To date, there is no relevant data supporting the role of salvage radiotherapy (sRT) in patients with refractory or relapsed primary central nervous system lymphoma (PCNSL). Herein, we aimed to investigate the impact of sRT in patients with refractory or relapsed PCNSL following upfront HD-MTX.

We retrospectively reviewed 89 patients who had refractory (n = 16) or recurrent disease after an initial favorable response (n = 73); among them, 41 were treated with sRT and 48 were treated without sRT (nRT). Event-free survival (rEFS) and overall survival (rOS) after first recurrence were considered from the date of recurrence to date of each event.

Overall, the first failure was diagnosed at a median of 11.0months [interquartile range (IQR), 5.6-26.4] after first treatment. More than half of the patients had recurrent disease involving initial tumor bed (n = 47), deep structure (n = 67), and multiple lesions (n = 58). Among 19 patients who were initially treated with 23.4Gy of whole brain RT, 10 patients rece. Further studies of patient selection could stratify patients who can benefit from sRT.

We found favorable response rate and comparable survival outcomes following sRT compared with non-local treatments for patients with refractory/relapsed PCNSL. Further studies of patient selection could stratify patients who can benefit from sRT.Biomass burning for cooking prevalent in the developing countries is an issue which has been a concern for the past several decades for the noxious emissions and subsequent effects on the health of women and children due to the exposure of particulate matter (PM) and other gases. In this study, PM (PM1, PM2.5, and PM10) were measured in biomass-burning households for different communities of Brahmaputra Valley region northeast India by a 31-channel aerosol spectrometer. The levels of emission of PM in the case of different community households were found to be significantly different. Also, the emission characteristics of different cooking time of the day were found to be different across communities. The emission levels in the biomass-burning households were compared with emission in household using "clean" LPG fuel, and it was found that the biomass fuels emitted 10-12 times more PM2.5 and 6-7 times more PM10. The number densities of the emission were found to be more with smaller sizes of particulates which could explain why such biomass-burning emissions can pose with greater health risks. The exposure doses were calculated and were found to be about three times higher in biomass-burning houses than "clean" LPG fuel. It is important to note that the exposure from biomass burning while cooking has a gender perspective. The woman of the house generally takes care of the activities in the kitchen and get exposed to the noxious PM and the gases. Children often accompany their mothers and face the same fate. The subterranean insect Bradysia cellarum Frey (Diptera Sciaridae) is a notorious and major pest of Chinese chives, Allium tuberosum Rottler ex Sprengle (Amaryllidaceae) in China. Current chemical control of B. cellarum results in low insecticide efficacy, high cost and pesticide resistance, therefore there is an urgent need for sustainable management. Here, greenhouse experiments were conducted to evaluate the potential biocontrol agent Stratiolaelaps scimitus Womersley (Acari Laelapidae) against B. cellarum. The number of B. cellarum larvae in soil declined from 17.6 to 0 in 4 months after releasing predatory mites in high density (5,000 adults per row); treatment was less effective under low densities of 2500 adults per row. To determine whether S. scimitus can be used in combination with soil solarization by film mulching over 40 °C for 4 h, we also evaluated heat tolerance of S. scimitus in laboratory and its control efficacy against B. cellarum after high-temperature treatment mimicking the film mulching in greenhouse. As our results showed that egg hatchability of S. scimitus was 2.6% at 38 °C and adult survival rate was 2% at 40 °C for 4 h, respectively, we concluded S. scimitus was largely inviable and could not reproduce at 40 °C. This temperature was the baseline of soil solarization, suggesting predatory mites should be released after soil solarization. When using S. scimitus after soil solarization or when using soil solarization as single treatment, fly larvae declined similarly from initial density of 18 to 0 or 17.2 to 0, respectively, within a month. Thus, our study suggests the potential of S. scimitus as a biocontrol agent of B. cellarum in greenhouse, and the most effective strategy is to combine film mulching and predatory mites (after soil heating) to control B. cellarum in chive productions. The epidemics of obesity and type 2 diabetes have led to intensive investigation of the underlying mechanisms of these diseases and their main complications such as cardiovascular diseases and non-alcoholic fatty liver disease. This search has contributed to better understand how organs and tissues communicate with each other in the so-called inter-organ crosstalk. Adipose tissue, the liver, or skeletal muscle can actively release secreted factors termed "organokines" which can interact with other distant targets in complex networks. More recently, other novel mediators of inter-organ crosstalk such as extracellular vesicles and their non-traditional cargoes as miRNAs and lncRNAs are gaining importance and represent potential therapeutic targets. In the present chapter we summarize some of the current knowledge on inter-organ communication with a focus on adipose tissue-released factors and their modulation on other organs and tissues like pancreas, liver, skeletal muscle, the cardiovascular system, and the gut in the context of obesity and its progression to insulin resistance. We also provide a perspective on mediators of inter-organ crosstalk as potential therapeutic targets.Elsholtzia rugulosa Hemsl., a species of the Labiatae family, has a long history of use as a honey plant, herbal tea, and folk medicine in China. However, little is known about its composition and biological activities. The present study aimed to investigate the total phenol and flavonoid contents, phytochemical composition, and multiple biological activities of this plant. The total flavonoid content of the ethyl acetate fraction (EAF) was higher than those of the petroleum ether fraction (PEF), n-butanol fraction (NBF), and water fraction (WF). The EAF also had much stronger antioxidant, cytotoxic, hepatoprotective, and acetylcholinesterase (AChE) and α-glucosidase inhibitory activities than the PEF, NBF, and WF. More importantly, the IC50 values of the EAF and NBF against α-glucosidase were much lower than that of the positive control acarbose, indicating their potent α-glucosidase inhibitory activities. The isolation of the EAF led to the acquisition of 9 compounds, four of which (β-daucosterol, methyl rosmarinate, betulinic acid, and oleanolic acid) possessed significant α-glucosidase inhibitory activities. Maltol 6'-O-(5-O-p-coumaroyl)-β-D-apiofuranosyl-β-D-glucopyranoside and rosmarinic acid were the major phenolic compounds in the EAF according to the HPLC-DAD analysis. All these findings indicate that the EAF, NBF, and some isolated compounds have the potential to be developed as antidiabetic drugs. Moreover, the dual inhibition of AChE and butyrylcholinesterase (BChE) of certain fractions indicates their potential in the development of anti-Alzheimer's disease drugs. The present study provides a new understanding of the phytochemistry and bioactivity of E. rugulosa.Today, problems of treating wastewater spent caustic produced by refinery units with high toxic compounds and chemical oxygen demand (COD) become concerns of many managers of these industries and environmental experts. Hence, emission without application of treatment methods will have adverse environmental impacts. In this study, two direct acid neutralization (DAN) and wet air oxidation (WAO) processes were selected to treatment the wastewater of the Bandar Abbas oil refinery in southern Iran. The aim was to reduce COD and harmful substances, compare the two methods, optimize processes, and evaluate their performance. Experimental experiments were performed in a reactor system with different input variables related to two different methods. Parameter optimization was performed based on Box-Behnken (BBD) method and Design Expert software. The analysis of the results based on statistical methods and response procedure diagrams was used to evaluate the status of COD changes to the parameters. The best operating conditions of temperature, pressure, residence time, and stoichiometric coefficient of air were 148.269 °C, 15.716 bar, 3.563 h, and 8.415 l/h respectively in WAO with 68% reduction in COD, and for DAN process, temperature, pH, and agitation speed were 30.082 °C, 2.008, and 203.672 rpm, respectively, with 43% reduction in COD. Results of rapid impact assessment matrix (RIAM) showed that WAO process with a higher score is a more environmentally friendly method and DAN process has been considered by experts due to its popularity, ease of testing, less equipment requirements, and lower cost.Appendiceal primary peritoneal surface malignancies are rare and include a broad spectrum of pathologies ranging from indolent disease to aggressive disease. As such, the data that drive the management of appendiceal peritoneal surface malignancies is generally not based on prospective clinical trial data, but rather consists of level 1 data based on retrospective studies and high-volume institutional experiences. this website Complete surgical debulking typically offers the best chance for long-term survival. This review highlights the landmark articles on which management of primary appendiceal peritoneal surface malignancies are based.

