Breast ptosis is characterized by severe volumetric deficiency of the upper pole, excess of skin in the lower pole and descent of the nipple-areola complex (NAC). The aim of this study was to measure the recurrence of breast ptosis after mastopexy in aprospective study.

Female patients affected by unilateral or bilateral moderate or severe breast ptosis were enrolled in the study. All the patients underwent mastopexy performed only with the removal of skin excess following aWise pattern. The jugular notch-nipple distance was measured before surgery, immediately after surgery and after 1, 3, 6 and 12 months.

The aim of this study was to evaluate the frequency of depressive and anxiety disorders in patients with carpal tunnel syndrome (CTS) after surgery in comparison with normal individuals.

This cross-sectional analytical case-control study conducted on patients who underwent surgery with adiagnosis of carpal tunnel syndrome. In the first group, 35 patients with CTS who underwent surgery were randomly selected and evaluated for anxiety and depression with two standard questionnaires - of CES-D (Center for Epidemiologic StudiesDepression) and S-TAI (Spielberger State-Trait Anxiety Inventory). The second group of 35 normal people were randomly selected and examined after matching the age and sex. The data were compared and analyzed using SPSS V.22 software.

The mean score of Spielberger state and trait anxiety and depression in the case group was higher than the control group (P < 0.001 and P = 0.003 respectively). In both age groups (< 40 and >40years) the level of state and trait anxiety and depression was significantly higher in case than the control group. In women, the level of state and trait anxiety and depression was significantly higher in the case group than in the control group, but in men, only trait anxiety was significant.

The anxiety and depression disorders were more common in patients undergoing CTS than in normal individuals. Therefore, it is necessary to study the psychological status of these patients in the pre-discharge period and pay attention to its predictors in order to plan appropriate interventions.

The anterolateral thigh (ALT) flap was described as the fasciocutaneous flap. It can be harvested as apedicled and/orfree flap. Majority of the free flaps are harvested as afasciocutaneous flap. Their use in head and neck reconstruction and limb trauma is well established. Apart from these advantages, this flap has various applications which are less utilized. ALT flap can be used as amyocutaneous flap along with vastus lateralis muscle. When muscle and fasciocutaneous flaps are required, both can be harvested as achimeric flap which can cover two different regions of the wound. Moreover, harvest of the pedicled flap procedure is less time-consuming than that of afree flap. Here, we use a spatial prioritisation analysis to evaluate the consequences of considering only relatively well-mapped threats to biodiversity and compare this with planning scenarios that also account for more uncertain threats (in this case mining and armed conflict) under different management strategies. We evaluate three management strategies to address these more uncertain threats 1. to ignore them; 2. avoid them; or 3. specifically target actions towards them, first individually and then simultaneously to assess the impact of their inclusion in spatial prioritisations. We apply our approach to the eastern Democratic Republic of the Congo (DRC) and identify priority areas for conserving biodiversity and carbon sequestration services. We found that a strategy that avoids addressing threats of mining and armed conflict more often misses important opportunities for biodiversity conservation, compared to a strategy that targets action towards areas under threat (assuming a biodiversity benefit is possible). We found that considering mining and armed conflict threats to biodiversity independently rather than simultaneously results in 13 800-14 800 km2 and 15 700-25 100 km2 of potential missed conservation opportunities when undertaking threat-avoiding and threat-targeting management strategies, respectively. Our analysis emphasises the importance of considering all threats that can be mapped in spatial conservation prioritisation.

gov NCT02294461.It is clinically possible for patients with Alport syndrome (AS) to suffer from poststreptococcal acute glomerulonephritis (PSAGN). However, there is only one report of such a patient, and he had end-stage kidney disease. Here, we describe an 8-year-old male with X-linked AS and chronic kidney disease (CKD) stage G2. He presented with diffuse edema, gross hematuria, proteinuria, and body weight gain after streptococcal pharyngitis. Blood examination showed kidney dysfunction, hypocomplementemia, and increased anti-streptolysin-O levels. His kidney function did not improve with symptomatic treatment. Therefore, we started steroid administration on the 12th day of hospitalization. Then, his kidney function improved before he was discharged. We confirmed that his complement function had recovered at a later date. Pathological evaluation showed findings of AS and PSAGN, including cellular crescents in 3/30 glomeruli on light microscopy. In addition, electron dense deposits (EDDs) were seen in not only the visceral subepithelium but also the glomerular basement intramembrane and subendothelium, some of which were hump-like. Although AS and CKD are indicated to have a poor prognosis in PSAGN, our patient recovered after administration of steroids. Our case suggests that we can consider the administration of steroids, including pulse therapy for PSAGN, when patients have, for example, crescents on pathology, severe renal dysfunction, nephrotic proteinuria, or AS with CKD, as in our case.
