Hence, we developed a context-fusion convolution neural network (CNN) to explore the application of chest radiographs to distinguish the etiology of CAP in children. This retrospective study included 1769 cases of pediatric pneumonia (viral pneumonia, n = 487; bacterial pneumonia, n = 496; and mycoplasma pneumonia, n = 786). The chest radiographs of the first examination, C-reactive protein (CRP), and white blood cell (WBC) were collected for analysis. All patients were stochastically divided into training, validation, and test cohorts in a 712 ratio. Automatic lung segmentation and hand-crafted pneumonia lesion segmentation were performed, from which three image-based models including a full-lung model, a local-lesion model, and a context-fusion model were built; two clinical characteristics were used to build a clinical model, while a logistic regression model combined the best CNN model and two clinical characteristics. Our experiments showed that the context-fusion model which integrated the features of the full-lung and local-lesion had better performance than the full-lung model and local-lesion model. The context-fusion model had area under curves of 0.86, 0.88, and 0.93 in identifying viral, bacterial, and mycoplasma pneumonia on the test cohort respectively. The addition of clinical characteristics to the context-fusion model obtained slight improvement. Mycoplasma pneumonia was more easily identified compared with the other two types. Using chest radiographs, we developed a context-fusion CNN model with good performance for noninvasively diagnosing the etiology of community-acquired pneumonia in children, which would help improve early diagnosis and treatment.Anhedonia reflects a reduced ability to engage in previously pleasurable activities and has been reported in children as young as 3 years of age. We investigated whether age at diagnosis of differentiated thyroid carcinoma (DTC) may be a risk factor for structural disease and mortality after primary treatment in aging patients.

A multicenter, retrospective analysis including patients older than 60 years at DCT diagnosis was designed. All subjects were treated by surgery with or without radioiodine adjuvant therapy and had a minimum follow-up of one year. We compared elderly patients (group 1, 60-74 years) with very old patients (group 2, ≥75 years).

The study involved 1668 patients (74.3% women, median age 67 years, 87.6% papillary thyroid cancer, 73.3% treated with radioiodine), including 1397 in group 1 and 271 in group 2. Patients in group 2 had larger tumor size (1.8 [0.8-3.0] vs 1.5 [0.7-2.8] cm; P = 0.005), higher proportions of tumors with vascular invasion (23.8 vs 16.4%; P = 0.006), and lower proportion of lymphocytic thyroiditis (18.6 vs 24.9%; P = 0.013) than subjects in group 1. Kaplan-Meier survival analysis showed that patients in group 2 had significantly lower structural disease-free survival (DFS) and overall survival (OS) than patients in group 1 (P = 0.035 and <0.001, respectively). In multivariate Cox regression analysis, age was a risk factor significantly associated with OS. However, age was significantly associated with DFS in unadjusted analysis and in analysis adjusted by gender and risk classification variables, but not when pathological and therapy-related variables were introduced in the model. On the other hand, patients who received radioiodine had worse DFS (P < 0.001) compared to those who did not.

Although the recent focus on institutional and policy reforms has led to a higher focus on environmental discussion, little is known about the status of research on the Pollution Haven Hypothesis (PHH). Hence, the current study evaluates the research dynamics of this field by recognizing most central researchers and key publication outlets from the perspectives of most citations and productivity, research directions, common keywords, countries with the highest academic contribution, and changes in research matrices. Our selection of 494 journal articles from the WOS indicates that King Saud University and the University of Wah were the most productive research institutions, and China was the most productive geographical region. Environmental Science & Pollution Research was identified as the most common outlet for research publications. We also identified strong academic cooperation, notably between China and Pakistan. Moreover, the co-occurrence network identified the Pollution Haven Hypothesis and economic growth nexus, trade, pollution haven and developing economies and FDI, carbon emissions, and pollution haven nexus as the three main prevailing research themes. Lastly, we provide useful policy implications to maximize the impact of environmental reforms and avoid environmental degradation.The past few decades of extreme drought and flooding caused by changing climate conditions have significantly affected agricultural production globally. This study focuses on two vital crops in China-maize and rice-and provides a comprehensive analysis of how these crops are affected by climate change-induced factors over the periods 1978Q1-2015Q4. Four key findings were obtained. First, using a nonparametric approach to estimate actual and observed trends of climatic variables, the results show a significant positive trend in average temperature from February to October. On the other hand, seasonal temperature increases during spring, summer, and autumn. Second, the results show no significant change in the monthly, seasonal, and annual rainfall patterns when examined over the study period. Third, using an autoregressive distributed lag (ARDL) model, we find that while temperature and rainfall do not significantly support rice production in the long and short run, they play a substantial role in maize production in China. Finally, we find no significant difference in the results for rice when the quantile regression (QR) technique that controls for distributional asymmetry effects is employed. However, the impact of temperature on maize decreases at higher quantiles. Analytical results indicated that invertebrates from sites affected by the tailings breach had elevated metal concentrations relative to those from non-affected or reference sites. We conducted a controlled laboratory exposure to determine if laboratory-reared Hyalella azteca metal concentrations were related to field-collected water or sediments from the same sites as the field study. Half of the replicates prevented amphipods from directly contacting sediments (water-only exposure), while the other half allowed them direct access (sediment and water exposure). Whole-body Cu concentration was highest in Hyalella exposed to substrate from the most contaminated sites as well as in treatments where they were allowed direct access to sediments. Hyalella having direct access to metal-contaminated sediments showed reduced survival and growth relative to those in reference or control treatments. The soil Cd concentration used in the treatments was 100 g·kg-1, and the sulfur fertiliser application rates were 0.1, 0.2, and 0.3 g·kg-1. Using pot experiments, we explored the impacts of high, medium, and low amounts of sulfur fertiliser on Cd accumulation and the quality and activity of E. breviscapus. The results showed that the application of sulfur fertiliser promoted Cd transformation to residual Cd under oxidation conditions, reducing Cd accumulation in E. breviscapus. Throughout the growth period, the application of sulfur fertiliser increased the soluble protein content and antioxidant enzyme activity, which alleviated Cd toxicity.
