We aimed to investigate molar enamel development in fossil orangutans from Guangxi and shed light on the evolution of Asian great apes.

We collected 32 fossil orangutan molars, most of which were from Guangxi apothecaries and the Guangxi Daxin Heidong cave, and prepared histological sections of each molar. We then characterized aspects of dental development, including long period line periodicity, number of Retzius lines and lateral enamel formation time, cuspal enamel thickness, and enamel formation time.

The long period line periodicity in fossil orangutans ranged from 9 to 10 days (mean, 9.09 days). The molar lateral enamel formation time ranged from 1.48 to 3.17 years (540-1,152 days). Current recommendations suggest using short-acting anaesthetic agents in preference to standard agents to reduce this risk, but there is currently no evidence to support this. This randomised controlled triple-blind trial tested the hypothesis that a combination of short-acting agents (desflurane-remifentanil) would reduce the postoperative impact of general anaesthesia on sleep apnoea severity compared with standard agents (sevoflurane-fentanyl). Sixty patients undergoing hip arthroplasty under general anaesthesia were randomised to anaesthesia with desflurane-remifentanil or sevoflurane-fentanyl. Respiratory polygraphy was performed before surgery and on the first and third postoperative nights. The primary outcome was the supine apnoea-hypopnoea index on the first postoperative night. Secondary outcomes were the supine apnoea-hypopnoea index on the third postoperative night, and the oxygen desaturation index on the first and third postoperative nights. Additional outcomes included intravenous morphine equivalent consumption and pain scores on postoperative days 1, 2 and 3. Pre-operative sleep study data were similar between groups. Mean (95%CI) values for the supine apnoea-hypopnoea index on the first postoperative night were 18.9 (12.7-25.0) and 21.4 (14.2-28.7) events.h-1 , respectively, in the short-acting and standard anaesthesia groups (p = 0.64). These data should prompt an update of current recommendations.The 2004 Indian Ocean tsunami was an international natural disaster unlike any seen before, killing 166,561 people in Aceh province, Indonesia. It prompted an unprecedented humanitarian response and was a catalyst in ending almost 30 years of civil conflict in Aceh. Since the tsunami was followed by a multitude of events, we first conduct a systematic review to identify those events in Indonesia. We then use a synthetic control method to estimate the combination of those effects on child mortality indicators in Aceh for the 13 years that followed the disaster using data from 258,918 children born between 1990 and 2017. The results show a significant increase in under-5 mortality only the year after the tsunami and no effect in the medium term. However, younger and older children were affected differently in the medium term. In fact, we show a decrease in child mortality among children aged 1-4 years. In contrast, we observe an increase in mortality among children under-1 in 2009 and 2010. Overall, the resilience of Aceh province points to the importance of coordinated international disaster responses after natural disasters.The aim of this work is to estimate the area-under the blood concentration curve of tacrolimus (TAC) following b.i.d. or q.d. dosing in organ transplant patients, using Xgboost machine learning (ML) models. A total of 4,997 and 1,452 TAC interdose area under the curves (AUCs) from patients on b.i.d. and q.d. TAC, sent to our Immunosuppressant Bayesian Dose Adjustment expert system (www.pharmaco.chu-limoges.fr/) for AUC estimation and dose recommendation based on TAC concentrations measured at least at 3 sampling times (predose, ~ 1 and 3 hours after dosing) were used to develop 4 ML models based on 2 or 3 concentrations. For each model, data splitting was performed to obtain a training set (75%) and a test set (25%). The Xgboost models in the training set with the lowest root mean square error (RMSE) in a 10-fold cross-validation experiment were evaluated in the test set and in 6 independent full-pharmacokinetic (PK) datasets from renal, liver, and heart transplant patients. ML models based on two or three concentrations, differences between these concentrations, relative deviations from theoretical times of sampling, and four covariates (dose, type of transplantation, age, and time between transplantation and sampling) yielded excellent AUC estimation performance in the test datasets (relative bias less then 5% and relative RMSE less then 10%) and better performance than maximum a posteriori Bayesian estimation in the six independent full-PK datasets. The Xgboost ML models described allow accurate estimation of TAC interdose AUC and can be used for routine TAC exposure estimation and dose adjustment. They will soon be implemented in a dedicated web interface.

Persons with dementia frequently demonstrate distress behaviors in dementia (DBD), associated with poorer outcomes. This study aimed to create a measure of DBD from routinely administered Minimum Data Set (MDS 3.0) behavior section items that demonstrated sensitivity to change, for evaluation of intervention efforts for VA Community Living Center (CLCs) residents exhibiting DBD.

CLC residents with DBD (n = 302) were enrolled in an interdisciplinary behavioral intervention between 2013 and 2017.

A factor analysis of MDS behavior section items from assessments closest to baseline was conducted. Internal consistency, hypothesized associations between MDS factors and clinical measures, and sensitivity to detect change over time was explored.

Residents were assessed at baseline and post-intervention using the MDS behavior section items and a validated clinical measure of DBD.

The Distress Behavior in Dementia Indicator (DBDI) was created as a consistent factor with internal consistency, and was related to a validated measure as predicted at baseline and post-intervention. Sensitivity to change was demonstrated by using change score correlations (r = 0.40-0.50), effect size (d = 0.63), and reliable change indices.

The DBDI is recommended for routine use in CLCs to evaluate impact of intervention effectiveness and provide quality improvement feedback.

Targeted enrichment methods facilitate sequencing of hundreds of nuclear loci to enhance phylogenetic resolution and elucidate why some parts of the "tree of life" are difficult (if not impossible) to resolve. The mimosoid legumes are a prominent pantropical clade of ~3300 species of woody angiosperms for which previous phylogenies have shown extensive lack of resolution, especially among the species-rich and taxonomically challenging ingoids.

We generated transcriptomes to select low-copy nuclear genes, enrich these via hybrid capture for representative species of most mimosoid genera, and analyze the resulting data using de novo assembly and various phylogenomic tools for species tree inference. We also evaluate gene tree support and conflict for key internodes and use phylogenetic network analysis to investigate phylogenetic signal across the ingoids.

Our selection of 964 nuclear genes greatly improves phylogenetic resolution across the mimosoid phylogeny and shows that the ingoid clade can be resolved into several well-supported clades. However, nearly all loci show lack of phylogenetic signal for some of the deeper internodes within the ingoids.

Lack of resolution in the ingoid clade is most likely the result of hyperfast diversification, potentially causing a hard polytomy of six or seven lineages. The gene set for targeted sequencing presented here offers great potential to further enhance the phylogeny of mimosoids and the wider Caesalpinioideae with denser taxon sampling, to provide a framework for taxonomic reclassification, and to study the ingoid radiation.
