sed in a speech coding strategy.We evaluated outcomes of treatment with 5-fluorouracil (5-FU), doxorubicin, and streptozocin (FAS) in well-differentiated pancreatic neuroendocrine tumors (PanNETs) and its impact on subsequent therapy (everolimus or temozolomide). Advanced PanNET patients treated at our center from 1992 to 2013 were retrospectively reviewed. Patients received bolus 5-FU (400 mg/m2), streptozocin (400 mg/m2) (both IV, days 1-5), and doxorubicin (40 mg/m2 IV, day 1) every 28 days. Overall response rate (ORR) was assessed using RECIST version 1.1. Of 243 eligible patients, 220 were evaluable for ORR, progression-free survival (PFS), and toxicity. Oxaliplatin order Most (≥90%) had metastatic, nonfunctional PanNETs; 14% had prior therapy. ORR to FAS was 41% (95% confidence interval [CI] 36-48%). Median follow-up was 61 months. Median PFS was 20 (95% CI 15-23) months; median overall survival (OS) was 63 (95% CI 60-71) months. Cox regression analyses suggested improvement with first-line versus subsequent lines of FAS therapy. Main adverse events ≥ grade 3 were neutropenia (10%) and nausea/vomiting (5.5%). Dose reductions were required in 32% of patients. Post-FAS everolimus (n = 108; 68% second line) had a median PFS of 10 (95% CI 8-14) months. Post-FAS temozolomide (n = 60; 53% ≥ fourth line) had an ORR of 13% and median PFS of 5.2 (95% CI 4-12) months. In this largest reported cohort of PanNETs treated with chemotherapy, FAS demonstrated activity without significant safety concerns. FAS did not appear to affect subsequent PFS with everolimus; this sequence is being evaluated prospectively. Responses were noted with subsequent temozolomide-based regimens although PFS was possibly limited by line of therapy.

Patients with premorbid functional impairment are generally excluded from acute stroke trials. We aimed to determine the impact of including such patients in the Head Positioning in acute Stroke Trial (HeadPoST) and early additional impairment on outcomes.

Post hoc analyses of HeadPoST, an international, cluster-randomized crossover trial of lying-flat versus sitting-up head positioning in acute stroke. Associations of early additional impairment, defined as change in modified Rankin scale (mRS) scores from premorbid levels (estimated at baseline) to Day 7 ("early ΔmRS"), and poor outcome (mRS score 3-6) at Day 90 were determined with generalized linear mixed model. Heterogeneity of the trial treatment effect was tested according to premorbid mRS scores 0-1 versus 2-5.

Of 8,285 patients (38.9% female, mean age 68 ± 13 years) with complete data, there were 1,984 (23.9%) with premorbid functional impairment (mRS 2-5). A significant linear association was evident for early ∆mRS and poor outcome (per 1-point increase in ΔmRS, adjusted odds ratio 1.20, 95% confidence interval 1.14-1.27; p < 0.0001). Patients with greater premorbid functional impairment were less likely to develop additional impairment, but their risk of poor 90-day outcome significantly increased with increasing (worse) premorbid mRS scores (linear trend p < 0.0001). There was no heterogeneity of the trial treatment effect by level of premorbid function.

Early poststroke functional impairment that exceeded premorbid levels was associated with worse 90-day outcome, and this association increased with greater premorbid functional impairment. Yet, including premorbid impaired patients in the HeadPoST did not materially affect the subsequent treatment effect.

When the chemical potential coincides with the flat band, it is expected that the impurity response of this system will be very different from that of an ordinary Dirac or Weyl semimetal of which the density of states at the Fermi level vanishes. We first determine the phase diagram within the mean-field approximation. Then, we study the local moment regime by employing two different methods. In the low temperature regime, the Kondo screening is analyzed by the variational wavefunction approach and the impurity contributions to the magnetic susceptibility and heat capacity are obtained, while at higher temperature, we use the equation of motion approach to calculate the occupation number of the impurity site and the impurity magnetic susceptibility. The results are compared and contrasted with those in the usual Fermi liquid and the Dirac/Weyl semimetals.Aqueous zinc-ion hybrid supercapacitors are considered to be a newly emerging electrochemical energy storage devices. However, the exploration and design of advanced cathode materials remain a huge challenge. Herein, we developed a versatile one-step magnesiothermic reduction and self-activation process for the synthesis of highly-curved carbon nanosheets (HCCNs) with hierarchical pore structures. In this process, low-molecular weight organic potassium salts (e.g. potassium bitartrate, potassium acetate, potassium oxalate, potassium formate, potassium oleate, potassium sorbate), which usually used as the chemical activating reagents, serve as the carbon source whereas the Mg power acts as the reducing agent. The resulting HCCNs possess hierarchical porosity and unique HCCN geometry which can afford abundant active sites for charge accumulation as well as the highly efficient ions diffusion kinetics. Because of the high ratio of surface-controlled capacitive contribution and high ions diffusion coefficient, the optimized sample can exhibit excellent charge storage performance with an impressive reversible capacity (200.2 mAh g-1), excellent rate capability, and good cyclic stability. The excellent capacitive behaviors combined with the feasible synthetic procedure make the present synthetic protocol a promising choice towards well-designed nanocarbons for electrochemical energy storage applications.
