Diagnostic uncertainty (DU) in primary care is ubiquitous, yet no review has specifically examined its communication, or the associated ethical issues.
To identify what is known about the communication of DU in primary care and the associated ethical issues.
Systematic review, critical interpretive synthesis and ethical analysis of primary research published worldwide. Medline, Embase, Web of Science and SCOPUS were searched for papers from 1988 to 2020 relating to primary care AND diagnostic uncertainty AND [ethics OR behaviours OR communication]. Critical interpretive synthesis and ethical analysis were applied to data extracted.
Sixteen papers met inclusion criteria. Although DU is inherent in primary care, its communication is often limited. Evidence on the effects of communicating DU to patients is mixed; research on patient perspectives of DU is lacking. The empirical literature is significantly limited by inconsistencies in how DU is defined and measured. No primary ethical analysis was identifould help to strengthen the empirical evidence base.Two stem-boring moths, the yellow stemborer (YSB) Scirpophaga incertulas (Walker), and the striped stemborer (SSB), Chilo suppressalis (Walker), damage rice in Asia. YSB is the dominant species in much of tropical Asia. Both species are oligophagous on domesticated and wild rice. We investigated the roles of host plant preferences and larval performance in determining the larval densities of both species in rice plots. In screenhouse experiments, YSB showed significant preference-performance coupling. Adults preferred high-tillering rice varieties during early vegetative growth. In contrast, SSB did not demonstrate oviposition preferences under the same screenhouse conditions, but did oviposit less on the wild rice Oryza rufipogon Griff. than on domesticated rice varieties during a choice experiment. Despite differences in preference-performance coupling, larval survival and biomass across 10 varieties were correlated between the two species. YSB and SSB larvae occurred in relatively high numbers on rice varieties with large tillers (IR70, IR68, and T16) in wet and dry season field experiments. However, whereas YSB was the dominant species on IR68 and IR70, it was relatively less abundant on T16, where SSB dominated. Results suggest that YSB preferentially attacked fast-growing rice varieties with high tiller numbers early in the crop cycle. Although ASSR reduction is known to vary in terms of frequency and time, the neural mechanisms are poorly understood. We obtained high-density electrocorticography recordings from a wide area of the cortex in 8 patients with refractory epilepsy. In an ASSR paradigm, click sounds were presented at frequencies of 20, 30, 40, 60, 80, 120, and 160 Hz. We performed time-frequency analyses and analyzed intertrial coherence, event-related spectral perturbation, and high-gamma oscillations. We demonstrate that the ASSR is globally distributed among the temporal, parietal, and frontal cortices. The ASSR was composed of time-dependent neural subcircuits differing in frequency tuning. Importantly, the frequency tuning characteristics of the late-latency ASSR varied between the temporal/frontal and parietal cortex, suggestive of differentiation along parallel auditory pathways. This large-scale survey of the cortical ASSR could serve as a foundation for future studies of the ASSR in patients with neuropsychiatric disorders.
As long-term use of benzodiazepines increases, adverse effects also become more frequent, especially in elderly adults. Due the potential of causing dependence, poor patient adherence and a lack of awareness of side effects, deprescription is challenging.
This study aimed to identify what are the effective approaches to motivate and promote deprescription of benzodiazepines.
We used MeSH terms to search in five databases that were MEDLINE, Cochrane CENTRAL, LILACS, SCIELO and Science Direct. Then, we selected articles accordingly to inclusion and exclusion criteria. Risk of bias assessment for randomized controlled trials and prospective interventional studies was made using RoB 2.0 and ROBINS-I tools, respectively. For cohort studies, we used the clarity group by McMaster University tool.
Database search retrieved 412 results, and 11 studies were selected for analysis. Interventions focusing on patient education to improve community awareness about deprescription presented better discontinuation ratee and some form of cognitive evaluation that might influence the results of the intervention.Links among agriculture, nutrition, and health (ANH) are established, but the role of inequity is less understood. In this scoping review, we aimed to understand the range of ways that ANH research addresses inequity issues in low- and middle-income countries. We used PRISMA guidelines to structure our study. From an initial >26 000 studies, 243 published reports met inclusion criteria and were mapped. The number of reports addressing inequity in ANH research has increased over time from 40 in 2018. Within equity, a majority of articles (n = 327) focused on describing how nutrition and health outcomes differ for different groups. Many (n = 134) looked at the material circumstances that shape people's life chances. Fewer (n = 51) looked at the most basic structural determinants of (in)equity. The same aspects of equity remain the least studied in ANH research, including the intersections of equity issues shaping life chances, and inequity's structural determinants. We suggest ways forward for this community, drawing on conceptual frameworks and theory of inequity from different disciplines.
