Blood pressure variability (BPV), a modifiable risk factor, can compromise cerebral perfusion in critically ill patients. We studied the association between BPV in the intensive care unit (ICU) and short- and long-term cognitive outcomes.
All patients were ≥50 years old. The short-term cognitive end points were delirium and depressed alertness without delirium. The long-term outcome was change in the slope of longitudinal cognitive scores. Primary BPV measure was average real variability (ARV) of systolic blood pressure. Associations were assessed with multivariable multinominal logistic regression and linear mixed effects models.
Of 794 patients (1130 admissions) 185 developed delirium and 274 developed depressed alertness. There was a dose-response association of 24-h systolic ARV with delirium (adjusted OR, 95% CI 2.15 per 5 mm Hg increase, 1.31-3.06, P < 0.017) and with depressed alertness (OR 1.89, 95% CI 1.18-3.03, P < 0.008). For 371 patients with available longitudinal cognitive scores, the decline in cognitive trajectory was accelerated after discharge (annual change OR -0.097, 95% CI -0.122 to -0.073). This acceleration increased with delirium (additional decline -0.132 [-0.233 to 0.030], P = 0.011). We found no significant association between BPV and post-ICU cognitive trajectory.
BPV was associated with increased likelihood of delirium in the ICU. Delirium, but not BPV, was associated with long-term cognitive decline.

The resource use of cardiac surgery and neurosurgery patients likely differ from other ICU patients. We evaluated the relevance of these patient groups on overall ICU resource use.
Secondary analysis of 69,862 patients in 17 ICUs in Finland, Estonia, and Switzerland in 2015-2017. Direct costs of care were allocated to patients using daily Therapeutic Intervention Scoring System (TISS) scores and ICU length of stay (LOS). The ratios of observed to severity-adjusted expected resource use (standardized resource use ratios; SRURs), direct costs and outcomes were assessed before and after excluding cardiac surgery or cardiac and neurosurgery.
Cardiac surgery and neurosurgery, performed only in university hospitals, represented 22% of all ICU admissions and 15-19% of direct costs. Cardiac surgery and neurosurgery were excluded with no consistent effect on SRURs in the whole cohort, regardless of cost separation method. Excluding cardiac surgery or cardiac surgery plus neurosurgery had highly variable effects on SRURs of individual university ICUs, whereas the non-university ICU SRURs decreased.
Cardiac and neurosurgery have major effects on the cost structure of multidisciplinary ICUs. Extending SRUR analysis to patient subpopulations facilitates comparison of resource use between ICUs and may help to optimize resource allocation.

