The purpose of this paper was to examine changes in situational temptations to smoke among women in early to late pregnancy enrolled in a texting trial to help them quit smoking. We compared changes between (1) intervention arms, (2) those who quit, (3) those who reduced by 50% or more, and (4) those who reduced by less than 50%. We also examined cravings overtime in the intervention arm and the relationship between real-time cravings assessed via text message and situational temptations.

This was a secondary analysis of the Baby Steps trial, a randomized controlled trial testing the efficacy of a text-based scheduled gradual reduction (SGR) intervention on cessation. We used t-tests to examine changes across intervention arms and repeated measured proc mixed to explore changes in situational temptations and cravings.

Among all women, situational temptations decreased from early to late pregnancy for the positive, negative, and habitual subscales, (ps < 0.001). We found no difference in situational temptations across arms. We found a positive relationship between negative situational temptation and average craving during the Weeks 2 and Weeks 3 of the intervention. Negative ST increased by 0.11 for each unit increase of craving at Week 2.

As women progress through pregnancy their temptation to smoke reduces. A different relationship might exist, however with negative affect situations in which women reported higher craving but not in response to other temptations. Future work might have a particular focus on the intersection of negative affect with cravings and temptations to promote cessation during pregnancy.

NCT01995097.Patients with depression who ruminate repeatedly focus on depressive thoughts; however, there are two cognitive subtypes of rumination, reflection and brooding, each associated with different prognoses. Reflection involves problem-solving and is associated with positive outcomes, whereas brooding involves passive, negative, comparison with other people and is associated with poor outcomes. Rumination has also been related to atypical functional hyperconnectivity between the default mode network and subgenual prefrontal cortex. Repetitive pulse transcranial magnetic stimulation of the prefrontal cortex has been shown to alter functional connectivity, suggesting that the abnormal connectivity associated with rumination could potentially be altered. This study examined potential repetitive pulse transcranial magnetic stimulation prefrontal cortical targets that could modulate one or both of these rumination subtypes. Forty-three patients who took part in a trial of repetitive pulse transcranial magnetic stimulation completed the Rumination Response Scale questionnaire and resting-state functional magnetic resonance imaging. Seed to voxel functional connectivity analyses identified an anticorrelation between the left lateral orbitofrontal cortex (-44, 26, -8; k = 172) with the default mode network-subgenual region in relation to higher levels of reflection. Parallel analyses were not significant for brooding or the RRS total score. These findings extend previous studies of rumination and identify a potential mechanistic model for symptom-based neuromodulation of rumination.Intrauterine infections are an urgent problem of modern neonatology. One of the causes of intrauterine infective foetal lesions is physiological immunosuppression. The purpose of this study is to investigate the cytokine status in newborns infected with perinatal infections, depending on their body weight. The study examined 145 newborns. Taking into account their body weight, they were divided into 2 groups main and secondary. The study was conducted in the immunological laboratory of the Medical Centre of Marat Ospanov West Kazakhstan Medical University in the city of Aktobe, with the determination of the level of IgM and IgG to the herpes simplex virus (HSV) types 1, 2, cytomegalovirus (CMV), and chlamydia using the MULTISKANASCENT analyser with the "Chemo" T system. The main results of this study are the predominance of the anti-inflammatory component in both normal weight and underweight infants, which is evidence of the Th-cell-mediated immune response prevalence. The applied value of this study lies in the possibility of applying its results in practice to obtain effective methods to counteract the occurrence and development of intrauterine infections.

Reduced estimated creatinine clearance (eCrCl) is prevalent in older patients and impacts on drug prescription. In this study, the burden of eCrCl reduction and its associated factors and impact on outcomes were analyzed. Moreover, the rate of inappropriate drug prescription according to eCrCl and its impact on outcomes were described.

Data were obtained from "REgistro POliterapie SIMI" (REPOSI), a prospective observational register enrolling hospitalized patients aged ≥65 years. Patients enrolled from 2010-2016 with available data to calculate eCrCl according to the Cockcroft-Gault formula were included in this analysis.

A total of 5046 patients were available for analysis. Among these, we found an eCrCl of 45-59 mL/min in 1163 patients (23.0%), an eCrCl of 30-44 mL/min in 1128 (22.4%), an eCrCl of 15-29 mL/min in 702 (13.9%), and an eCrCl <15 mL/min in 152 (3.0%), with several clinical factors associated with decreasing eCrCl. CP-690550 During follow-up, a progressively higher risk for all-cause death, cardiovascular (CV) death, any death/re-hospitalization, and CV death/re-hospitalization was found across the renal function classes. Among patients with hypertension, diabetes mellitus, atrial fibrillation, coronary artery disease, and heart failure, 476 (10.9%) were inappropriately prescribed medications according to eCrCl. During follow-up, inappropriate prescription was associated with increased risk of all-cause death (odds ratio [OR] 1.49, 95% confidence interval [CI] 1.13-1.97) and any death/re-hospitalization (OR 1.30, 95% CI 1.03-1.63).

