Clinical features of central nervous system tuberculosis (CNS-TB) are nonspecific. The decision for treatment of the disease in an endemic area is challenging.
We aimed to study predictive factors for a definite diagnosis and outcome of patients with CNS-TB.
A case-control study was performed in adults with a provisional diagnosis of CNS-TB in Thailand to determine predictive factors for a definite diagnosis of CNS-TB. Predictive factors for a definite diagnosis of CNS-TB were analyzed by multivariable logistic regression analysis. Factors associated with two-year mortality after the diagnosis of definite CNS-TB were determined using a cox regression analysis.
A total of 114 patients received a provisional diagnosis of CNS-TB during the study period. A median (interquartile range) age was 40.8 (31.7-55.4) years, and 75 patients (65.8%) were male. Of these, 66 cases (57.9%) had definite CNS-TB, and 43 cases (38.4%) had HIV coinfection. By logistic regression, age, confusion, and nausea/vomiting were asight be helpful for the management of suspected CNS-TB cases as well as predicting the prognosis of CNS-TB cases in an endemic area.
There is an increasing trend worldwide in the incidence of
complex pulmonary diseases (MAC-PD) and the diagnosis is sometimes complicated. Recently, an enzyme immunoassay (EIA) kit that detects serum IgA antibody against MAC-specific glycopeptidolipid (GPL) core antigen had been developed and found to be useful in discriminating MAC-PD from other lung diseases. The antibody was subsequently also found to be elevated in patients suffering
pulmonary diseases (MAB-PD). This study is to evaluate this EIA kit in the serological diagnosis of MAC-PD in Hong Kong Chinese patients.
The study was conducted in Grantham Hospital, Hong Kong between July 2017 and July 2018. Assay of the IgA antibody level using the EIA kit was done on blood samples collected from patients suffering from MAC-PD, MAB-PD, pulmonary tuberculosis and other lung diseases.

Further larger scale studies in our local population for the usefulness of this antibody test in the diagnosis and monitoring of MAC and M. abscessus lung diseases might be warranted.A 49-year-old male HIV positive patient on treatment failure presented with complaints of fever and dysphagia of three weeks duration and later on developed cervical lymphadenopathy along with severe vomiting and abdominal pain. Liver function tests were found to be worsening with severe drop in CD4 counts. An extensive workup for pyrexia was done. FNAC and biopsy of lymph node showed features suggestive of granulomatous lymphadenitis. CBNAAT of the lymph node aspirate was negative for MTB. Blood culture and lymph node cultures were negative for Mycobacterium Avium Complex (MAC). MAC was however, finally detected and reported positive on Fluorescence in Situ Hybridization (FISH) of the cervical lymph node aspirate. Prompt treatment for MAC was initiated with Ethambutol 800 mg OD and Azithromycin 500 mg OD following which fever spikes subsided and lymph node resolved. The Patient's condition gradually improved and was discharged shortly with a good recovery on subsequent follow ups. Fever is one of the common symptoms in patients with MAC infection. Some other clinical manifestations include weight loss, hepatosplenomegaly and intra-abdominal lymphadenopathy. Diagnostic evaluation should be aggressive. As there is a high risk for MAC infection in advanced HIV cases with poor HAART compliance, FISH can be a valuable and effective diagnostic tool in early detection and treatment of MAC.

DNA of 50 MTB isolates were extracted by boiling method. The existence of
gene was determined using PCR-specific primer and finally PCR product was sequenced to examine the polymorphism in both direct and reverse directions. Sequencing results were aligned by chromas software.
The
gene was detected in all of the Beijing and non-Beijing isolates. The polymorphism in the sequences of this gene were not observed in all of the MTB isolates.
