Herein, we present two different routes for the synthesis of the perfluorinated trityl cation, which allowed the handling of the free, uncoordinated species in organic solvents for the first time. The usage of the weakly coordinating anion [Al(OTeF5 )4 ]- and its derivatives allows the characterization of this species by NMR spectroscopy and most importantly by single-crystal X-ray diffraction. The high hydride ion affinity of the cation is shown by hydrogen abstraction from isobutane. A key concept that emerges is the need for practitioners to make a risk-confidence assessment of the particular content of any decision to appropriately modulate their role in the practice of shared decision making.

Since it is unknown whether glucose variability (GV) is increased and whether this is related to worsening of insulin secretion and action in prediabetes, we have assessed insulin secretion and sensitivity, and daily GV in early stages of dysglycemia.

Twenty subjects with normal glucose tolerance (NGT; age 45.0±9.5years; BMI 31.1±6.4kg/m2), 25 with NGT and 1hrOGTT>8.6mmol/L (1hrOGTT; 45.7±8.5years; 32.4±7.0kg/m2), and 59 with isolated impaired glucose tolerance (iIGT; 47.7±11.2years; 31.3±6.1kg/m2) underwent OGTT and MMTT. CGM was performed with blinded FreeStyle Libre Pro for 24h under standard conditions. Parameters of beta-cell function, insulin sensitivity and GV were calculated.

Overall insulin secretion and action as well as GV progressively worsened across glucose tolerance categories. On a matrix analysis, GV parameters were inversely related to ISSI-2; r=-0.37 to -0.52; p<0.0001; and IGI; r=-0.28 to -0.48; p<0.0001 for CV, SD, J-index, LI, HBGI and MAGE. Insulin secretion (IGI) and b-cell function (ISSI-2) emerged as independent contributors to GV in early stage of dysglycemia accounting for about 16%-38% of its variability.

Despite the ubiquity of gaze-related methods in developmental science, current analysis techniques usually involve laborious post hoc coding, imprecise real-time coding, or expensive eye trackers that may increase data loss and require a calibration phase. As an alternative, we propose using computer vision methods to perform automatic gaze estimation from low-resolution videos. At the core of our approach is a neural network that classifies gaze directions in real time. We compared our method, called iCatcher, to manually annotated videos from a prior study in which infants looked at one of two pictures on a screen. We demonstrated that the accuracy of iCatcher approximates that of human annotators and that it replicates the prior study's results. Pulmonary fibrosis was induced by a single dose of bleomycin by intratracheal instillation in adult mice. ATR was administered (20 mg/kg ip) to mice with healthy and pulmonary fibrosis for 10 days from Day 7 of the experiment. Mice were dissected on the 21st day. The levels of alpha-smooth muscle actin (α-SMA), pSMAD2/3, LOXL2, and p-Src were determined by Western blot analysis in the lungs. Furthermore, a group of MRC5 was differentiated into myofibroblasts by transforming growth factor-beta (TGF-β). Another group of MRC5s was treated with 10 µM ATR at 24 h after TGF-β stimulation. Cells were collected at 0, 24, 48, and 72 h. The effects of ATR on myofibroblast differentiation, apoptosis, and TGF-β and Wnt/β-catenin signaling activations were examined by Western blot analysis and flow cytometry in MRC5s. ATR attenuated pulmonary fibrosis by regulating myofibroblast differentiation and interstitial accumulation of collagen, by acting on LOXL2, p-Src, and pSMAD2/3 in mice lungs. Single cycle induction chemotherapy (IC) with platinum and 5-flurouracil (PF) and treatment based on clinical response predicts organ preservation in laryngeal cancer. Other agents offer intriguing alternatives with potentially increased ease of administration, reduced risk for severe toxicities, and increased platinum sensitivity.

We report the results of a phase II bioselection trial in advanced resectable laryngeal cancer utilizing an IC regimen of two cycles of platinum plus docetaxel (TP) with a Bcl-2 inhibitor. The primary endpoint was organ preservation rate at 12 weeks post chemoradiation.

Fifty-four patients were enrolled. Response to IC was 72%. The organ preservation rate was 59% with a laryngectomy free survival of 46%. Induction related grade ≥3 toxicities were observed in 56% of patients with two grade 5 events.
