones, made from BegoStone Plus in a weight ratio of 153 or 156 powder to water, replicating hard and soft urinary stones, respectively. Experimental data of three lithotripters, the Lithoclast Master, the Lithoclast Trilogy, and the ShockPulse-SE, were assessed. Experiments aimed at comparing the stone clearance times (SCTs) of the devices as well as different probe diameters, probe disposability, and hardness of the stones. The in vivo experiment was performed in a porcine model after the approval from the State Services. Before in vivo lithotripsy, stones had been placed through percutaneous access established by using the Bull's eye technique. The SCT between the Lithoclast Trilogy and the ShockPulse-SE was compared. Results All the devices were statistically more efficient in the treatment of soft stones in both in vitro (p less then 0.001) and in vivo settings (p less then 0.008). The use of larger-diameter probes resulted in shorter SCT (p less then 0.007). The disposable probes of the ShockPulse-SE showed higher performance than the multiple-use probes (p less then 0.05). link2 In the direct comparison between the devices, the Lithoclast Trilogy was found to be the fastest during all the in vitro (p less then 0.001) and in vivo (p less then 0.008) trials. link2 Conclusion Modern dual-energy lithotripters proved to possess high performance in the treatment of stones. Faster SCTs were observed by using the larger-diameter and disposable probes. The Lithoclast Trilogy was superior in terms of SCT over all the experimental settings compared with other lithotripters.La Promoción de la Salud (PS) es una función esencial de la salud pública que se ha puesto en tensión frente a la pandemia de la COVID-19, dado que los discursos y estrategias basados en la prevención y curación de la enfermedad han invisibilizado las condiciones de vida e inequidad que son centrales para la PS. La salud colectiva latinoamericana plantea cuestionamientos prácticos y epistemológicos sobre las acciones ante la epidemia en los países del Sur Global, proponiendo enfoques alternativos al paradigma biomédico y a lo que este entiende como PS. Desde la salud colectiva, la PS tiene como elementos centrales la autonomía de las comunidades, la importancia de sus saberes, y el fomento de acciones colectivas territoriales. Este artículo, a través de una cronología crítica de la pandemia en dos territorios, describe situaciones documentadas que ponen en evidencia el despliegue de condiciones esenciales de posibilidad para una PS desde la salud colectiva, es decir, el papel de la justicia social en la organización comunitaria, la perspectiva territorial y los procesos emancipatorios y de construcción de autonomía. link3 Los casos analizados corresponden a un territorio insular del sur de Chile y a una comuna urbana de la capital del país, elaborados mediante el involucramiento directo de los investigadores y una revisión documental y de prensa. link3 Sus antecedentes y contextualización evidencian las modalidades concretas que toma la PS durante la pandemia en dos contextos con características diversas, permitiendo identificar desafíos y arribar a aprendizajes iniciales sobre el desarrollo de una PS territorialmente situada.Tissue engineering (TE) studies for Achilles tendon (AT) defects are a difficult and popular field in orthopedic medical practice. In this study, we applied electrohydrodynamic three-dimensional (3D) printing technology to construct scaffolds made of poly-(ɛ-ɛ-caprolactone) (PCL) and Pluronic F127 (F127) with different mass-volume ratios. The fibers and porous capabilities of the scaffolds were controlled using this technology. We found that F127 improved the hydrophilicity and degradation of PCL in vitro. The PCL scaffolds with 5% F127 were mostly favorable for cell adhesion and growth, suggesting that the scaffolds had good biocompatibility in vitro. Scaffolds with 5% F127 seeded with C3H10T1/2 cells were transplanted into AT defects in rats. A histological analysis indicated that the TE scaffolds were beneficial for the accumulation and arrangement of collagen fibers. Thus, this study provides fundamental experimental data for future clinical applications regarding TE for ATs.
