Background Total knee arthroplasty is a successful elective orthopedic procedure with an increasing number being undertaken. Original knee arthroplasties used an all-polyethylene tibia; however, with concerns over tibial loosening, the trend moved toward the metal-backed variety. Modern designs providing more conformity and changes in manufacturing of the polyethylene make it an equivalent but cheaper option. Methods We analyzed the medium-term outcome in 1092 patients with an all-polyethylene tibial component in their total knee arthroplasty. Results Twenty-six patients had further surgery on their knee for a variety of reasons. Assessing reoperation for any reason, the 7-year survival rate was 96.96% with an overall infection rate of 0.37%. Seven-year survival analysis using aseptic loosening as a criteria was 99.28%. Both the Short Form 12 physical score and Western Ontario and McMaster Universities Osteoarthritis Index score significantly improved and was sustained at 7 years. Body mass did not correlate to either outcomes or complications. Conclusion An all-polyethylene tibia in a total knee arthroplasty has good outcome data and high patient satisfaction with good survivorship in the medium term, coupled with potential cost-saving benefits.Introduction Sexually transmitted infections (STIs) are common in our environment, and trends have been increasing in the last few years. Different methods for STIs diagnosis have been applied by microbiology laboratories over years, but real-time PCR has improved this process. Our objective was to evaluate VIASURE Sexually Transmitted Diseases Real-Time PCR Detection kit (CerTestBiotec, S.L.) comparing with the real-time PCR technique used in our laboratory (Allplex™ STI7 Essential Assay, Seegene) which was considered as reference assay. Methods A total of 948 samples from different sites (vaginal, endocervical, urethral, rectal, pharyngeal swabs and urine samples) were analyzed from July to September 2018. Results A discordant result was obtained in 4.5% (43 samples). These discrepancies were mainly observed in threshold cycle (Ct) value next to the limit of detection. The k coefficient obtained shows a very high agreement between both methods with k values from 0.92 to 0.99. Conclusions VIASURE Sexually Transmitted Diseases Real-Time PCR Detection kit provides a very good correlation with Allplex STI7 and therefore, it's a good tool for the diagnostic of STIs. Positive results with Ct value obtained from 35 and low amplification signal should be applied with caution and should be interpreted based on the patient's clinical data. Background The development of various targeted therapies against Epidermal Growth Factor Receptor (EGFR) has been a major step in therapeutic advancements in lung cancer. However, the response to tyrosine kinase inhibitors (TKI) therapy in a real-world setting has not been well elucidated. Methods As part of a retrospective analysis, patients with EGFR mutated non-small cell lung cancer at 4 tertiary care Institutions in North India between December 2007 and August 2018 were evaluated. The overall response rate, disease control rate, progression-free survival (PFS) and factors affecting PFS were analyzed. Results A total of 483 patients were included, including 52.4% males, with mean (±SD) age of 56.7 (±12.4) years. Majority (63.8%) had good performance status (Eastern Cooperative Oncology Group 0 or 1) and 77.4% were nonsmokers. Among the EGFR mutations, exon 19 deletion was the most common mutation detected (68.1%), followed by L858R mutation in exon 21 (26.9%). Extra-thoracic metastasis was present in 69.5% patients and majority of them had ≤ 2 metastatic sites (85.1%). TKIs were used as the first-line therapy in 64.8% patients, and gefitinib was the most frequently used TKI (67.3%), followed by erlotinib (26.7%). The overall response rate and disease control rate were 65.9% and 90.7% respectively. The median PFS was 9.3 months and brain was the exclusive site of progression in 18.0% patients. On univariate analysis, the factors that significantly affected PFS were, the number of metastatic sites and the type of EGFR mutation. On multivariate analysis, the number of metastatic sites was the only factor that affected the PFS [HR (95% CI) 2.5 (1.7-3.6); Pvalue 2) was the only significant factor associated with a worse PFS.Introduction Pseudochromhidrosis denotes the production of colourless sweat that acquires colour after coming into contact with exogenous factors such as dyes in clothing, chemicals or chromogenic microorganisms. Patients and methods A 9-year-old girl presented with progressive brown pigmentation predominantly on the nose, forehead, perioral region and cheeks. It was easily removable by rubbing with a moist compress, leaving normal-coloured skin as well as brown pigment on the compress. The same pigmentation recurred within 2h. The patient was not using cosmetics. Bacteriological culture of samples taken by rubbing the skin in affected areas on the face was positive for commensal bacteria of the skin (Actinomyces viscosus, Staphylococcus epidermidis, Cutibacterium acnes and Streptococcus sanguinis). Antibiotic therapy achieved total resolution of symptoms. Conclusion Bacteria constitute the most frequent aetiology of pseudochromhidrosis. Where such a cutaneous condition exists, even in the absence of positive bacteriological testing, antibiotic therapy would seem to be indicated as a therapeutic test. Biopsy does not appear to be essential as a first-line approach where a bacterial cause is suspected, but it may be proposed in the event of resistance to antibiotics. Background The ability to polymerize light-activated dental materials with dental light-curing units (DLCUs) has revolutionized dentistry. However, proper DLCU use is essential for ensuring the effectiveness and performance of these materials. Methods The authors developed an electronic cross-sectional survey in the American Dental Association Qualtrics Research Core platform. selleck chemical The survey included questions about DLCU use, unit type and selection, training, maintenance, technique, and safety measures. The authors deployed the survey to 809 American Dental Association Clinical Evaluators (ACE) panelists on October 9, 2019, and sent reminder links to nonrespondents 1 week later. They conducted exploratory and descriptive analyses using SAS software Version 9.4. Results Of the 353 ACE panelists who completed the survey, most used a DLCU in their practices (99%), and light-emitting diode multiwave units were the most common type of DLCU units (55%). Dentists use DLCUs for over one-half of their appointments each day (mean [standard deviation], 59% [22%]).
