Cases were sorted into six categories of death (i) sudden traumatic/non-natural (ST), (ii) sudden natural (SN), (iii) prolonged traumatic/non-natural (PT), (iv) prolonged natural (PN), (v) diabetic ketoacidosis (DKA), and (vi) alcoholic ketoacidosis (AKA). The mean BHB for all cases was 1.67 mmol/L (17.4 mg/dL; range 0.11-18.02 mmol/L). The numbers of DKA, AKA, PN, PT, SN, and ST deaths were 21, 5, 155, 258, 275, and 253, respectively. Their mean vitreous BHBs were as follows 11.04 mmol/L (DKA), 8.88 mmol/L (AKA), 1.56 mmol/L (PN), 1.55 mmol/L (PT), 1.26 mmol/L (SN), and 1.38 mmol/L (ST). There was a statistically significant difference between the mean BHBs of the PN and SN death groups (p 96%. A receiver operator characteristic curve found 3.43 mmol/L to be the optimal cutoff value, demonstrating a specificity of 98.3% and a sensitivity of 96.2%. © 2020 American Academy of Forensic Sciences.Skin homeostasis is constantly challenged by environmental factors, affecting its delicate redox balance. The skin is also home to a wide variety of bacterial species, including Staphylococci. The cutaneous redox state is governed by the Nrf2-keap1 pathway, which is responsible for the induction of phase II cytoprotective enzymes, thus sustaining a healthy oxidative state. As part of normal metabolism, both bacteria and cutaneous tissue emit copious amounts of volatile organic compounds (VOCs), one subgroup of which are aldehydes. α,β-unsaturated aldehydes are known activators of Nrf2-keap1 pathway by direct oxidation of the keap1 protein. However, we did not encounter reports of Nrf2 activation by saturated or aromatic aldehydes, neither bacteria nor skin derived. We hypothesized, that non α,β-unsaturated aldehydes derived from skin or cutaneous bacteria may act as Nrf2-keap1 pathway activators and therefore afford protection against environmental insults. The saturated aldehydes Nonanal and Decanal (known skin metabolites) and the aromatic aldehyde Benzaldehyde (known skin and Staphylococcus epidermidis metabolite) were shown to induce the Nrf2-keap1 pathway in human keratinocytes. We also identified a newly described aromatic aldehyde, 3-furaldehyde (3-FA), emitted from S. aureus and S. epidermidis cultures, which also activated the pathway. Moreover, Nrf2-keap1 induction led to a significant protection against UVB-induced apoptosis. The mechanism involved in this activation has been partially elucidated. This work emphasizes the importance of cutaneous bacteria, as well as healthy skin lipid peroxidation processes in the maintenance and regulation of the cellular antioxidant response, namely with regard to coping with environmental stressors. However, the duration of RNA shedding in Wuhan is rarely known and the meaning of the prolonged shedding is still under investigation. Almost 10% patients diagnosed of COVID-19 had a RNA shedding longer than 30 days even if the symptom elimination. The IgM was in a high level in the 9th week after symptom onset in these prolonged-RNA-shedding patients. Further study should be conducted to know the infectivity of the virus and the relationship between RNA shedding and antibody expression. All rights reserved.Determining the Postmortem Submersion Interval (PMSI) is challenging for forensic anthropologists due to the variability of decomposition in aquatic environments. One possible cause of this variability that has received little attention in the literature is the impact of laminar flow. In order to determine whether flow affects the pattern or rate of decomposition, this study analyzed six rabbit carcasses submerged in two different sites one with slow, laminar flow ("River Site") and one without flow ("Canal Site"). A pulley system was designed in order to periodically bring the carcasses out of the water for sampling, with the pattern of decomposition monitored using visual descriptions of the decomposition process and the rate monitored using the Heaton et al. 2010 method of scoring aquatic decomposition in conjunction with accumulated degree days (ADD). The rate of decomposition was found to be significantly different between the two sites (F(1, 126) = 89.569, p less then 0.01). In addition, decomposition was observed to occur in four distinct stages, which were modeled as a series of curves, and a Staged Decomposition Model was created as the possible basis for an improved predictor of PMSI versus the current loglinear approach. While the traditional loglinear approach appears to give a good approximation to the Staged Decomposition Models in environments with laminar flow, this research suggests that it may be less accurate for nonflowing sites. © 2020 American Academy of Forensic Sciences.Although exogenous melatonin supplementation has been suggested to be effective for episodic migraine prophylaxis, there is no conclusive evidence comparing the efficacy of exogenous melatonin supplementation to the other FDA-approval pharmacotherapy for episodic migraine prophylaxis. The aim of the current network meta-analysis (NMA) was to compare the efficacy of exogenous melatonin supplementation in patients with episodic migraine. The randomized controlled trials (RCTs) of placebo-controlled or trials incorporating a placebo in the study designs were eligible for our analyses. All of the NMA procedures were conducted under the frequentist model. The primary outcome was changes in frequency of migraine days and response rate after migraine prophylaxis with melatonin supplementation or pharmacologic interventions. We included 25 RCTs in total with 4499 patients (mean age = 36.0 years, mean female proportion = 78.9%). The NMA demonstrated that migraine prophylaxis with oral melatonin 3 mg/day (immediate release) at bedtime was associated with the greatest improvement in migraine frequency [mean difference = -1.
