However, batch chromatography has the disadvantage of ineffective solvent utilization, under-utilization of resin, and low throughput. In this work, we overcome these challenges by using a 3-column Periodic Counter-current Chromatographic (PCC) system for continuous refolding of IBs, formed during the production of L-asparaginase by recombinant E. coli cultures. Initial experiments were conducted in batch processes using single-column immobilized metal-affinity chromatography. Different gradient operations were designed to improve the protein loading for the single-column, batch-MAR processes. Optimized conditions, based on the batch-MAR experiments, were used for designing the continuous-MAR processes using the PCC system. The continuous-MAR experiments were carried out over 3 cycles (∼ 30 h) in the PCC system. A detailed quantitative comparison based on recovery, throughput, buffer consumption, and resin utilization was made for the three modes of operation pulse-dilution, single-column batch-MAR, and 3-Column PCC-based continuous-MAR processes. While recovery (73%) and throughput (11 mg/h) were the highest in PCC, specific buffer consumption (6.9 ml/mg) was the least. Also, during PCC operation, resin utilization improved by 92% in comparison to the single-column batch-MAR process. Among the strategies proposed to provide a more balanced trade-off between bleeding and ischemic events at a single patient level, a guided selection of P2Y12 inhibitors, by using platelet function or genetic testing, has shown promising results. In this review, we provide a focused summary of the rationale and evidence on the use of platelet function and genetic testing-guided antiplatelet therapy, and we explore the implications for their use in the modern setting of patients undergoing PCI.
Inguinal lymph node dissection (ILND) is an essential component in the diagnosis, management, and treatment of penile cancer. Recent advances in minimally invasive surgery may play an important role in decreasing the adverse effects and complications of lymph node dissections. We present our technique utilizing a single-port (SP) robot assisted laparoscopic bilateral ILND in a patient with pT3N2Mx penile cancer s/p partial penectomy and sentinel lymph node biopsy.
We present a case of a 64-year-old man who underwent a radical penectomy for previously diagnosed penile cancer. Pathology report showed invasive squamous cell carcinoma of the penis. In accordance with NCCN guidelines, we performed a bilateral inguinal and pelvic lymph node dissection using robotic assisted SP laparoscopy with the DaVinci Single-Site platform. Our methods are detailed in this technical report.
Total operative time was 3 hours and 38 minutes in duration with minimal blood loss (<20 mL). A 3 cm inguinal lymph node was excised and positive for malignancy without involvement of other nodes. The patient was discharged 90 minutes after recovery in PACU without narcotics and returned to normal bowel function within 6 hours.

However, VA-ECLS increases left ventricular (LV) afterload, resulting in greater myocardial oxygen demand, which can impair myocardial recovery and worsen pulmonary edema. These complications can be ameliorated by various LV venting strategies to unload the LV. Evidence suggests that LV venting improves outcomes in VA-ECLS, but there is a paucity of randomized trials to help guide optimal strategy and the timing of venting. In this review, we discuss the available evidence regarding LV venting in VA-ECLS, explain important hemodynamic principles involved, and propose a practical approach to LV venting in VA-ECLS.
We sought to develop machine learning (ML) models trained on administrative data which predict risk of readmission in patients with heart failure and to evaluate and compare the ML model with the currently used LaCE score using clinically informative metrics.
This prognostic study was conducted in Alberta, Canada, on 9845 patients with confirmed heart failure admitted to hospital between 2012 and 2019. The outcome was unplanned all-cause hospital readmission within 30 days of discharge. We used 80% of the data for the ML model development and 20% for independent validation. We reported, using the validation set, c-statistics (area under the receiver operating characteristic curves)and performance metrics (likelihood ratio, positive predictive values) for the XGBoost model and a modified LaCE score within their respective predictive thresholds. Boosted tree-based classifiers had higher area under the receiver operating characteristic curves (0.65 for XGBoost) compared with others (0.58 for neural networks)espite predicting readmissions better than the LaCE, even the best ML model trained on administrative health data (XGBoost) did not provide substantially informative prediction performance as it only generated a moderate shift from pre to post-test probability. Health systems wishing to deploy such a tool should consider training ML models with additional data. Adding other techniques like natural language processing, along with ML, to use other clinical information (like chart notes) might improve prediction performance.
Vascular surgery is facing an impending workforce shortage as the population ages and the demand for vascular surgical services increases. The integrated vascular surgery residency (0+5) paradigm is well-established and provides a mechanism to increase the number of board-certified vascular surgeons. Recruitment of medical students to these programs has proven challenging with unfilled positions in each of the past 2 years. The aim of this study is to explore factors that influence medical students' interest in vascular surgery and their decision to ultimately pursue a career in the field.
Medical students listed on the Society for Vascular Surgery "Find a VSIG (Vascular Surgery Interest Group)" webpage were contacted via email to participate in the study. A snowball sampling technique was employed to recruit additional participants, including recent medical school graduates who had matched into a 0+5 program. Fifteen students participated in 5 focus groups. Directed content analysis was employed to quali vascular surgeons, and faculty and student leadership of VSIGs to optimize recruitment programs and increase the vascular surgery workforce.
Medical students pursue a career in vascular surgery based on early exposure to the specialty, experiential learning through hands-on VSIG events, clinical experiences, and longitudinal faculty mentorship. The unique aspects of the specialty, including professional identity and intellectuality, should be highlighted to both attract and maintain students' interest in the field. These findings can be used by national vascular surgery leaders, practicing vascular surgeons, and faculty and student leadership of VSIGs to optimize recruitment programs and increase the vascular surgery workforce.
To describe causes, clinical signs, experience and endovascular techniques for extraction of intravenous foreign bodies (IFB) and literature review.
This retrospective study was based on data collected from the medical records of 51 consecutive patients (26 women and 25 men) treated from July 2007 to May 2020 at a single quaternary center in Brazil and case series with data, published in the literature since 2000 on IFB removal, of at least 5 patients. The average patient age was 43.54 years (range, 2 months to 84 years).
