The instantaneous helical axis (HA), is a mathematical model used to visualize globally rigid body kinematics. It can be applied to mandibular motion for quantification of movement patterns and irregularities. Aim of this study was to analyze HA pathways in subjects with unilateral and bilateral TJR and compare them to a control group. An optoelectronic system was employed to track mouth opening/closing cycles (n = 3) of 15 patients (7 operated unilaterally, 8 bilaterally, 11 F, aged 24-72) and 12 controls (6 F, aged 23-40). HA position in space was determined for 30 equally-distributed steps of the observed movement. Total mandibular rotation around HA (Φtot) and total translation along HA (Ttot) were determined. Angles between HA and the anatomical coordinate system of the head (θx, θy, θz); global fluctuation of HA spatial orientation (θe), distance between condylar center (CP) and HA (dCP) and its projections on the axes (xdCP, ydCP, zdCP) were calculated. Overall, Φtot was larger in controls than in bilaterally operated subjects (p = 0.002, p = 0.003) and θz was larger in unilaterally operated subjects than in controls (p = 0.004) and bilaterally operated subjects (p = 0.002, p = 0.024). During opening, θe¯ was smaller in controls than in unilaterally operated subjects (p = 0.01). The distance dCP was smaller for alloplastic joints than for controls (p less then 0.01 overall). Collectively, our data show that circAMD1 functions as a miR-27a-3p sponge that inhibits the functional and phenotypical alteration of P63-mutant HDFs and may be a critical marker in pathogenesis regarding P63-associated traits.

Sleep disordered breathing (SDB) is common in adults with chronic heart failure (CHF), but its prevalence in children remains unclear. Continuous positive airway pressure (CPAP) is the treatment of SDB but deleterious hemodynamic effects have been reported.

We prospectively analyzed SDB in children with CHF and the effect of CPAP on work of breathing (WOB) and cardiac index (CI). Children aged 6 months to 18 years old with CHF due to 1) dilated cardiomyopathy (DM) with an ejection fraction<45%, 2) functional single ventricle (SV) or 3) aortic or mitral valve disease awaiting surgery (VD) were eligible for the study. A polysomnography (PSG), measurement of WOB and CI during spontaneous breathing (SB) and CPAP (6, 8 and 10 cmH

O) were performed.

Thirty patients with mean age of 6.4±5 years were included (16 DM 16, 10 SV, 4 LV). Twenty (73%) patients had a normal sleep efficiency. Median apnoeas hypopnea index (IAH) was within normal range at 1.6 events/h (0, 14) events/hour. Only one patient had central sleep apnoeas, none had Cheyne-Stokes respiration, and 3 patients had an obstructive AHI between 5 and 10 events/hour. Optimal CPAP level decreased WOB (p=0.05) and respiratory rate (p=0.01).

Severe SDB was uncommon in children with CHF. However, CPAP may be beneficial by decreasing WOB and respiratory rate without deleterious effects on CI.

Sleep and physical activity changes are common in pregnancy, but longitudinal data starting before conception are scarce. Our aim was to determine the changes of the daily total sleep time (TST) and physical activity duration (PAD) from before conception to end of pregnancies in respect of pregestational maternal factors.

This longitudinal observational study formed part of the CONIMPREG research project and recruited healthy women planning to become pregnant. Sleep and physical activity were recorded around-the-clock for ≥4 days via actigraphy before conception and during each trimester of pregnancy. Data were adjusted according to pregestational maternal body composition, parity and age.

Among 123 women with eligible data, the unadjusted mean (95% confidence interval) TST increased from 415.3min (405.5-425.2min) before conception to 458.0min (445.4-470.6min) in the 1

trimester, remaining high through the 2

and 3

trimesters. Variation was substantial before conception (±2SD range 307-523min). The unadjusted mean PAD before conception was 363.
