The emergence and spread of carbapenem-resistant Enterobacteriaceae (CRE) represent a major clinical problem and raise serious health concerns. The present study aimed to investigate and ascertain the occurrence of CRE among hospitalized patients of Mohamed VI University Hospital, Marrakech, Morocco. Biological samples were collected over a one-year period (2018). The bacterial isolates were identified by MALDI-TOF-MS. Antibiotic susceptibility testing was performed using disc diffusion and Etest. The modified Hodge test and combined disc diffusion test were used for phenotypic detection. CRE hydrolyzing enzyme encoding genes blaOXA-48, blaKPC, blaIMP, blaVIM, and blaNDM were characterized by PCR and DNA sequencing. In total, 131 non-duplicate CRE clinical strains resistant to Ertapenem were isolated out of 1603 initial Enterobacteriaceae. Klebsiella pneumoniae was the most common species (59%), followed by Enterobacter cloacae (24%), E. coli (10%), Citrobacter freundii (3%), Klebsiellaoxycota (2%), Serratia marcescens (1%), and Citrobacter braakii (1%). Of these, 56.49%, 21.37%, 15.27%, 3.38%, and 3.05% were collected from blood, urine, pus, catheters and respiratory samples, respectively. Approximately 85.5% (112/131) of the isolates were carbapenemase producers (40 blaOXA-48, 27 blaNDM, 38 blaOXA-48 + blaNDM and 7 blaVIM). All metallo-β-lactamases isolates were NDM-1 and VIM-1 producers. This is the first documentation of blaOXA-48 genes from C. freundii and C. braakii in Morocco.In order to improve the user experience of public fitness equipment, based on the principles of experience design principle, the Norman 3-level theory is used to obtain product materials, using safety, equipment practicality, product interest, and humanistic construction experience. SPME conditions, such as fiber coating, pre-incubation temperature, extraction temperature, and time were studied. The feasibility was compared between SPME-arrow and the traditional fiber by loading different sample amounts. It was found that the SPME-arrow was more suitable for the MHS-SPME. The limit of detection (LODs) and limit of quantitation (LOQs) of pyrazines were in the range of 2-60 ng and 6-180 ng/g oil, respectively. The relative standard deviation (RSD) of both intra- and inter-day were lower than 16%. The mean recoveries for spiked pyrazines in rapeseed oil were in the range of 91.6-109.2%. Furthermore, this newly established method of MHS-SPME-arrow was compared with stable isotopes dilution analysis (SIDA) by using [2H6]-2-methyl-pyrazine. Moreover, by arranging the meta-grating to form a quasi-continuous metasurface, subsequent simulations indicated that the designed device exhibited an ultralow specular reflectivity below 10% and a tunable thermal emissivity from 14.5% to 91%. Furthermore, we explain the advantages that come from using AI over any existing traditional methods.For patients with hypertension, serious complications, such as myocardial infarction, a common cause of heart failure, occurs in the late stage of hypertension. link2 Hypertension outcomes can lead to complications, including death. Hypertension outcomes threaten patients' lives and need to be predicted. In our research, we reviewed the hypertension medical data from a tertiary-grade A class hospital in Beijing, and established a hypertension outcome prediction model with the machine learning theory. We first proposed a gain sequence forward tabu search feature selection (GSFTS-FS) method, which can search the optimal combination of medical variables that affect hypertension outcomes. Based on this, the XGBoost algorithm established a prediction model because of its good stability. We verified the proposed method by comparing other commonly used models in similar works. The proposed GSFTS-FS improved the performance by about 10%. The proposed prediction method has the best performance and its AUC value, accuracy, F1 value, and recall of 10-fold cross-validation were 0.96. 0.95, 0.88, and 0.82, respectively. link3 It also performed well on test datasets with 0.92, 0.94, 0.87, and 0.80 for AUC, accuracy, F1, and recall, respectively. Citation databases were searched for English-language economic evaluations reporting on cascade genetic testing. Nineteen publications were included. In four, genetic testing was used to identify new index patients-cascade effects were also considered; thirteen assessed cascade genetic testing strategies for the identification of at-risk relatives; and two calculated the costs of cascade genetic testing as a secondary objective. Methodological challenges associated with incorporating cascade effects in economic evaluation are related to study design, costing, measurement and valuation of health outcomes, and modeling. As health economic studies may currently be underestimating both the cost and health benefits attributable to genetic technologies through omission of cascade effects, development of methods to address these difficulties is required.Transdermal drug delivery of lidocaine is a good choice for local anesthetic delivery. Microemulsions have shown great effectiveness for the transdermal transport of lidocaine. Oil-in-water nanoemulsions are particularly suitable for encapsulation of lipophilic molecules because of their ability to form stable and transparent delivery systems with good skin permeation. However, fabrication of nanoemulsions containing lidocaine to provide an extended local anesthetic effect is challenging. Hence, the aim of this study was to address this issue by employing alginate-based o/w nanocarriers using nanoemulsion template that is prepared by combined approaches of ultrasound and phase inversion temperature (PIT). In this study, the influence of system composition such as oil type, oil and surfactant concentration on the particle size, in vitro release and skin permeation of lidocaine nanoemulsions was investigated. Structural characterization of lidocaine nanoemulsions as a function of water dilution was done using DSC. Nanoemulsions with small droplet diameters (d less then 150 nm) were obtained as demonstrated by dynamic light scattering (DLS) and cryo-TEM. These nanoemulsions were also able to release 90% of their content within 24-h through PDMS and pig skin and able to the drug release over a 48-h. This extended-release profile is highly favorable in transdermal drug delivery and shows the great potential of this nanoemulsion as delivery system.Rigid polyurethane (RPU) foams have been successfully glycolyzed by using diethylene glycol (DEG) and crude glycerol (CG) as transesterification agents. However, DEG did not allow to achieve a split-phase process, obtaining a product with low polyol purity (61.7 wt %). On contrary, CG allowed to achieve a split-phase glycolysis improving the recovered polyol purity (76.5%). This is an important novelty since, up to now, RPUs were glycolyzed in single-phase processes giving products of low polyol concentration, which reduced the further applications. Moreover, the nanosilica used as filler of the glycolyzed foams was recovered completely pure. The recovered polyol successfully replaced up to 60% of the raw polyol in the synthesis of RPU foams and including the recovered nanosilica in the same concentration than in glycolyzed foam. Thus, the feasibility of the chemical recycling of this type of polyurethane composites has been demonstrated. Additionally, PU foams were synthesized employing fresh nanosilica to evaluate whether the recovered nanosilica has any influence on the RPU foam properties.
