Since the outbreak of the coronavirus pandemic in 2019 (COVID-19), healthcare systems around the world have been hit to varying degrees. As a neurologist team, for patients with acute ischemic stroke (AIS), we compared the situations of intravenous thrombolysis (IVT) treatment from 2019 to 2020 to investigate the influence of COVID-19 pandemic on the attendance and prognosis of the IVT patients.
We collected the messages of objects who had received IVT (Bridging surgery was ruled out) during 2019-2020. We analyzed differences in age, gender, time from onset to start IVT, door to needle time (DNT), pretreatment NIHSS score, postoperative NIHSS score, and so on. Statistical tests were also performed to respectively compare the discharged modified Rankin score (mRS) and discharged NIHSS score between two years.

The follow-up is the development of the double-constrained correspondence analysis (dc-CA), integrating traits as species-related predictors. Further, methods have been proposed to integrate information on phylogenetic relationships and space variability. We expand this framework, proposing a dc-CA-based algorithm for decomposing variation in community structure and testing the simple and conditional effects of four sets of predictors environment characteristics and space configuration as predictors related to sites, while traits and niche (dis)similarities as species-related predictors. In our approach, ecological niches differ from traits in that the latter are distinguished by and characterize the individual level, while niches are measured on the species level, and when compared, they are characteristics of communities and should be used as separate predictors. The novelties of this approach are the introduction of new niche parameters, niche dissimilarities, synthetic niche-based diversity which we related to environmental features, the development of an algorithm for the full variation decomposition and testing of the community-environment-niche-traits-space (CENTS) space by dc-CAs with and without covariates, and new types of diagrams for the results. see more Applying these methods to a dataset on freshwater mollusks, we learned that niche predictors may be as important as traits in explaining community structure and are not redundant, overweighting the environmental and spatial predictors. Our algorithm opens new pathways for developing integrative methods linking life, environment, and other predictors, both in theoretical and practical applications, including assessment of human impact on habitats and ecological systems.
The association between advanced age and postoperative morbidity and mortality after major gastroenterological surgeries remains unclear. This study aimed to assess the association between old age and the short-term postoperative outcomes of gastroenterological surgeries.
We evaluated 327,642 patients who underwent any of the seven major gastroenterological surgeries-esophagectomy, total gastrectomy, distal gastrectomy, right hemicolectomy, low anterior resection, hepatectomy, and pancreatoduodenectomy-and were registered with the Japanese national surgical registry between January 2011 and December 2013. Perioperative characteristics, frequency/nature of postoperative morbidities, and postoperative mortality were compared according to age at the time of surgery.
Overall, 18% (59,182/327,642) of the entire cohort were aged ≥ 80years. The overall mortality rates in the entire cohort and in those aged ≥ 80years were 1.7% and 3.3%, respectively. The postoperative mortality increased with increasing age forhe mortality risk should be carefully considered before recommending major gastroenterological surgeries for older patients.This observational study done during April-December 2020 at a tertiary-care hospital in Haryana (India) enrolled 152 SARS-CoV-2-exposed neonates. Among them, 150 neonates had perinatal SARS-CoV-2 exposure and 2 neonates had late postnatal exposure. Stable infant-mother dyads were roomed-in with precautions to support breastfeeding. Nasopharyngeal swabs collected from neonates were tested for SARS-CoV-2 by reverse transcriptase-polymerase chain reaction (RT-PCR) test. There was a high incidence of prematurity (23%), low birth weight (31%), intrauterine fetal distress (16%), perinatal asphyxia (6%), admission to neonatal intensive care unit (24%), and mortality (1.3%) among neonates with perinatal SARS-CoV-2 exposure. In this sub-group, 20 neonates tested positive for SARS-CoV-2 in nasopharyngeal swab sample(s). Although emphasis has recently been placed on the importance of diet high in plant-based foods, the association between plant-based diet and long-term risk of overall and cause-specific mortality has been less studied. We aimed to investigate whether plant-based diet was associated with lower death risk.
This prospective cohort study used data from the US National Health and Nutrition Examination Survey. Diet was assessed using 24h dietary recalls. We created three plant-based diet indices including an overall plant-based diet index (PDI), a healthful plant-based diet index (hPDI), and an unhealthful plant-based diet index (uPDI). Deaths from baseline until December 31, 2015, were identified. Multivariable-adjusted hazard ratios (HRs) and 95% confidence intervals (CIs) were calculated using Cox regression.
We documented 4904 deaths among 40,074 participants after a median follow-up of 7.8years. Greater adherence to PDI was associated with lower risk of overall (HR comparing extreme quintiles 0.80, 95% CI 0.73, 0.89, p
< 0.001) and cancer-specific (HR = 0.68, 95% CI 0.55, 0.85, p
< 0.001) mortality. These inverse associations remained for hPDI and overall mortality with a HR of 0.86 (95% CI 0.77, 0.95, p
= 0.001), but not for cancer or CVD mortality. Conversely, uPDI was associated with higher risk of total (HR = 1.33, 95% CI 1.19, 1.48, p

