if therapy is paused in the third trimester.

Infliximab clearance decreases significantly in the second and third trimesters, leading to increasing maternal infliximab concentrations in any given regimen. Maternal infliximab levels may thus be maintained as constant in a de-intensified regimen by therapeutic drug monitoring guidance in inflammatory bowel disease.

JM 3100 We aimed to evaluate healthcare workers' perceptions of the influenza vaccination in the Qassim region in Saudi Arabia. A cross-sectional study was conducted at selected hospitals from November to March 2020, in which healthcare workers completed a self-administered questionnaire. Of 327 participants, most were equally distributed between the ages 18-30 and 31-45 years (42.8% each), with 57.5% female and 42.5% male. Both Saudi (47.7%) and non-Saudi participants (52.3%) were included. The majority were physicians (29.7%), pharmacists (28.1%), and nurses (27.5%). Overall, 60.9% had good knowledge, 89% had positive perceptions, and 10.7% had negative perceptions. The primary reason for not getting vaccinated was a concern for complications. Moreover, 20.8% had never previously been vaccinated. Knowledge was positively correlated with nationality, educational level, and perception (p = .002, p = .047, and p = .021, respectively). Perceptions were significantly correlated with nationality (p =.009). Furthermore, 24.5% completely disagreed with compulsory vaccination and believe it would not improve coverage. Once fitted using a multinomial regression model, an r-square value of 0.026 indicated that nationality and history of previous vaccination significantly contributed to negative perceptions. We concluded that most healthcare workers had good knowledge and positive perceptions, and more than a third of the participants adhered to seasonal vaccination. Saudi patients and those who had never been vaccinated were more likely to have negative perceptions. The introduction and rapid transmission of SARS-CoV-2 in the United States resulted in methods to assess, mitigate, and contain the resulting COVID-19 disease derived from limited knowledge. Screening for testing has been based on symptoms typically observed in inpatients, yet outpatient symptoms may differ. Classification and regression trees recursive partitioning created a decision tree classifying participants into laboratory-confirmed cases and non-cases. Demographic and symptom data from patients ages 18-87 years enrolled from March 29-June 8, 2020 were included. Presence or absence of SARS-CoV-2 was the target variable. Of 832 tested, 77 (9.3%) tested positive. Cases significantly more often reported diarrhea (12 percentage points (PP)), fever (15 PP), nausea/vomiting (9 PP), loss of taste/smell (52 PP), and contact with a COVID-19 case (54 PP), but less frequently reported sore throat (-27 PP). The 4-terminal node optimal tree had sensitivity of 69%, specificity of 78%, positive predictive value of 20%, negative predictive value of 97%, and AUC of 76%. Among those referred for testing, negative responses to two questions could classify about half (49%) of tested persons with low risk for SARS-CoV-2 and would save limited testing resources. Outpatient symptoms of COVID-19 appear to be broader than the inpatient syndrome.Initial supplies of anticipated COVID-19 vaccines may be limited and administration of first such available vaccines may need to be prioritized for essential workers, the most vulnerable, or those likely to have a robust response to vaccine. Another priority group could be those not previously infected. Those who screen out of testing may be less likely to have been infected by SARS-CoV-2 virus thus may be prioritized for vaccination when supplies are limited.Agricultural biotechnology is enhancing agricultural productivity, food security, and livelihoods globally. Some developing countries have established functional biosafety regulatory systems and have commercialized genetically modified (GM) crops. Release of GM crops requires enhanced capacity for regulatory compliance and product stewardship to help ensure sustainable use of biotechnology products. We conducted a survey of 66 stakeholders, mostly from Africa and Asia, in two-week international agricultural biotechnology short courses. Respondents showed knowledge of biotechnology benefits and expressed potential barriers to commercialization. They identified 16 crops in the "pipeline for commercialization." Stakeholders also shared ideas about how to build capacity for product stewardship. Product stewardship is a concept which requires each person in the product life cycle - innovators, scientists, and technology users, to share responsibility. This paper focuses on adoption of product stewardship for post-release management of GM crops which encompasses trait performance, resistance management, integrated pest management (IPM), good agricultural practices, high-quality seeds and planting material, intellectual property management, labeling, identity preservation, consumer acceptance, and effective marketing.Although it is generally accepted that dietary fiber is health promoting, the underlying immunological and molecular mechanisms are not well defined, especially with respect to cellulose, the most ubiquitous dietary fiber. Here, the impact of dietary cellulose on intestinal microbiota, immune responses and gene expression in health and disease was examined. Lack of dietary cellulose disrupted the age-related diversification of the intestinal microbiota, which subsequently remained in an immature state. Interestingly, one of the most affected microbial genera was Alistipes which is equipped with enzymes to degrade cellulose. Absence of cellulose changed the microbial metabolome, skewed intestinal immune responses toward inflammation, altered the gene expression of intestinal epithelial cells and mice showed increased sensitivity to colitis induction. In contrast, mice with a defined microbiota including A. finegoldii showed enhanced colonic expression of intestinal IL-22 and Reg3γ restoring intestinal barrier function.
