ObjectiveThe aim of this study is to investigate the effect of modified uvulopalatopharyngoplasty (UPPP) combined with endoscopic hypothermic Plasma glossectomy (Eco-TBR) on the severe Obstructive sleep apnea syndrome. MethodSixty patients with severe OSAHS were diagnosed by polysomnography, and their obstructive plane located in the oropharynx and tongue base. Patients were divided into the control group (30 cases of simple H-UPPP) and the experimental group (30 cases of H-UPPP combined with Eco-TBR) according to their random hospital sequence. SPSS 20.0 software package was used to analyze the preoperative and postoperative data of the two groups. ResultFifty-seven OSAHS patients had full data and a minimum 6 month follow up to assess the efficacy. The total effective rate in the control group was 41.38% lower than that in the experimental group 67.85%, and the difference was statistically significant (χ²=4.03, P0.05). ConclusionThe effect of H-UPPP combined with Eco-TBR on severe OSAHS patients with obstructive plane of oropharynx and tongue root is definite.ObjectiveTo evaluate the value of improved Mallampati grading combined with NoSAS questionnaire in screening for obstructive sleep apnea (OSA). MethodA total of 344 patients admitted to our hospital for sleep disorders were studied. All patients were measured for their height, weight, neck circumference and other parameters. NoSAS scores, improved Mallampati grading and polysomnography (PSG) were performed in these patients. According to AHI in PSG monitoring results, patients were divided into non-osa group (AHI30) according to the PSG result. The ROC curve was plotted to evaluate the screening value of NoSAS and improved Mallampati grading combined with NoSAS for OSA. ResultWith the NoSAS score of 8 or 9 as cutoffs for analysis, the sensitivity for OSA was 0.733 and 0.701; the specificity for OSA was 0.538 and 0.624, respectively. The sensitivity and specificity of NoSAS combined with improved Mallampati grading for screening OSA were 0.813 and 0.710, respectively. ConclusionAs a new screening tool, NoSAS questionnaire is simple and convenient, and has certain screening value to OSA. The improved Mallampati grading combined with NoSAS questionnaire can obviously improve the screening sensitivity and specificity of Osa, and has higher application value.ObjectiveTo compare the efficacy of cochlear implantation in deaf patients with Mondini malformation and normal anatomical structure of inner ear, and to determine whether cochlear implantation can be an effective way to reconstruct hearing in hearing loss patients with Mondini malformation, so as to guide clinical work. Higher ELR value ,lower NLR value, and association with allergic rhinitis, asthma can be used as prediagnostic indicators for eCRSwNP before invasive pathological results and CT examination.