Extending SRUR analysis to patient subpopulations facilitates comparison of resource use between ICUs and may help to optimize resource allocation.Commonly known as "Kissing Spines" (KS), the pathological mechanisms underlying impingement and overriding of spinous processes (ORSPs) in horses are poorly understood. Thoroughbreds, Warmbloods, and stock-type breeds, including Paint Horses and Quarter Horses are at increased risk for developing clinical signs of KS. A total of 155 stock-type and Warmblood horses presented at collaborating veterinary clinics and hospitals were examined using a strict clinical and radiographical phenotyping scheme to grade each horse from 0 for unaffected controls to 4 for severe KS. Following genotyping with the Illumina Equine SNP70 array (Illumina, Inc.) a Genome Wide Association Study (GWAS) using 61,229 filtered individual Single Nucleotide Polymorphisms (SNPs) was performed to the KS grade phenotype. Two significantly associated SNPs (BIEC2-668062 and BIEC2-668013) on chromosome 25 defined a ~1.4 Gb candidate region containing approximately 17 coding genes (EquCab3) and 195 ENSEMBL annotated variants. Investigation of the best associated SNP (BIEC2-668062) on chr25 demonstrates a significant correlation with an increase in one KS grade, on average, per A allele in this population. A significant effect of breed group, age, height or sex was not observed in this population. These preliminary results demonstrate the potential for KS diagnosis and preventative measures for WB/ST individuals supported by increased genetic risk for more severe KS grade. We propose further research including other affected breeds and evaluating causative variants, as well as the effect of BIEC2-668062 in these populations.This study set out to examine the effects of fermented distillers grains with solubles (DDGS) partially replaced with soybean meal on growth performance, some blood parameters, meat quality, intestinal microflora, and immune response in broilers. A total of 504 chicks were randomly allocated into 6 groups with 3 replicates. All chicks were fed with one of the following formulated diets i) basal diet based on the maize-soybean meal (C), ii) partially replaced with non-fermented DDGS (NC), iii) partially replaced fermented DDGS with B. read more subtilis (BS), iv) partially replaced with BS + multienzyme (BSE), v) partially replaced fermented DDGS with S. cerevisiae (SC), vi) partially replaced with SC + multienzyme (SCE). Results showed no significant difference between groups for body weight, daily weight gain (DWG), and feed intake (FI) (P > 0.05). However, feed conversion ratios (FCR) of BS, BSE, and SCE groups were significantly lower than the C and NC groups (P less then 0.001). Albumin, total protein, alanine aminotransferase (P less then 0.01), Total antioxidant status, aspartate aminotransferase, high-density lipoprotein, low-density lipoprotein, and uric acid (P less then 0.05) were significantly affected by treatments. The meat color of the SC and SCE groups was darker after 24 h compared to the C group (P less then 0.01). The highest LAB counts of ileal and cecum were observed in the BSE and SCE groups (P less then 0.001). These results suggest that partial replacement of soybean meal with fermented DDGS had a positive effect on FCR without affecting DWG and FI, and combining fermented DDGS with multienzymes decreased FCR and improved immune and gut health status.Bovine infectious infertility represents a problem due to the high impact on animal production and, in many cases, in public health. A lack of information on the characteristics of the bacterial population of the bovine reproductive system can hamper a comprehensive understanding of reproductive pathologies and the role that the microbiome could play. A metagenomic study based on the V3-V4 hypervariable region of the bacterial 16S rRNA gene was performed in 1029 preputial samples from bulls raised in an extensive regimen in Spain (944 from herds with low fertility rates -case group-, and 85 samples from reproductively healthy herds -control group-). The most representative phyla as well as the most 10 abundant bacterial families and their abundance did not show significant differences in both case and control groups. Similarly, the (alpha and beta) diversity of the bacterial populations was similar in both type of herds the Shannon and Simpson indices show a high diversity of species, while the Bray-Curtis dissimilarity index did not show relevant differences in the bacterial communities. A deeper analysis of the operational taxonomic units showed the presence of one genera, Mycoplasma spp. significantly associated with fertility problems. Our study highlights the promising potential that the application of sequencing techniques (e.g. 16S rRNA-based metagenomics) possesses in examining bovine infertility, as they are able to reveal different pathogens that could go unnoticed using diagnostic approaches for only the main known pathogens.
The aim of this prospective study was to assess whether the Sequential Organ Failure Assessment (SOFA) score could be indicative of outcome (survival to discharge) in dogs with parvoviral enteritis.
In 35 naturally infected dogs, the SOFA score and clinical score were calculated and the presence of systemic inflammatory response syndrome was verified on admission and during the first four days of hospitalization.
26 dogs survived, and out of the 9 non-survivors, 6 dogs had positive blood cultures. Mean SOFA scores and clinical scores between survivors and non-survivors and between septic and non-septic dogs on admission and on each hospitalization day were significantly different. Trends in SOFA score indicated that in non-survivors and septic dogs there was an increase in SOFA score during the first four days of hospitalization and a decrease occurred in survivors and non-septic dogs. The area under the curve (ROC curve analysis) for SOFA score predicting the outcome was 0.797 and predicting sepsis was 0.834. The best cut-off point of SOFA score for predicting the final outcome was 3.5 and the best cut-off of SOFA score for predicting sepsis was also 3.5.

Either single values or trends in SOFA score can assist in suspecting sepsis and reaching prognosis in parvoviral enteritis.Canine cognitive dysfunction (CCD) is a highly prevalent neurodegenerative disease considered the canine analog of early Alzheimer's disease (AD). Unfortunately, CCD cannot be cured. However, early therapeutic interventions can slow the progression of cognitive decline and improve quality of life of the patients; therefore, early diagnosis is ideal. In humans, electroencephalogram (EEG) findings specific to AD have been described, and some of them have successfully detect early stages of the disease. In this study we characterized the EEG correlates of CCD, and we compared them with the EEGs of healthy aging dogs and dogs at risk of developing CCD. EEG recordings were performed in 25 senior dogs during wakefulness. Dogs were categorized in normal, at risk of CCD or with CCD according to their score in the Rofina questionnaire. We demonstrated that, quantitative EEG can detect differences between normal dogs and dogs with CCD. Dogs with CCD experience a reduction in beta and gamma interhemispheric coherence, and higher Joint Lempel Ziv complexity. Dogs at risk of developing CCD, had higher alpha power and interhemispheric coherence, making these features potential markers of early stages of the disease. These results demonstrate that quantitative EEG analysis could aid the diagnosis of CCD, and reinforce the CCD as a translational model of early AD.Coxiella burnetii is the etiologic agent of Q fever, a zoonotic infectious disease of worldwide distribution that has a wide clinical spectrum. Transmission of C. burnetii occurs by inhalation of contaminated secretions and excreta of infected animal species, particularly goats, cattle and sheep. Activities associated with livestock contact represent the principal risk factor, however participation of wildlife reservoirs is underestimated. Thus, the aim of this study was to evaluate the presence of C. burnetii DNA in blood from bats. Molecular analyses using a qPCR targeting the IS1111 specific gene to detect DNA of C. burnetii in blood samples from 126 bats captured in the Macaregua cave, Colombia, between 2014, 2015 and 2018 were performed. Molecular evidence of C. burnetii was found in 6.3%. Results obtained in the present study represent the first detection of C. burnetii among bats in Colombia, suggesting that more studies need to be done in order to determine the role of these animals in the eco-epidemiology of Q fever.
