Novel laryngotracheal wound coverage devices are limited by complex anatomy, smooth surfaces, and dynamic pressure changes and airflow during breathing. We hypothesize that a bioinspired mucoadhesive patch mimicking how geckos climb smooth surfaces will permit sutureless wound coverage and also allow drug delivery.

Polycaprolactone (PCL) fibers were electrospun onto a substrate and polyethylene glycol (PEG) - acrylate flocks in varying densities were deposited to create a composite patch. Sample topography was assessed with laser profilometry, material stiffness with biaxial mechanical testing, and mucoadhesive testing determined cohesive material failure on porcine tracheal tissue. Degradation rate was measured over 21 days in vitro along with dexamethasone drug release profiles. Increased flocking density was inversely related to cohesive failure in mucoadhesive testing, with a flocking density of PCL-PEG-2XFLK increasing failure strength to 6880 ± 1810 Pa compared to 3028 ± 791 in PCL-PEG-4XFLK density and 1182 ± 262 in PCL-PEG-6XFLK density. The PCL-PEG-2XFLK specimens had a higher failure strength than PCL alone (1404 ± 545 Pa) or PCL-PEG (2732 ± 840). Flocking progressively reduced composite stiffness from 1347 ± 15 to 763 ± 21 N/m. concentration was maintained over 21 days. Optimized PCL-PEG-2XFLK density flocked patches were easy to maneuver endoscopically in laryngeal evaluation.
This novel, sutureless, patch is a mucoadhesive platform suitable to laryngeal and tracheal anatomy with drug delivery capability.

N/A Laryngoscope, 2020.Community participation can be considered a pillar for the promotion of social justice and well-being for immigrants in new countries. Participation may be influenced by different forms of oppression which decrease opportunities for immigrants to be engaged. The present study explores the difficulties that Peruvian immigrants encountered and still encountering to participate in Santiago de Chile through in-depth qualitative interviews. Eighteen semi-structured interviews were conducted with Peruvian leaders of Ethnic Community Based Organizations (ECBOs) in Santiago de Chile. Interviews focused on the community engagement of Peruvians highlighting the difficulties they encountered when deciding whether to engage and throughout the process of carrying out their commitment, along with their perceptions when trying to engage their compatriots. The present study contributes to the literature in three aspects. First, it focused on the phenomenon of South-South migration. Secondly, it delved into the psychological and structural barriers that immigrants' experience, considering their disadvantaged conditions. Thirdly, it used Situational Analysis, along with the constructionist drift of Grounded Theory, which is widely used in critical, qualitative research, and is sensitive to producing situated knowledge. Coding and mapping analysis identified experiences related to historical trauma, transnational bonds, and dominant master narratives in both countries as well as challenges due to balancing time and priorities, surviving institutional deterrents, and inter-organizations competitiveness. Finally, transnational commitments, mechanisms of social disconnection, and under valuated rights that Peruvians may live in Chile were pointed out. These results intend to have practical implications for immigrants and for community psychologists.
To evaluate the long-term swallowing outcomes after surgical treatment for chronic aspiration in patients treated with radiotherapy for head and neck cancer.
This was a retrospective study.
The data of patients who underwent radiotherapy for head and neck cancer and who subsequently required a laryngectomy or a tubed supraglottic laryngeal closure (TSLC) for recurrent aspiration pneumonia between 2004 and 2017 were retrieved from a tertiary referral hospital dysphagia clinic. The Functional Oral Intake Scale (FOIS) and the Swallowing Performance and Status Scale (SPSS) were used to assess swallowing function.
Of the 17 patients who required surgery for chronic aspiration secondary to radiotherapy for head and neck cancer, two underwent a laryngectomy and 15 a TSLC. During a mean follow-up of 77 months, the FOIS and SPSS scores significantly improved at 12, 24, and 36 months after laryngectomy and TSLC relative to the baseline (P < .05). Both patients who underwent laryngectomy and 11 of the 15 (73.3%) who underwent a TSLC resumed oral feeding. Both laryngectomy patients had episodes of recurrent aspiration pneumonia after surgery due to leakage through the tracheoesophageal puncture or prosthesis, whereas none of the TSLC patients had these episodes.
A tubed supraglottic laryngeal closure, which is a reversible procedure that preserves the larynx and allows for natural phonation, should be considered an alternative to laryngectomy for the control of chronic aspiration.

To evaluate the comparative effectiveness of external facilitation (EF) vs external+internal facilitation (EF/IF), on uptake of a collaborative chronic care model (CCM) in community practices that were slower to implement under low-level implementation support.
Primary data were collected from 43 community practices in Michigan and Colorado at baseline and for 12months following randomization.
Sites that failed to meet a pre-established implementation benchmark after six months of low-level implementation support were randomized to add either EF or EF/IF support for up to 12months. Key outcomes were change in number of patients receiving the CCM and number of patients receiving a clinically significant dose of the CCM. Moderators' analyses further examined whether comparative effectiveness was dependent on prerandomization adoption, number of providers trained or practice size. Facilitation log data were used for exploratory follow-up analyses.
Sites reported monthly on number of patients that had received the CCM.
