To probe the ability of different root tissues to detoxify As(III) produced by HAC1 we generated A. thaliana lines expressing HAC1 in different cell-types. We investigated the As(V) tolerance phenotypes root growth, As(III) efflux, arsenic translocation, and arsenic chemical speciation. We showed that HAC1 can function in the outer tissues of the root (epidermis, cortex and endodermis) to confer As(V) tolerance, As(III) efflux and to limit arsenic accumulation in shoots. HAC1 is less effective in the stele at conferring As(V) tolerance phenotypes. The exception is HAC1 activity in the protoxylem, which we found to be sufficient to restrict As translocation, but not to confer As(V) tolerance. In conclusion, we describe cell-type specific functions of HAC1 that separate spatially the control of As(V) tolerance and As translocation. Further, we identify a key function of protoxylem cells in As(V) translocation, consistent with the model where endodermal passage cells, above protoxylem pericycle cells, form a 'funnel' loading nutrients and potentially toxic elements into the vasculature.

We aimed to explore the prevalence of portal hypertension in the most common etiologies of patients with compensated advanced chronic liver disease (cACLD) and develop classification rules, based on liver stiffness measurement (LSM), that could be readily used to diagnose or exclude clinically significant portal hypertension (CSPH) in clinical practice.

This is an international cohort study including patients with paired LSM/hepatic venous pressure gradient (HVPG), LSM ≥10 kPa, and no previous decompensation. Portal hypertension was defined by an HVPG >5 mm Hg. A positive predictive value ≥90% was considered to validate LSM cutoffs for CSPH (HVPG ≥10 mm Hg), whereas a negative predictive value ≥90% ruled out CSPH.

A total of 836 patients with hepatitis C (n = 358), nonalcoholic steatohepatitis (NASH, n = 248), alcohol use (n = 203), and hepatitis B (n = 27) were evaluated. Thirteen subjects (age, 23.3 ± 3.5 years) performed parallel and half-squat exercises (80% of one repetition maximum, eight repetitions, five sets) using a Smith machine. Sagittal magnetic resonance diffusion-weighted and spin-echo images of lumbar intervertebral disks were obtained by using a 1.5-Tesla MR system before and after each squat exercise; apparent diffusion coefficient (ADC; an index of water movement) and T2 relaxation time (an index of water content level) of the nucleus pulposus were calculated at all lumbar intervertebral disks. Additionally, we measured the angles of lumbar lordosis and anterior pelvic tilt at the bottom position of each squat using a three-dimensional motion-capture system. The nucleus pulposus of L4/5 (-5.0%, P less then .01) and L5/S1 (-6.6%, P less then .01) intervertebral disks showed decreased ADC values after parallel squat exercise. Moreover, post-exercise ADC value in parallel squat exercise was lower than that in half-squat exercise at L5/S1 intervertebral disk (P less then .05). In contrast, the nucleus pulposus of all lumbar intervertebral disks had no significant T2 change before and after both squat exercises. The angles of lumbar lordosis (P less then .01) and anterior pelvic tilt (P less then .01) were smaller in parallel squat than in half-squat. We aim to study the diagnostic performance of this screening strategy in a country with intermediate gastric cancer risk.

A retrospective cohort study using a prospective endoscopy database from 2015 to 2017 was performed. Patients included were individuals age >40 who underwent opportunistic upper endoscopy at the same setting of colonoscopy without any OG symptoms. Neoplastic OG lesions are defined as cancer and high-grade dysplasia. Pre-neoplastic lesions include Barrett's esophagus (BE), intestinal metaplasia (IM), and atrophic gastritis (AG).

The study population involved 1414 patients. Neoplastic OG lesions were detected in five patients (0.35%). Pre-neoplastic lesions were identified in 174 (12.3%) patients. IM was found in 146 (10.3%) patients with 21 (1.4%) having extensive IM. The number needed to scope to detect a neoplastic OG lesion is 282.8 with an estimated cost of USD$141400 per lesion detected. On multivariate regression, age ≥60 (RR 1.84, 95% CI 1.29-2.63) and first-degree relatives with gastric cancer (RR 1.64, 95% CI 1.06-2.55) were independent risk factors for neoplastic or pre-neoplastic OG lesion.

For countries with intermediate gastric cancer risk, opportunistic upper endoscopy may be an alternative screening strategy in a selected patient population. Prospective trials are warranted to validate its performance.

Prospective trials are warranted to validate its performance.Mechanical circulatory support (MCS) refers to a range of rescue devices to assist circulation for the treatment of heart failure, including venoarterial extracorporeal membrane oxygenation (VA-ECMO) and ventricular assist devices (VADs). This review aims at evaluating the transplant outcome of the livers procured from brain-dead donors on MCS, who are currently considered as having extended criteria. We identified 22 records (17 on VA-ECMO and 5 on VADs), most of which (68.2%) were case reports. We performed a meta-analysis only when the outcome was reported homogeneously among studies; otherwise, we illustrated the results with narrative synthesis. A total of 156 liver transplants (LTs) have been reported, where VA-ECMO was initiated in the donor with resuscitative intent or as a bridge to donation. Early graft survival approached 100% in most studies. The pooled rate of primary nonfunction was 1% (95% CI 0-3%). Only three successful LTs from VAD donors have been reported. Particular attention should be paid to cardiological history, biochemical tests, and imaging, as well as MCS parameters, to determine graft eligibility for transplantation. Although further analysis is needed in this field, the results of this review advocate a more systematic consideration of brain-dead patients on MCS as potential liver donors.