In older hospitalized patients, impaired eCrCl is prevalent and associated with several factors, polypharmacy in particular. Patients with reduced eCrCl have a higher risk of major clinical outcomes, and >10% of them are prescribed an inappropriate drug, with a higher risk for major clinical outcomes.

10% of them are prescribed an inappropriate drug, with a higher risk for major clinical outcomes.Groundwater is considered as an imperative component of the accessible water assets across the world. link2 Due to urbanization, industrialization and intensive farming practices, the groundwater resources have been exposed to large-scale depletion and quality degradation. The prime objective of this study was to evaluate the groundwater quality for drinking purposes in Mewat district of Haryana, India. For this purpose, twenty-five groundwater samples were collected from hand pumps and tube wells spread over the entire district. Samples were analyzed for pH, electrical conductivity (EC), total dissolved solids (TDS), total hardness (TH), turbidity, total alkalinity (TA), cations and anions in the laboratory using the standard methods. Two different water quality indices (weighted arithmetic water quality index and entropy weighted water quality index) were computed to characterize the groundwater quality of the study area. Ordinary Kriging technique was applied to generate spatial distribution map of the WQIs. Fouctors explaining 69.81% of the total variance. These factors reveal how processes like rock water interaction, urban waste discharge and mineral dissolution affect the groundwater quality.Herein, we conducted a study of the Zhangsanxi karst groundwater system in Zhangjiajie, Hunan Province, China, and used the results to develop a method for identifying the aquifer medium and its structural characteristics. To begin, rainfall, underground river flow dynamics, and groundwater level dynamics in the Zhangsanxi karst groundwater system were subjected to high-resolution monitoring to elucidate the groundwater system's rainfall-hydrological response characteristics under varying rainfall amounts and intensities. Next, an exponential attenuation curve was employed to analyze how the law of attenuation applies to underground river discharge and groundwater level during a typical rainfall event. Finally, we determined the degree of karst development at different depths, so the data can serve as a reference for local decision makers regarding karst water disaster prevention and water resource utilization. The results show that the flow response lag time in the karst groundwater system depicts good correlation with the rainfall amount but is also affected by the rainfall intensity. link3 Thus, under conditions of identical rainfall, increasing rainfall intensity corresponds to a progressively shorter underground river flow lag time. The area's rainfall can be divided into four types, based on its concentration and intensity characteristics. The underground river flow generally has no evident response to type I rainfall, while the flow response lag time to types II III and IV rainfall is approximately 110, 60, and 40 h, respectively. Furthermore, the Zhangsanxi karst system's water-bearing medium is dominated by karst fissures and pores, which account for about 2/3 of its open space. Large karst conduits and caves account for the remaining 1/3. The degree of karst development in this system depicts evident depth variation but, overall, tends to increase as a function of burial depth.Responsible editor Xianliang Yi.The world's interest now is focusing on the applying of the principles of sustainable development in managing natural resources, especially in managing freshwater, which is one of the greatest challenges the whole world face. In this paper, the illite ore was tested to remove the excess of iron and manganese from groundwater which is used for drinking and household purposes in Assiut. To accomplish this goal, the study was based on two directions. The first direction focused on achieving a physicochemical analytical survey for all the groundwater produced by wells in Assiut governorate, and its averages are plotted on GIS maps, to illustrate the difference in the concentrations of iron and manganese. However, the second direction focused on the preparation and examination of the modified illite ore, and it was characterized by using XRD, SEM, FT-IR, and XRF techniques. The results showed that the sodalite-bearing modified illite (SBMI) was able to remove the excess of iron and manganese from raw groundwater with the efficiency of 99% and 97%, respectively.Based on the complexity of cognitive-related influences and the specificity of Chinese liquor culture, this study aimed to explore the associations and potential interactions between demographic characteristics, alcohol consumption, life and atmospheric environment, and cognitive function in seniors through a comprehensive analysis, in order to provide evidence support and feasible recommendations. The study sample was selected from the Chinese Longitudinal Healthy Longevity Survey, which included 40,583 seniors aged 65-115 years. Data analysis and processing were performed by R 4.0.4. The relationship between the factors and cognition was modeled and analyzed by generalized additive model, and the interaction was explored by combining the ANOVA. The generalized additive model confirmed that alcohol consumption was detrimental to the cognitive status of older adults, especially for liquor (≥ 38°) and beer. The higher the average daily alcohol consumption, the greater the impairment. SO2 and PM2.5 showed the same negative effects.
