Microbiological surveillance of bronchoscopes and automatic endoscope reprocessors (AERs)/washer disinfectors as a quality control measure is controversial. Experts also are divided on the infection risks associated with bronchoscopic procedures.
We evaluated the impact of routine microbiological surveillance and audits of cleaning/disinfection practices on contamination rates of reprocessed bronchoscopes.
Audits were conducted of reprocessing procedures and microbiological surveillance on all flexible bronchoscopes used from January 2007 to June 2020 at a teaching hospital in France. Contamination rates per year were calculated and analyzed using a Poisson regression model. The risk factors for microbiological contamination were analyzed using a multivariable logistical regression model.
In total, 478 microbiological tests were conducted on 91 different bronchoscopes and 57 on AERs. The rate of bronchoscope contamination significantly decreased between 2007 and 2020, varying from 30.2 to 0% (P < .n.This study investigated the association between the different types of plant-based diets and dyslipidemia in Korean adults using data from the nationally representative sample. Using the 2012-2016 Korea National Health and Nutrition Survey data, a total of 14,167 adults (≥19 years old) participated in this study. Dietary intake was assessed by a semi-quantitative food frequency questionnaire. Three different plant-based diet indices (overall plant-based diet index (PDI), healthful plant-based diet index (hPDI), unhealthful plant-based diet index (uPDI)), were calculated. Dyslipidemia and its components (hypertriglyceridemia, hypercholesterolemia, low high-density lipoprotein cholesterol (HDL-C), high low-density lipoprotein cholesterol (LDL-C), use of anti-hyperlipidemia agent) were measured. Multivariable logistic regression analysis was used to examine the associations between plant-based diet and dyslipidemia and individual lipid disorders. Totally, 47% of overall population had dyslipidemia. Individual in the highest quintile of uPDI had 22% greater odds of dyslipidemia (95% CI 1·05, 1·41) and 48 % higher odds of hypertriglyceridemia (95% CI 1·21, 1·81) and 16% higher odds of low HDL-C (OR 1·16, 95% CI 1·00, 1·35) than those in the lowest quintile of uPDI. PDI was associated with 16 % higher odds of low HDL-C and hPDI were associated with 25% lower odds of high LDL-C. However, Neither PDI nor hPDI was significantly associated with the prevalence of dyslipidemia. Greater adherence to unhealthful plant-based diets was associated with greater odds of the dyslipidemia and its components suggesting the importance of the quality of plant-based diet in South Korean adults for dyslipidemia prevention.This study determined the validity, reproducibility and usability of a smartphone app - APPetite - for the measure of free-living, subjective appetite. Validity was assessed compared with the criterion tool of pen-and-paper visual analogue scale (VAS) (n=22). Appetite was recorded using APPetite and VAS, one immediately after the other, upon waking and every hour thereafter for twelve hours. This was repeated the next day with the order of tool reversed. Agreement between tools was assessed using Bland-Altman analysis. Reproducibility and usability were assessed in a separate experiment (n=22) of two trials (APPetite vs. VAS), separated by seven days. Appetite was recorded in duplicate upon waking and every hour for twelve hours using APPetite or VAS. Agreement between duplicate measures was assessed using Bland-Altman analysis and coefficient of variation (CV) was compared between tools. Usability was assessed by comparing compliance and by qualitative evaluation. APPetite demonstrated good criterion validity with trivial bias of 1.65 units/mm·hr-1 between APPetite- and VAS-derived AUC appetite scores. Limits of agreement were within a maximum allowed difference of 10%. However, proportional bias was observed. APPetite demonstrated high reproducibility, with minimal bias (-0.578 units·hr-1) and no difference in CV between APPetite and VAS (1.29±1.42% vs 1.54±2.36%, p = 0.64). Compliance was high with APPetite (92.7±8.0%) and VAS (91.6±20.4%, p = 0.81). Ninety percent of participants preferred APPetite, citing greater accessibility, simplified process and easier/quicker use. While proportional bias precludes using APPetite and VAS interchangeably, APPetite appears a valid, reproducible and highly usable tool for measuring free-living appetite in young-to-middle-aged adults.Frailty, a multifactorial ageing-related syndrome characterized by reduced resistance to stressors and possibly associated with low-grade systemic inflammation (LGSI), results in negative health outcomes and compromises healthy ageing. There is a growing body of evidence on the relationship between dietary habits, LGSI and the risk of frailty. Consumption of dietary ultra-processed products (UPP) could negatively contribute to these conditions. In this article, we intend to (i) discuss the role that ultra-processed food products (UPP) might have on the development of frailty considering the inflammatory potential of this type of food; and (ii) to raise awareness on deleterious effects of excess UPP intake in development of adverse health outcomes, in particular, frailty and compromised healthy ageing. UPP are industrial formulations whose nutrient profile has been associated with inflammation and altered gut microbiota. Besides, diets with a greater presence of unprocessed foods and antioxidants have been linked to the reduction of oxidative stress and the expression of inflammatory biomarkers. Because inflammation is believed to be a contributing factor in the development of frailty, it is