This study aimed to determine the feasibility of using a wrist-based fitness tracking device to assess sleep among Obstetrics and Gynecology (OBGYN) trainees who engaged in a yoga-based wellness program. We also sought to evaluate the effects of yoga on sleep.
A quality improvement initiative consisting of an 8-week wellness program of weekly yoga classes, nutrition, and physical challenges was implemented for OBGYN residents and Maternal-Fetal Medicine fellows. The Polar A370 fitness tracker device was provided and synced to the Polar Flow for Coach program for inclusion. Data obtained included total and restful sleep from each night the device were worn. Pre- and post-assessment of the Pittsburg Sleep Quality Index (PSQI) were compared. Linear mixed models were used to estimate and test the effect of yoga on sleep while controlling for on-call shifts.
Of the 15 participants who synced their device, 13 (87%) were included for analysis. Sleep data from 572 nights were analyzed. In recent years, the sacroiliac joint has become increasingly important as a generator of low back pain with and without pseudo-radicular pain in the legs. Up to 27% of reported back pain is generated by disorders in the sacroiliac joint.
This review is based on a selective literature search of the sacroiliac joint (SIJ) as a possible pain generator. It also considers the anatomical structures and innervation of the sacroiliac joint.
The SIJ is a complex joint in the region of the posterior pelvis and is formed by the sacrum and the ilium bones. The SIJ is very limited in movement in all three planes. Joint stability is ensured by the shape and especially by strong interosseous and extraosseous ligaments. Different anatomical variants of the sacroiliac joint, such as additional extra-articular secondary joints or ossification centres, can be regularly observed in CT scans. There is still controversy in the literature regarding innervation. However, there is agreement on dorsal innervation of the sacroilding structures opens the possibility that pain arises from this area. The SIJ and its surrounding structures should be included in the diagnosis and treatment of back and leg pain. Published literature include a number of plausible models for the sacroiliac joint as pain generator. The knowledge of the special anatomy, the complex innervation as well as the special and sometimes very individual functionality of this joint, enhance our understanding of associated pathologies and complaints.
Since the introduction of shared electric scooters in Germany in June 2019, surgeons have been treating injuries caused by these devices.
In our Institution, we implemented the first registry to examine the pattern of injuries and to gather epidemiological data on persons injured while riding electric scooters. We conducted a prospective analysis of all patients treated in our Emergency Department for an injury sustained while riding an electric scooter between June 2019 and June 2020. All patients were noted in a registry (E-SCORE = E-Scooter Register). The study was approved by the institutional ethics board. Outcomes were first time of use, injuries, imaging, alcohol and helmet use, age, length of hospital stay and surgical therapy.
90 patients were identified. Mean age was 35.6 years (± 15.4); 65.5% were male. We diagnosed 32 fractures and 7 ligament injuries. Injuries to the head were found in 38 patients; 8 had relevant intracerebral or maxillofacial injuries. Alcohol use was noted in 18 (20%) pat was not unusual. In our opinion, scooter related injuries are leading to an growing number of vehicle related injuries and could probably benefit from some legal regulations, such as a blood alcohol limit and recommendation for helmet use. Implementation of a nationwide registry might be useful in evaluating injuries related to electric scooters.BACKGROUND Endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) is recommended for the diagnosis of solid pancreatic masses. We aimed to evaluate whether a high needle movement acceleration value during puncture increases diagnostic accuracy. METHODS EUS-FNA needle acceleration was measured using a PocketLab accelerometer connected by Bluetooth to a smartphone. Two passes (fast and slow, with higher and lower than 1g [9.8 m/s2] needle acceleration values, respectively) were performed in a random order. The sample cellularity and quality were measured using semiquantitative scales. RESULTS 51 patients were included (32 women; mean age 63). The mean (standard deviation [SD]) acceleration values were 1.59g (0.66g) for the fast pass and 0.32g (0.19g) for the slow pass (P less then 0.001). The fast pass yielded significantly higher levels of EUS-FNA accuracy (84.3 % vs. 68.6 %; P = 0.02) and adequate quality scores (94.1 % vs. 76.5 %; P = 0.007). High cellularity scores were seen with similar frequencies (15.7 % vs. 11.8 %; P = 0.32). CONCLUSIONS A higher than 1g EUS-FNA needle acceleration may increase the diagnostic accuracy and specimen quality from solid pancreatic lesions.The production and use of the highly addictive stimulant methamphetamine are a serious public health problem in the United States and globally. Because of its increased popularity with recreational drug users, accidental or intentional poisoning incidents in companion animals have become an unavoidable scenario in veterinary medicine. We describe a case of methamphetamine poisoning in a 4-year-old female German Shepherd with postmortem analytical quantitation of methamphetamine and its metabolite, amphetamine, in bodily tissues and fluids. Many tissues and bodily fluids can be tested to confirm methamphetamine