Rheumatoid arthritis (RA), if left untreated, can lead to joint damage and deformity, disability, and even death. Hence, early diagnosis and management are essential to improve clinical and functional outcomes. This study aimed to identify the most common variables and risk factors related to RA activity among patients living in the Kingdom of Saudi Arabia (KSA).
This study was conducted between January 2018 and March 2019 on consecutive patients diagnosed with RA at a tertiary care hospital in KSA. Adult patients (≥18 years old) diagnosed with RA based on the American College of Rheumatology 2010 criteria were recruited. The Disease Activity Score-28 for Rheumatoid Arthritis with CRP (DAS28-CRP) and health assessment questionnaire disability index (HAQ-DI) were calculated for 75 patients attending the rheumatology clinic during the study period to evaluate the rate of remission and functional capacity, and to compare findings with other local studies after assessing the relationship of these factors with medication use and existing comorbidities.
The majority of the 75 patients were female (n=64), with a mean age of 49.7 years and average disease duration of 130 days. TPEN nmr The median HAQ-DI was less than 0.5 (range 0-1.95). The DAS28-CRP scores revealed moderate disease activity in 45.3% and low disease or remission in 38.6% of the patients. Many patients (45.3%) were treated with methotrexate, and the most commonly used biological treatment was adalimumab in 14.6%. Comorbidities included hypertension (26.7%) and diabetes mellitus (18.7%). There was a strong association between cardiovascular diseases and a high DAS28-CRP score (p < 0.001).
A higher RA activity rate was observed. This may be related to difficultly accessing rheumatology clinics in our facility and financial difficulties accessing biological treatments.

The ability of individuals and couples to anticipate and achieve their desired number of children as well as the spacing and timing of their birth is family planning. Access to family planning and ensuring that needs are met is based on human rights. Nonetheless, for most women, this need has not been met. In addition, in this study field, there was minimal evidence regarding this. Thus, the aim of this study was to assess the prevalence and associated factors of unmet need of modern contraceptive methods among currently married women in Damot Woyde district, southern Ethiopia.
This community based cross-sectional study was conducted from February 20 to 28, 2019. A multi-stage sampling procedure was employed to select 658 currently married women of child-bearing age in the study areas. A pretested interviewer administered questionnaire was used for data collection. Bivariate and multivariate logistic regression analyses were used to identify the predictors of dependent variables and the adjusted odds ratind discussion at grass root level in order to promote the right time for using family planning. District health office and other concerned bodies should collaborate with the education office to expand formal and non-formal education for women in order to decrease the unmet need for modern contraceptive methods.
This study revealed that unmet need for modern contraceptive methods was found to be high compared to the national and regional figures. Therefore, community health workers and health professionals should be strengthening communication and discussion at grass root level in order to promote the right time for using family planning. District health office and other concerned bodies should collaborate with the education office to expand formal and non-formal education for women in order to decrease the unmet need for modern contraceptive methods.Over the past 15 years, investigators have reported on the utility and safety of cardiac glycosides for numerous health benefits including those as treatments for malignant disease, stroke-mediated ischemic injury and certain neurodegenerative diseases. In addition to those, there is a growing body of evidence for novel antiviral effects of selected cardiac glycoside molecules. One unique cardiac glycoside, oleandrin derived from Nerium oleander, has been reported to have antiviral activity specifically against 'enveloped' viruses including HIV and HTLV-1. Importantly, a recent publication has presented in vitro evidence for oleandrin's ability to inhibit production of infectious virus particles when used for treatment prior to, as well as after infection by SARS-CoV-2/COVID-19. This review will highlight the known in vitro antiviral effects of oleandrin as well as present previously unpublished effects of this novel cardiac glycoside against Ebola virus, Cytomegalovirus, and Herpes simplex viruses.
Cholangiocarcinoma is a locally invasive, poorly treatable malignancy of the biliary tract that uncommonly metastasizes to the brain and rarely causes neuro-ophthalmologic complications.
A 34-year-old woman with an isolated sixth cranial nerve palsy underwent brain neuroimaging and was found to have a large sellar/suprasellar mass invading the cavernous sinus. Gross total resection was performed with improvement in the sixth cranial nerve nerve palsy. Next-generation sequencing and histology studies revealed an adenocarcinoma with a fibroblast growth factor receptor (

gene mutation. Positron emission tomography/computed tomography scan demonstrated a large hypermetabolic partially necrotic hepatic mass with local invasion, and liver biopsy confirmed a diagnosis of cholangiocarcinoma. At three weeks after resection, the brain lesion recurred and the patient developed worsening diplopia. The patient then received stereotactic radiotherapy applied to the brain lesion and began treatment with gemcitabine and cisplatin.