The increase in PM2.5 above the background level around a mine represents a potential disturbance to caribou. To quantify the spatial distribution of the elevated PM2.5, we investigated three different sampling schemes to measure PM2.5 concentration using a portable monitor. We found that the best sampling scheme was to use the regional background PM2.5 as the reference and analyze the anomaly of PM2.5 measured at sites around the mine complex from the background level. The regional background PM2.5 values were measured at the Daring Lake Tundra Research Station during 2018 and 2019. Our results indicated that the background PM2.5 was not a low and constant value but varied with rain events, wind direction, and the impacts of forest W-SE axis. A similar result was found in the downwind direction when the wind came from north of the NW-SE axis, with Zepm = 4.4 km. In the upwind direction, the value of Zepm was much smaller, being 0.7 km (or 1.0 km) when the wind came from the north (or south) of the NW-SE axis. For the period of tar between 96 and 192 hours, Zepm further expanded to 21.2 km when the wind from the south of the NW-SE axis. The results from this study indicated that this reference paradigm that uses the regional background PM2.5 as the reference in combination with a portable PM2.5 monitor worked well for quantifying the tempo-spatial patterns of PM2.5 at locations in remote and mostly pristine Arctic. However, their effectiveness for other regions needs further investigation.
To establish the CQS inter-rater reliability and rating time and to compare both against that of the Jadad scale and Cochrane's Risk of Bias Tool (ROBT).
Four independent raters rated 45 trial reports. The inter-rater reliability was established by use of the Brennan-Prediger coefficient (BPC). The coefficients were compared using the two-sample z-test. Secondary analysis included comparison of the inter-rater reliability of the randomization component of all tools, as well as of the allocation concealment component of the CQS to that of the ROBT. The mean rating time with standard deviation (SD) for each tool was determined using one-way repeated measures analysis of variance. Post hoc comparisons were made using the Tukey-Kramer adjustment for three pair-wise multiple comparisons.
The inter-rater reliability was significantly higher for the CQS (BPC, 95% CI 0.95, 0.87-1.00) compared to Jadad (0.70, 0.58-0.82) (adjusted p = 0.0005) and most components of ROBT. The mean (SD) time to complete the CQS (4.0 (1.0) min) did not differ significantly from that of the Jadad scale 4.8 (1.1) min (adjusted p = 0.11), but was significantly shorter compared to that of the ROBT 15.3 (5.9) min (adjusted p < 0.0001).
The results suggest the CQS to be a very reliable and fast trial appraisal tool. Clinical relevance The higher the inter-rater reliability, the higher the probability that trial results reflect therapeutic truth. The CQS will need to take further bias sources into consideration, in order to increase its utility.

MGL-3153 gene of Malassezia globosa (M. globosa) encodes a protein which is homologous of the M. restricta, M. sympodialis, M. Pachydermatis, and U. maydis GpiPs. Real-time PCR assay showed that the expression of MGL_3153 gene was significantly up-regulated among M. globosa isolated from patients with pityriasis versicolor (PV) compared to a healthy individual, suggesting the contribution of this gene in the virulence of M. globosa. Accordingly, the sequence of this protein was analyzed by bioinformatics tools to evaluate the structure of that. The conservation analysis of MGL-3153 protein showed that the C-terminal region of this protein, which is responsible for GPI-anchor ligation, was highly conserved during evolution while the N-terminal region just conserved in Malassezia species.
