Acute kidney injury (AKI) and fluid overload affect a large number of children undergoing cardiac surgery, and confers an increased risk for adverse complications and outcomes including death. Survivors of AKI suffer long-term sequelae. The purpose of this narrative review is to discuss the short and long-term impact of cardiac surgery associated AKI and fluid overload, currently available tools for diagnosis and risk stratification, existing management strategies, and future management considerations.

Improved risk stratification, diagnostic prediction tools and clinically available early markers of tubular injury have the ability to improve AKI-associated outcomes. One of the major challenges in diagnosing AKI is the diagnostic imprecision in serum creatinine, which is impacted by a variety of factors unrelated to renal disease. In addition, many of the pharmacologic interventions for either AKI prevention or treatment have failed to show any benefit, while peritoneal dialysis catheters, either for passive drainage or prophylactic dialysis may be able to mitigate the detrimental effects of fluid overload.

Until novel risk stratification and diagnostics tools are integrated into routine practice, supportive care will continue to be the mainstay of therapy for those affected by AKI and fluid overload after pediatric cardiac surgery. A viable series of preventative measures can be taken to mitigate the risk and severity of AKI and fluid overload following cardiac surgery, and improve care.

Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) is one of the most serious extra-articular RA manifestations. find more RA-ILD is associated with worse physical function, lower quality of life, and increased mortality. RA-ILD is comprised of heterogeneous subtypes characterized by inflammation and fibrosis. Diagnosis can be difficult since the presentation of RA-ILD is characterized by non-specific symptoms and imaging findings. Management of RA-ILD is also challenging due to difficulty in precisely measuring pulmonary disease activity and response to treatment in patients who may also have articular inflammation. We provide a current overview of RA-ILD focusing on prevalence, risk factors, and treatment.

Research interest in RA-ILD has increased in recent years. Some studies suggest that RA-ILD prevalence may be increasing; this may be due to underlying biologic drivers or increases in imaging and recognition. Novel RA-ILD risk factors include the

promotor variant, articular disease activity, autoantibodies, and biomarkers of damaged pulmonary parenchyma. Treatment should focus on controlling RA disease activity, which emerging data suggest may reduce RA-ILD risk. Immunomodulatory and antifibrotic drugs may also treat RA-ILD.

These collisions can lead to either growth or disruption of the bodies involved and are largely responsible for shaping the final characteristics of the planets. Despite their critical role in planet formation, an accurate treatment of collisions has yet to be realized. While semi-analytic methods have been proposed, they remain limited to a narrow set of post-impact properties and have only achieved relatively low accuracies. However, the rise of machine learning and access to increased computing power have enabled novel data-driven approaches. In this work, we show that data-driven emulation techniques are capable of classifying and predicting the outcome of collisions with high accuracy and are generalizable to any quantifiable post-impact quantity. In particular, we focus on the dataset requirements, training pipeline, and classification and regression performance for four distinct data-driven techniques from machine learning (ensemble methods and neural networks) and uncertainty quantification (Gaussian processes and polynomial chaos expansion). We compare these methods to existing analytic and semi-analytic methods. Such data-driven emulators are poised to replace the methods currently used in N-body simulations, while avoiding the cost of direct simulation. This work is based on a new set of 14,856 SPH simulations of pairwise collisions between rotating, differentiated bodies at all possible mutual orientations.

Such reinforcers offer considerable promise in applied behavior analysis as a means of both potentiating training of new responses and response classes and enhancing previously learned ones. The concept of jackpot reinforcement, however, is rife with not only a lack of definitional and procedural clarity but also a paucity of research, either basic or applied, on such reinforcement. Considerations in undertaking such research include defining the parameters of jackpot reinforcers, identifying suitable dependent variables, and creating experimental designs appropriate for their assessment. The few experimental analyses of jackpot reinforcer effects on either response acquisition or maintenance have produced little evidence of systematic effects, despite the use of several different methods and behavioral measures. Negative results could reflect either the absence of systematic effects of these jackpot reinforcers or unsuitable methods of analysis. Only further research will take the topic of jackpot reinforcers beyond opinion and testimonial and into the realm of the science of behavior.Knowledge and perception on transmission and control of SARS-COV-2 infection are key to preventing outbreak of the disease in healthcare settings and in dealing with the COVID-19 pandemic. This study assessed the level of knowledge on SARS-COV-2 infection prevention, transmission and symptoms of COVID-19, as well as perceptions regarding prevention of SARS-COV-2 infection among allied radiation medicine professionals. Cross-sectional descriptive survey was carried out among 145 radiation medicine professionals in Ghana using facts on COVID-19 as presented on the website of the World Health Organization and data was analyzed based on weighted average indices. Overall, the extent of knowledge among allied radiation medicine professionals on the symptoms of COVID-19, transmission and control of SARS-COV-2 infection in radiation medicine facilities were all adequate, with weighted average indices of 3.8, 4.1 and 4.4 respectively. However, overall perception of the respondents regarding the use of radiation medicine procedures in management of COVID-19 was diverse, with weighted index of 3.5. The facts about COVID-19 that were identified to be most known were shortness of breath being a serious symptom of the disease and fever being a common symptom. The extent of knowledge on the fact that "SARS-COV-2 infection can be transmitted through small droplets from the nose or mouth of an infected person" was almost excellent, with weighted average index of 4.9. Also, the thinking that provision of hand washing and sanitizing facilities is a measure for controlling the infection was almost perfectly shared among the respondents. The interaction is modeled by the fact that the outbreak time of the epidemic in a sub-cluster is a random variable with probability density slowly varying in time. The explanation is independent of the law according to which the epidemic evolves in the single sub cluster.The experimental results regarding optical absorption and steady-state photoconductivity of amorphous single-layer structures (Al-As0.40S0.30Se0.30-Al, Al-Ge0.09As0.09Se0.82-Al, and Al-Ge0.30As0.04S0.66-Al) and of an amorphous heterostructure (Al-As0.40S0.30Se0.30/Ge0.09As0.09Se0.82/Ge0.30As0.04S0.66-Al) at different values of the voltage, with positive or negative polarity, applied to the illuminated top Al electrode are presented and discussed. The complex structure of the photocurrent spectra is attributed to the different values of the optical bandgap of the involved amorphous layers (Eg ≈ 2.0 eV for As0.40S0.30Se0.30 and Ge0.09As0.09Se0.82 and Eg ≈ 3.0 eV for Ge0.30As0.04S0.66). The obtained experimental results are discussed taking into account the light absorption depending on the nature and the thickness of each amorphous layer, on the wavelength, and on contact phenomena at the interfaces between different layers and between the amorphous layers and the metal electrodes with different work functions.
