Interest in postoperative healthcare utilization has increased following the implementation of episode-of-care funding for elective orthopedic surgery. Most efforts have focused on readmission; however, little has been reported on emergency department (ED) presentation. We analyzed elective, primary total hip or knee arthroplasty (THA and TKA) cases to determine the rate, reasons, risk factors, timing, and hospital cost associated with 30-day ED presentations.

An observational study of patients who underwent primary, elective TKA and THA between January 1, 2016, and December 31, 2017, was performed. The primary outcome was an ED visit within 30-days of the index operation. Secondary outcomes included reasons, risk factors, timing, and hospital cost of ED visits. A multivariable logistic regression was undertaken to determine patient factors associated with ED presentation.

Overall, 1690 patients were included, of which 9.2% presented to the ED within 30-days of surgery. Approximately two-thirds of the v on developing interventions to reduce these visits.

The COVID-19 pandemic has contributed to risk factors for child abuse and neglect and disrupted conventional abuse surveillance.

The goal of this study was to assess how counts of criminal charges have been affected by COVID-19 social distancing measures and related policy changes.

This study used publicly available court filings pertaining to child abuse and neglect from Jan 1, 2010 to June 30, 2020.

Autoregressive integrated moving average (ARIMA) algorithms were constructed with case data from January 2010 to January 2020 to forecast trends in criminal charges for February to June 2020. These forecasted values were then compared to actual charges filed for this time period.

Criminal cases filed between February and June 2020, had an overall 25.7 percent lower average than forecasted. All individual months had progressively lower cases than forecasted with the exception of March. June had the largest deviation from forecasted with 60.1 percent fewer cases than predicted.

Although risk factors for child abuse have increased due to COVID-19, these findings demonstrate a declining trend in child abuse charges. Rather than a decreasing incidence of child abuse and neglect, it is more likely that less cases are being reported. The results warrant immediate action and further investigation in order to address the dangers this pandemic poses for children in abusive situations.

The study prospectively assessed motor development from infancy to adolescence in patients with esophageal atresia (EA).

At one year of age motor performance was evaluated with the Psychomotor Developmental Index (PDI) of the Bayley Scales of Infant Development, Second Edition (BSID-II), and as adolescents reevaluated with Motor Assessment Battery for Children, Second Edition (MABC-2). Associations to clinical factors were assessed.

23 EA patients were followed from infancy to adolescence. The median total PDI score in infancy was 102 (56-118) and the corresponding mean z-score was -0.006 (SD 0.995) and not significantly different from the reference values (p=0.48). The median total MABC-2 score in adolescence was 75 (32-93) and the corresponding mean z-score -0.43 (SD 0.998) which is significantly below normal (p=0.03). Children with impaired motor function in adolescence underwent significantly more rethoracotomies than those with normal motor performance (p=0.037); whereas the two groups did not differ with respect to other clinical characteristics.

From infancy to adolescence the motor performance in the group of EA patients deteriorated from within normal range to significantly impaired compared to reference values. Interdisciplinary follow-up programs from infancy to adolescence with close monitoring for motor function is necessary to detect motor impairments.

Compared to marine sediments, knowledge about the occurrence of MP in freshwater sediments, especially in river sediments, is limited. Although MP concentrations in sediments and soils have been reported in a considerable number of studies, no standardized method is available for sampling and sample processing. Thus, a comparison of results is hardly possible. The present study reviews over 47 articles to evaluate reports of MP in river sediments and current sampling and processing techniques by highlighting various techniques, equipment and approaches for implementing quality assurance and quality control procedures. The authors emphasize that MP quantification techniques could lead to overestimation or underestimation depending on how sampling and sample processing is conducted. Standardization and harmonization of these techniques are crucial to underpin monitoring decisions aimed at safeguarding the ecological integrity of freshwater environments.