possible that UPP would contribute to the onset or increase of this condition. Importantly, the increasing consumption of UPP in younger populations might pose a greater risk to the development of compromised healthy ageing in the long term.The prevalence of malnutrition is high among oncology patients in Northern China. Malnutrition is related to the longer hospital stay, and it can be used to predict the prognostic outcome of patients. This work focused on investigating the relationship of nutritional condition with the length of hospital stay (LOS) in Northern Chinese patients with lung adenocarcinoma. The PG-SGA, Nutritional Risk Screening 2002 (NRS 2002) score, recent weight loss and BMI were assessed in a probabilistic sample of 389 lung adenocarcinoma patients without EGFR mutations. This study collected the demographic and clinical features of patients in a prospective manner. Then, we examined the association of nutritional status with LOS among the population developing lung adenocarcinoma. According to the PG-SGA, 63 (16.3%), 174 (44.7%) and 78 (20.1%) patients were at risk for undernutrition, moderate undernutrition and severe undernutrition, respectively. Nutritional risk was found in 141 (36.2%) patients based on the NRS 2002. The average LOS for tumor patients in Northern China was 12.5 days. At admission, a risk of undernutrition or undernutrition according to the PG-SGA (P less then 0.001), NRS 2002 (P less then 0.001), and latest weight loss (P less then 0.001) predicted the longer LOS. Length of stay was related to nutritional status and hospitalization expenses (P less then 0.001). Lung adenocarcinoma patients who stayed in the ICU had a poorer nutritional status and a longer LOS (P less then 0.001). In Northern Chinese patients with lung adenocarcinoma, a risk for undernutrition evaluated by the PG-SGA, the NRS2002 and recent weight loss, but not BMI, could predict a longer LOS.Dystrophic neuronal processes harboring neuritic plaque (NP) tau pathology are found in association with Aβ plaques in Alzheimer's disease (AD) brain. Microglia are also in proximity to these plaques and microglial gene variants are known risk factors in AD, including loss-of-function variants of TREM2. We have further investigated the role of Aβ plaque-associated microglia in 5XFAD mice in which NP tau pathology forms after intracerebral injection of AD brain-derived pathologic tau (AD-tau), focusing on the consequences of reduced TREM2 expression and microglial depletion after treatment with the colony-stimulating factor 1 (CSFR1) inhibitor, PLX3397. Young 5XFAD mice treated with PLX3397 had a large reduction of brain microglia, including cortical plaque-associated microglia, with a significant reduction of Aβ plaque burden in the cortex. A corresponding decrease in cortical APP-positive dystrophic processes and NP tau pathology were observed after intracerebral AD-tau injection in the PLX3397-treated 5XFAD2+/- and TREM2-/- mice suggest that the former may better model the single copy TREM2 variants associated with AD risk.Understanding of the value of patient and public involvement in research has grown in recent years, but so too has uncertainty about how best to practice and how best to report such involvement in research outputs. One way proposed to report such involvement is through checklists, such as the GRIPP2, which aims to improve quality, transparency, and consistency in such reporting. We critique the unproblematised use of such a tool because of two main concerns. First, being asked to complete a GRIPP2 for a recent publication felt divisive given that the service user researcher was as much a member of the authorship team as the other researchers (whose involvement did not necessitate a checklist). Second, checklists do not actually address the power imbalances and tokenism that is rife in patient and public involvement in research. Indeed, the false sense of objectivity fostered by meeting the minimum requirements of the checklist means that researchers may not go further to engage in reflexive research practices and reporting. Rather than rote use of such checklists, we recommend mindful reflexive reporting in research outputs of patient and public involvement processes. We also recommend future iterations of the GRIPP consider (a) incorporating criteria about whether the checklist is completed by or with service user researchers or not, (b) addressing criteria that position service user research as needing to be justified, and (c) expanding the "critical perspective" element of the checklist to explicitly consider power differentials.
The liver micronucleus (MN) assay is an effective and important in vivo test for detecting genotoxic compounds. In particular, the repeated-dose liver MN (RDLMN) assay which greatly facilitates incorporation of the liver MN assay into the general toxicity study has been developed. Usefulness of the RDLMN assay was appraised highly in the 7th International Workshops on Genotoxicity Testing (2017 in Tokyo) in that sufficient numbers and types of chemicals were studied and easy integration into the general toxicity study is preferred from the 3R's point of view. However, it was pointed out that it is necessary to evaluate the effect of age at the start of 4-week repeated administration, since there are limited data, where only those of rats of 6 week of age at the start of administration are available. In this study, we conducted the 4-week RDLMN assay using rats of 6 and 8 weeks of age (at the start of administration) to investigate the effect of age on the liver MN inducibility. 4-PBA order Clofibrate, a weak inducer of liver MN, was used in this study to detect the slight difference in the liver MN induction.
