The addition of HER2-targeted agents to neoadjuvant chemotherapy has dramatically improved pathological complete response (pCR) rates in early-stage, HER2-positive breast cancer. Nonetheless, up to 50% of patients have residual disease after treatment, while others are likely overtreated. Here, we performed multiplex spatial proteomic characterization of 122 samples from 57 HER2-positive breast tumors from the neoadjuvant TRIO-US B07 clinical trial sampled pre-treatment, after 14-21 d of HER2-targeted therapy and at surgery. We demonstrated that proteomic changes after a single cycle of HER2-targeted therapy aids the identification of tumors that ultimately undergo pCR, outperforming pre-treatment measures or transcriptomic changes. We further developed and validated a classifier that robustly predicted pCR using a single marker, CD45, measured on treatment, and showed that CD45-positive cell counts measured via conventional immunohistochemistry perform comparably. However, PE at the High School level in New Zealand has assessment components that are still underpinned by traditional and outdated skill learning theories. In response to this challenge, two motivated Heads of Department in PE undertook a department-wide professional development initiative to teach the national standard assessment via the use of a contemporary skill acquisition approach, which is student-centred, with an emphasis on enhancing exploratory learning and encouraging autonomy. Each department worked together over a 10-week period with a Higher Education specialist in skill acquisition to design and teach using contemporary skill acquisition approaches. Qualitative data was collected via semi-structured focus group interviews. Insightful data on the influence of teaching using contemporary skill acquisition approaches was acquired from the teachers in the two PE departments. It was found that substantial pedagogical practice changes were achieved by the teachers (e.g., less focus on ideal technique and more on varying the context). Twelve national-level swimmers of both genders executed, on different occasions and after a standardized warm-up, a swimming protocol consisting of either a submaximal (Submax 8 efforts of 50 m) or a maximal interval (Max 4 efforts of 15 m), followed by two series of four maximal 25 m efforts. Near-infrared spectroscopy in terms of muscle oxygen saturation (SmO2), heart rate (HR), and blood lactate concentration (BLa) were analyzed at three testing points after the Submax or the Max protocol (TP1), after the 1st 4 × 25-m (TP2), and after the 2nd maximal 4 × 25-m set (TP3). Most of existing methodologies enabling efficient diagnosis are based on the data analysis, and in particular on some statistical quantities derived from the data. The present work explores the use of an original approach that instead of analyzing quantities derived from the data, analyzes the "shape" of the data, that is, the time series topology based on the homology persistence. We will prove that this procedure is able to extract valuable patterns able to discriminate the trajectories that the robot follows depending on the particular patch in which it operates, as well as to differentiate the robot behavior before and after undergoing a maintenance operation. Even if it is a preliminary work, and it does not pretend to compare its performances with respect to other existing technologies, this work opens new perspectives in considering quite natural and simple descriptors based on the intrinsic information that data contains, with the aim of performing efficient diagnosis and prognosis.

Migrant women born in Somalia often have poorer pregnancy outcomes. Access to care around pregnancy is vital to improve outcomes. The views and experiences of Somali migrant women accessing maternity care in the UK have not been extensively explored. This study therefore explores and describes these with regard to accessing maternity care in the UK, with the hope of gaining a better understanding of perceptions and factors that influence their access to care around pregnancy.

A qualitative approach was used to investigate the views and experiences of women born in Somalia who had migrated to Leicester. Data collected were transcribed and analyzed using a constant comparison method. The software package Nvivo 10 was used to organize themes, and verbatim quotes were used to support their interpretation.

Main finings included 1) positive attitudes of community midwives and availability lead to positive and meaningful experiences; 2) language difficulties and ineffective communication are barriers to effective access; 3) lack of cultural awareness and preconceived ideas by some hospital caregivers makes them unsupportive and insensitive; 4) need for continuity of care and the provision of resources are needed to build important trusting relationships with care providers; and 5) personal, community and religious factors impact access to care.

The results highlight a number of important potential barriers to accessing care around pregnancy for migrant women born in Somalia and how these could be addressed to improve pregnancy outcomes.

Data-driven approaches based on large databases or electronic health records can identify optimal combinations and often using artificial intelligence or machine learning to crunch through many possible combinations but are limited to the pharmacology of existing marketed drugs and are highly dependent upon the quality of the training sets. Knowledge-driven in silico modeling approaches use multi-scale biophysically realistic models of neuroanatomy, physiology, and pathology and can be personalized with individual patient comedications, disease state, and genotypes to create 'virtual twin patients'. Such models simulate effects on action potential dynamics of anatomically informed neuronal circuits driving functional clinical readouts. Informed by data-driven approaches this knowledge-driven modeling could systematically and quantitatively simulate all possible target combinations for a maximal synergistic effect on a clinically relevant functional outcome. This approach seamlessly integrates pharmacokinetic modeling of different therapeutic modalities. A crucial requirement to constrain the parameters is the access to preferably anonymized individual patient data from completed clinical trials with various selective compounds. We believe that the combination of data- and knowledge driven modeling could be a game changer to find a cure for this devastating disease that affects the most complex organ of the universe.