Merkel cell polyomavirus (MCPyV) is associated with the development of Merkel cell carcinoma (MCC). Antibody (MCPyV-Ab) titers may have prognostic implications. This study evaluated the impact of the presence or absence of MCPyV-Ab on the 2-year overall survival (OS) and disease-free survival (DFS) of MCC patients.

This single-center, IRB-approved, retrospective cohort study evaluated 51 adult patients with MCC from 2014 to 2021 using a prospectively maintained database. Patients were compared by MCPyV-Ab status, and Kaplan-Meier analysis was used to evaluate 2-year OS and DFS.

Of the 51 patients, 13 (25.4%) were seropositive, 41 (80.4%) underwent wide excision, 40 (80.0%) received radiotherapy, and 43 (84.3%) received multimodal therapy. The median follow-up period was 15.5 months (range 1-69.5 months). The median 2-year OS of the entire cohort was not reached. The median 2-year OS was not reached for either the seronegative or the seropositive patients. The difference in 2-year OS between the groups wtudy substantiated the value of MCPyV-Ab assessment for MCC.

Spontaneous angiogram-negative subarachnoid hemorrhage (SAH) is considered a benign illness with little of the aneurysmal SAH-related complications. We describe the clinical course, SAH-related complications, and outcome of patients with angiogram-negative SAH.

We retrospectively reviewed all adult patients admitted to a neurosurgical intensive care unit during 2004-2018 due to spontaneous angiogram-negative SAH. Our primary outcome was a dichotomized Glasgow Outcome Scale (GOS) at 3 months. We assessed factors that associated with outcome using multivariable logistic regression analysis.

Of the 108 patients included, 84% had a favorable outcome (GOS 4-5), and mortality was 5% within 1 year. The median age was 58 years, 51% were female, and 93% had a low-grade SAH (World Federation of Neurosurgical Societies grading I-III). The median number of angiograms performed per patient was two. Thirty percent of patients showed radiological signs of acute hydrocephalus, 28% were acutely treated with an external ventricular drain, 13% received active vasospasm treatment and 17% received a permanent shunt.