exposure. More importantly, the higher concentrations found in stomach contents, liver, kidney and heart tissue suggest these are the most useful diagnostic specimens for postmortem confirmation of toxicosis in pets especially in cases in which source material is not available for testing or in cases with no postmortem evaluation.We argue that the current environmental changes stressing the Earth's biological systems urgently require study from an integrated perspective to reveal unexpected, cross-scale interactions, particularly between microbes and macroscale phenomena. Such interactions are the basis of a mechanistic understanding of the important connections between deforestation and emerging infectious disease, feedback between ecosystem disturbance and the gut microbiome, and the cross-scale effects of environmental pollutants. These kinds of questions can be answered with existing techniques and data, but a concerted effort is necessary to better coordinate studies and data sets from different disciplines to fully leverage their potential.The sow at parturition is challenged with respect to energy status due to increases in energetic expenses associated with 1) nest building 2) uterine contractions, and 3) colostrum production. A previous study indicated that sows were depleted of glucogenic energy around farrowing. The aim was to investigate whether intravenous infusion of glucose from observed nest-building behavior to 24 h postpartum affected the farrowing kinetics and colostrum production in sows. Ten multiparous sows (DanBred landrace × DanBred Yorkshire) were fitted with a jugular vein catheter on each side (one for infusion and the other one for blood sampling). Sows were infused with either 0.9% saline (CON; n = 5) or 10% glucose (GLU; n = 5) solution at a constant rate of 125 mL/h. From day 108 of gestation, sows were fed once daily with 3.33 kg of a standard lactation diet. During farrowing, sows were monitored to register the onset of farrowing, time of birth, birth status (live or dead), sex, stillbirth rate (SR), and weight of newd with CON sows during the colostrum period. No effect of GLU infusion was seen on colostrum and milk composition and yield. The increase in lactate was most likely due to a shift toward a greater proportion of glucose oxidation and insufficient O2 supply to fuel uterine contractions. In conclusion, infusion of glucose reduced the frequency of SR and FA, and improved energy status of the sow which seems to be crucial to enhance total piglet survival.These initial data suggest that with prenatal vitamins and choline supplements, we might decrease one risk factor associated with poorer health outcomes disproportionally affecting Black families, ie, preterm birth. Dissemination of this research fulfills the principle of Justice in the Belmont Report, to ensure that participants from different racial, ethnic and socioeconomic groups receive benefits from research directed to their specific problems.The purpose of the present work was to evaluate the use of resampling statistical methods for analysis of visual grading data-implemented in the software VGC Analyzer-by comparing the reanalyzed results from previously performed visual grading studies with the results calculated by gold standard receiver operating characteristic (ROC) methodology, Obuchowski-Rockette (OR)-Dorfman-Berbaum-Metz (DBM) multiple-readers and multiple-case (MRMC) and by analysis of simulated visual grading data where the true distribution was presumed to be known. The reanalysis was performed on two multiple-reader studies with non-paired data and paired data, respectively. The simulation study was performed by simulating a large number of visual grading characteristics (VGC) studies and by analyzing the statistical distribution of null hypothesis (H0) rejection rate. The comparison with OR-DBM MRMC showed good agreement when analyzing non-paired data for both fixed-reader and random-reader settings for the calculated area under the curve values and the confidence intervals (CIs). For paired data analysis, VGC Analyzer showed significantly lower CIs compared with the ROC software. This effect was also illustrated by the simulation study, where the VGC Analyzer, in general, showed good accuracy for simulated studies with stable statistical basis. For simulated studies with unstable statistics, the accuracy in the H0 rejection rate decreased. The present study has shown that resampling methodology can be used to accurately perform the statistical analysis of a VGC study, although the resampling technique used makes the method sensitive to small data sets.Canola meal (CM) contains less crude protein (CP) and more fiber and anti-nutritional factors such as glucosinolates than soybean meal (SBM) and consequently has a lower nutrient digestibility. Therefore, processing strategies that may increase the feeding value of CM warrant study. In two experiments, the effects of extrusion of Brassica napus CM on apparent (AID) and standardized ileal digestibility (SID) of amino acids (AA), apparent total tract digestibility (ATTD) of gross energy (GE) in growing pigs, and growth performance and diet digestibility in weaned pigs were assessed. Solvent-extracted CM was extruded using a single-screw extruder at three screw speeds 250 (CM-250), 350 (CM-350), or 450 (CM-450) rpm. In exp. 1, in a double 4 × 4 Latin square, eight ileal-cannulated barrows (initial body weight [BW], 68.1 kg) were fed corn starch-based diets containing 50% CM or extruded CM. The CM sample contained 43.2% CP, 33.2% total dietary fiber (TDF), and 8.9 µmol of total glucosinolates/g on a dry matter (DM) basis.
