OBJECTIVE We evaluated the characteristics of patients with diverticular bleeding in whom emergency endoscopy should be proactively performed and those in whom it is unnecessary for spontaneous hemostasis following conservative treatment. METHODS This study involved 132 patients in whom diverticular bleeding was diagnosed on lower gastrointestinal endoscopy. We evaluated the rate of identification of the bleeding diverticulum during endoscopy and the rate of spontaneous hemostasis following conservative treatment. RESULTS In 26 patients (20%), bleeding diverticulum was identified during endoscopy. Extravasation or fluid collection on CT imaging was an important factor of successful identification of the bleeding source on endoscopy. Of the 104 patients in the conservative treatment group, 91 (87%) were able to be discharged after spontaneous hemostasis. Univariate analysis revealed a high rate of spontaneous hemostasis in patients without extravasation and fluid collection on CT imaging, those without adhesiothe type of cases wherein urgent endoscopy is effective and the type wherein it is unnecessary need to be clarified. Thus far, there have been very few reports of the characteristics of patients with diverticular bleeding in whom spontaneous hemostasis was achieved. We aimed to assess the characteristics of patients in whom emergency endoscopy should be proactively performed and those for whom it is unnecessary. Thus, we retrospectively analyzed the identification rate for the responsible diverticulum in patients with diverticular bleeding and the rate of spontaneous hemostasis following conservative treatment.The target species (Aphia minuta and Pseudaphya ferreri) of the transparent goby fishery on Mallorca Island (Balearic Islands, western Mediterranean) are currently discarded when the maximum daily catch is exceeded or when the sorting process is unworkable. The mortality suffered by this discarded fraction remains unknown, although it may be important for resource management. Accordingly, the aims of this study were to (1) assess the environmental drivers of the relative post-release survival of the discarded target species and (2) test the correlations between post-release survival and two behavior-related variables (swimming speed and its increase after a stimulus, assessed via video recording). To do so, mortality of the target species from 47 hauls sampled under normal fishing boat operations was monitored when the gear was onboard and after a few hours. At the reference level (an average depth of 25.7 m and temperature of 15.7 ºC), the immediate survival was 99.9% (95% CI 97.9 to 100%), but the estimated post-release survival decreased to 47.2% (33.8 to 65.8%). Relative post-release mortality doubled when the water temperature increased by 2.8 ºC or when the fishing depth increased up to 32 m. Furthermore, the swimming speed of the target species was significantly correlated with the estimated post-release survival; thus, this vitality metric may offer a promising strategy for more easily estimating post-release mortality in other fisheries.Accurate segmentation of myocardial in cardiac MRI (magnetic resonance image) is key to effective rapid diagnosis and quantitative pathology analysis. However, a low-quality CMR (cardiac magnetic resonance) image with a large amount of noise makes it extremely difficult to accurately and quickly manually segment the myocardial. In this paper, we propose a method for CMR segmentation based on U-Net and combined with image sequence information. The method can effectively segment from the top slice to the bottom slice of the CMR. During training, each input slice depends on the slice below it. In other words, the predicted segmentation result depends on the existing segmentation label of the previous slice. 3D sequence information is fully utilized. Our method was validated on the ACDC dataset, which included CMR images of 100 patients (1700 2D MRI). Experimental results show that our method can segment the myocardial quickly and efficiently and is better than the current state-of-the-art methods. Aside from extreme contrasts in body size, these breeds originate from diverse global locations and each possess uniqupinnings of diverse phenotypes within the horse.BACKGROUND Maternal HIV coinfection is a key factor for mother-to-child transmission (MTCT) of HCV. However, data about HCV MTCT in HIV/HCV-coinfected pregnant women on combined antiretroviral treatment (ART) are scarce. This study assessed the HCV MTCT rate in the Madrid Cohort of HIV-infected women. METHODS Retrospective study within the Madrid Cohort of HIV-infected pregnant women (2000-2012). Epidemiological, clinical and treatment related variables were analysed for the mother and infant pairs. HCV MTCT rate was determined. RESULTS Three hundred thirty-nine HIV/HCV-coinfected women and their exposed infants were recorded. A total of 227 (67%) paired mother-children had available data of HCV follow-up and were included for the analysis. Sixteen children (rate 7.0%, 95%CI 3.7-10.4%) were HCV infected by 18 months of age, none of them coinfected with HIV. HIV/HCV-coinfected pregnant women were mostly of Spanish origin with a background of previous injection drug use. HCV-genotype 1 was predominant. The characteristics of mothers that transmitted HCV were similar to those that did not transmit HCV with respect to sociodemographic and clinical features.
