Subspecialty training is a common part of orthopedic surgical training. The factors which influence resident subspecialty choice have important residency design and workforce implications. Our objective was to present survey data gathered from orthopedic residents regarding their fellowship plans and relative importance of factors which influence those plans.
An anonymous online survey tool was developed and distributed to orthopedic residents through their program directors at academic institutions across the country with orthopedic surgery residency programs.
227 residents completed the survey. 97% planned to pursue fellowship training after residency. The most common presumptive subspecialties were sports (29.7%), joints (17.3%) and shoulder/ elbow (12.8%). The majority of senior residents (57%) reported that their subspecialty choice had changed during residency. When making their choice of subspecialty, residents were most influenced by their experiences working on the subspecialty service in questor residency design, mentorship structuring, career counseling and for addressing subspecialty surpluses or shortages which arise in the future.Level of Evidence IV.
Female orthopedic surgeons have different life choices, experiences, and influences that may alter their decisions when choosing a fellowship. There is no data that describes why women choose their specialty and if it is related to their mentorship, athletic background, family, family planning, physical attributes needed, location, practice setting, or discrimination.After a five year orthopedic residency, what influences their specialty choice to work in a specific field?Questions/ purposes To understand why women in orthopedics chose a certain fellowship and if they are influenced by any specific factor.
A 28 question survey created through SurveyMonkey was emailed to all members of the Ruth Jackson Orthopedic Society (RJOS) in July 2019. After six weeks the survey was closed and data was analyzed through SurveyMonkey.
252 members of RJOS completed the 28 question survey. https://www.selleckchem.com/products/lazertinib-yh25448-gns-1480.html 94% of the women who responded did a fellowship after their orthopedic residency. 62% chose their fellowship specialty based on pure enjoyment. 79% were not influenced by a female role model, 92% were not influenced by a significant other, 85% were not influenced by wanting children or a family, and 96% were not influenced by being pregnant or planning on being pregnant. 64% were not influenced by physical attributes or perception of strength needed for the specialty.

The risks of radiation exposure in orthopaedic surgery have become a topic of increasing interest in the setting of widespread fluoroscopy use and concern for an increased prevalence of breast cancer among female orthopaedic surgeons. The aim of this national study of 31 female orthopaedic surgeons was to achieve a deeper understanding of fluoroscopic use in the OR and its associated exposure to radiation, by comparing female orthopaedic trauma and arthroplasty surgeons.
A total of 31 surgeons wore dosimeters for 10 operating days each to track cumulative radiation exposure. Surgeons were not asked to modify their practice in any way, with no requirement that the operating days had to be chosen with the knowledge that fluoroscopy would be used. Participants were also asked to fill out a form at the end of each day, detailing the number of cases that day, the number of hours spent in the OR, and the total amount of time using fluoroscopy.
Trauma surgeons received significantly higher radiation doses in the OR. The fluoroscopic times in this study therefore may be a useful self-assessment tool for attending trauma and arthroplasty surgeons. Awareness of these differences will hopefully increase an individual surgeon's mindfulness toward the length of fluoroscopy use in each case, regardless of orthopaedic subspecialty.Level of Evidence IV.
15% of orthopedic surgery trainees in 2018-2019 in ACGME accredited programs are female, which lags behind all other specialties.
The bottleneck for achieving gender diversity in orthopedic surgery is that female medical students do not choose orthopedic surgery as a career. In 2018-2019, twelve ACGME accredited programs had no women trainees, highlighting the uneven distribution of female trainees across residency programs. Social science has outlined that 30% representation within a population is the diversity goal.

A 57-year-old female with a history of decompensated cirrhosis with early stage breast cancer was treated with 25 Gy in one fraction. At her 7-month follow up visit, there was a complete resolution of disease on imaging. This case represents a novel approach for the treatment of breast cancer with SABR when surgery is contraindicated.
A cluster randomised controlled trial (cRCT) performed from July 2018 to March 2019 demonstrated the clinical impact of a community pharmacist delivered minor ailment service (MAS) compared with usual pharmacist care (UC).MAS consisted of a technology-based face-to-face consultation delivered by trained community pharmacists. The consultation was guided by clinical pathways for assessment and management, and communication systems, collaboratively agreed with general practitioners. link2 MAS pharmacists were trained and provided monthly practice support by a practice change facilitator. The objective of this study was to assess the cost utility of MAS, compared to UC.
Participants recruited were adult patients with symptoms suggestive of a minor ailment condition, from community pharmacies located in Western Sydney. Patients received MAS (intervention) or UC (control) and were followed-up by telephone 14-days following consultation with the pharmacist. A cost utility analysis was conducted alongside the cRCT. Trin the Australian context is cost effective.

Gastric cancer is a serious malignant tumor associated with aberrant circular RNAs (circRNAs) expression. In this study, we aim to investigate the role and the underlying mechanism of circ_0000190, a circRNA in gastric cancer.
Circ_0000190 expression in vivo was examined in gastric cancer and adjacent normal tissues by RT-PCR. Circ_0000190 expression in gastric cancer cell lines was detected by FISH and RT-PCR. The role of the circRNA in gastric cancer cells was assessed by the analysis of cell viability, apoptosis, proliferation, cell cycle and migration. The potential effector of circ_0000190 was predicted by computational screen and validated by luciferase reporter assay. Furthermore, Mice model of human gastric cancer was established to observe the underlying mechanisms of circ_0000190.
Circ_0000190 was down-regulated in gastric cancer tissues and cells, with a major location in cytoplasm. Circ_0000190 inhibited gastric cancer cell viability, proliferation and migration, and induced apoptosis and cell cycle arrest by regulating the expression of capase-3, p27 and cyclin D. In addition, the circRNA was validated as a sponge of miR-1252, which directly targeted PAK3. The effects of circ_0000190 on the cellular processes were blocked by miR-1252 mimics, which could be rescued after further overexpression of PAK3.

This study aims to investigate the mechanism underlying the high level of long non-coding RNA FOXD3-AS1 in cisplatin-resistant NSCLC cells.
Cisplatin-resistant cells were generated from A549 cells. CCK-8 were used to evaluate cell proliferation. The FOXD3-AS1, miR-127-3p, MDM2 and MRP1 mRNA expression levels were confirmed by qRT-PCR. Protein levels of MDM2 and MRP1 were determined by western blot assay. link3 Luciferase reporter and RNA pull-down assays were evaluated the relationship between miR-127-3p and FOXD3-AS1/MDM2. In vivo tumor growth was evaluated in a xenograft nude mice model.
FOXD3-AS1 was up-regulated in cisplatin-resistant NSCLC cells (A549/DDP and H1299/DDP cells) in comparison with their parental cell lines. Overexpression of FOXD3-AS1 promoted cisplatin-resistance in A549 and H1299 cells; while FOXD3-AS1 knockdown sensitized A549/DDP and H1299/DDP cells to cisplatin treatment. FOXD3-AS1 regulated miR-127-3p expression by acting as a competing endogenous RNA, and miR-127-3p repressed MDM2 expression via targeting the 3'UTR.
