BACKGROUND Lumbar discitis caused by Clostridium perfringens is extremely rare. There have only been 7 published cases of confirmed discitis caused by Clostridium perfringens. We write this report to underscore this unusual relationship by discussing an additional case and providing a review of the previously published cases so clinicians can adequately evaluate and treat patients presenting with discitis. CASE REPORT A 64-year-old morbidly obese man presented with an acute onset of worsening back pain and generalized weakness after incurring physical trauma related to falling. Additionally, he also developed fever and chills before the presentation. Based on the clinical presentation and elevated serum levels of inflammatory markers, magnetic resonance imaging was ordered, which showed L5-S1 discitis with extension of infection into the epidural space. Fluoroscopy-guided aspiration of the L5-S1 epidural space facilitated the detection of Clostridium perfringens as the involved pathogen. Based on the antibiotic susceptibility report, the patient was treated with intravenous ampicillin for 8 weeks, after which his symptoms resolved. CONCLUSIONS Diagnosis of discitis can be very challenging due to its ambiguous clinical presentation, especially in the elderly population due to the presence of underlying degenerative changes. Even though Clostridium perfringens remains a rare cause of lumbar discitis, it should be considered as a pathogen capable of causing infection of the vertebrae and intervertebral discs, thus allowing clinicians to make necessary diagnostic evaluations to provide appropriate targeted treatment to patients presenting with discitis.BACKGROUND The aim of the study was the cross-cultural adaptation and validation of the Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians. MATERIAL AND METHODS The process consisted of the following stages forward translation, backward translation, analysis by committee of experts, pretest of Polish-language version of questionnaire, and cross-cultural validation psychometric evaluation. In the last step, a convenience sample of 31 professional orchestra musicians (professional experience 16.7±9.5 years) was used to validate the new tool. The primary study outcome measurements were construct validity and internal consistency as measured by exploratory factor analysis (EFA) and Cronbach's alpha, respectively. RESULTS A final Polish version of the questionnaire was developed. Using EFA, the 2-factor structure (pain intensity and pain interference) was obtained, taking into account all 9 items, explaining about 76% of the total variance. The next step would be to estimate cutoff values for severity of health status in orchestra musicians.BACKGROUND This randomized controlled trial aimed to investigate the effect of everolimus (EVL) with low-dose tacrolimus (Tac) on the development of post-transplantation diabetes mellitus (PTDM) in kidney transplantation (KT). MATERIAL AND METHODS Seventy-seven kidney transplant patients from 4 transplant centers were included. Patients were randomized to the "EVL group" (n=38) and the "TAC group" (n=39). The target Tac trough level was 2 to 5 ng/mL in the EVL group and 5 to 10 ng/mL in the TAC group. RESULTS The 1-year cumulative incidence of PTDM in all patients was 7.8%, and no difference was found between the 2 groups (P=0.0819). Insulin resistance measured with the homeostatic model assessment for insulin resistance showed a significant increase only in the TAC group (1.11 to 1.30, P=0.0492). Allograft rejection rate and estimated glomerular filtration rate (eGFR) follow-ups every 3 months were not significantly different between the 2 groups. However, the EVL group showed a significant increase in the mean eGFR at 9 months and 12 months after KT compared to the baseline value (P=0.0242 and 0.0491, respectively). The EVL group showed lower insulin resistance and higher allograft function in comparison to the TAC group. CONCLUSIONS EVL-based immunosuppressive therapy with lower Tac exposure could be a safer alternative for maintenance treatment.

We investigated the effects of water extracts of

(WEAA) in C3H10T1/2, a mesenchymal stem cell line, by measuring the level of intracellular fat accumulation and the expression of genes associated with adipocyte differentiation. We also evaluated anti-obesity effects of WEAA in Zucker rats, a genetic model for the study of obesity, and in Sprague Dawley rats with high-fat diet (HFD)-induced obesity.
