Surgical site infection is one of the most common causes of readmission in hospitals and it also leads to an overall increased burden. This can be decreased by applying basic concepts of Nightingale (1860) "Environmental Theory" while giving care to the patients. Keeping in view Nightingale's environmental theory, this paper aims to discuss the case of a patient suffering from a surgical site infection. It will help readers to understand the environmental factors which affect the patients' health and provide a way to promote healing and fast recovery by modifying their environment.Pleural effusion is a frequently seen medical problem caused by pulmonary and non-pulmonary diseases. Spondylodiscitis is a very rare cause of pleural effusion and is typically diagnosed based on clinical, laboratory, microbiological and radiological findings. The low incidence and different clinical presentations of Spondylodiscitis make its diagnosis and treatment challenging. We present the case of a 78-year-old female who was initially admitted due to chest pain and upon chest radiography, was found to have pleural effusion; and eventually diagnosed with spondylodiscitis.Antagonists of the Adenosine Diphosphate (ADP) receptor, P2Y12, may inhibit platelet aggregation as a result of stimulation with arachidonic acid (AA). The potent P2Y12 blocker, Ticagrelor has greater anti-platelet effects than Clopidogrel. We explored the effects of Ticagrelor versus Clopidogrel on mean maximum aggregation ratios (MAR%) in response to AA stimulation in patients receiving aspirin in conventional doses. A total of 613 acute coronary syndrome (ACS) patients were followed from October 2017 to October 2018. At the one- and six-month follow-up visit, mean AA-MAR% was lower in the Ticagrelor group when compared with the Clopidogrel group (28.9% vs 31.7%, 28.4% vs 31.0%, p less then 0.001 and p=0.001, respectively). BARC1-2 bleeding occurred with greater frequency with Ticagrelor than in patients treated with Clopidogrel (29.3% vs 9.5%, p less then 0.001; 23.5% vs 9.3%, p less then 0.001). Excessive platelet inhibition and decreased AA-MAR% were considered the main reasons for the severe subcutaneous/dermal bleeding in Ticagrelor treated patients.Privacy and confidentiality are considered a cornerstone in the practice of medical ethics. However, these notions may play out differently in the cultural context of Pakistan. In order to understand the perceptions and expectations of privacy and confidentiality, a cross-sectional mixed method study was undertaken in a tertiary care hospital in Karachi, Pakistan. While the subjects demonstrated unfamiliarity with the Western terms, majority of them also exhibited a high expectation for privacy (both informational and physical) and confidentiality. Patients appeared most comfortable with sharing private medical information with the primary physician, indicating the level of trust placed in the physician. Participants also showed high expectations for confidentiality, thus, even in a collectivist society, patients may not want their private information shared across the medical team and also among family members. The onus is, therefore, on healthcare professionals to assess patients' preferences and choices.

To analyse clinical and molecular features in patients with surgically resected patients with lung cancer harbouring anaplastic lymphoma kinase fusion.

The retrospective study was conducted at Zhejiang Cancer Hospital, Hangzhou, China, and comprised data from November 2013 to August 2015 of lung cancer patients. Anaplastic lymphoma kinase, epidermal growth factor receptor, kirsten rat sarcoma viral oncogene, v-raf murine sarcoma viral oncogene homolog, REarranged during Transfection proto-oncogene, c-ros oncogene 1 receptor kinase, V-Erb-B2 avian erythroblastic leukaemia viral oncogene homolog 2 and mesenchymal epithelial transition factor were noted using next generation sequencing. Clinicopathological parameters were also investigated. All patients were followed up till August 10, 2017. Data was analysed using SPSS 22.

Of the 19 patients, 15(79%) were non-smokers. Anaplastic lymphoma kinase rearrangements occurred in the acinar predominant in 6(31.6%), solid predominant 6(31.6%) and mucinous predominant 4(21%) adenocarcinomas. There was 1(5.2%) patient with epidermal growth factor receptor 21 G863D mutation. The 3-year disease-free survival rate in 5(26.3%) cases of anaplastic lymphoma kinase variant 1 was 5(100%), while in the 14(73.7%) cases of non-variant 1 group it was 9(64.3%) (p=0.257).

It is one of the most devastating diseases which has an enormous psychosocial and economic burden. The treatment available includes haemodialysis, peritoneal dialysis and kidney transplantation. All treatment options have their pros and cons, needs and preferences, though haemodialysis is one of the most available entity in Pakistan. There are a few intrinsic problems associated with haemodialysis which has significant environmental as well as economic impact. One of the most important is the need of huge quantity of water to carry out the haemodialysis procedure along with production of solid waste in the form of disposables and electricity consumption. Peritoneal dialysis on the other hand is more environment-friendly and cost-effective and it should be one of the preferred options for our CKD patients.A cross-sectional observational study was conducted to assess the knowledge, attitude, and practice (KAP) of breast self-examination (BSE) among female undergraduate medical students in Poonch Medical College, Azad Kashmir. Out of 306 respondents, 250 completed the 16-item questionnaire (response rate of 81.7%). The correlations between knowledge, attitude, and practice were observed. Out of the total 250 respondents, 180 (72.0%) had ever heard of BSE, and 62 (24.8%) had performed BSE. The most important source of information regarding BSE was 'internet', which delivered knowledge to 64 (25.6%) students. see more The majority 200 (80%) thought that publicity and campaigns can motivate students to perform BSE. There was mild uphill correlation between knowledge and attitude, and knowledge and practice (p less then 0.001). The study identified poor KAP of BSE among the students, although most of the students gave high importance to BSE practice. Nationwide BSE campaigns can be undertaken to increase the practice as most of the respondents were positive about mass campaign.
