Intraductal papillary neoplasm of the bile ducts is a rare tumor type. Management decisions are currently based upon a small case series. The authors have large own experience with IPNB.

The review aims at reporting on clinicopathological features of IPNB in order to provide guidance for management.

We searched PubMed, Medline, Microsoft Academic and Embase databases to identify studies of relevance. The analysis of own experience was also included.

We analyzed 59 retrospective series and 25 cases from authors' clinical experience. The main sign was jaundice and cholangitis, 33% and 48%, respectively. CT's were performed in 63-76% and MR in 40-56%. Intraductal mass was found in 31-32% and duct dilatation in 27-30%. Endoscopic Retrograde Cholangio-Pancreatography (ERCP) was performed in 48-62%. IPNB with invasive carcinoma was found in 35.7-60% and IPNB with intraepithelial neoplasia in 36-60%. Histopathological confirmation before surgery was rare. The main treatment of IPNB is resection, in our material, both, hepatectomy and hepatectomy plus bile duct resections were performed in 40% of patients. The percentage of postoperative complications was 20%. The 5-year survival rate of all IPNB's patients was 53.6%; in patients with associated invasive carcinoma - 22.2% and without invasive carcinoma - 100% (p​=​0.001).

Early surgery is advisable for radiologically suspected IPNB. The results of treatment depend on histopathology. They are worse at intraductal invasive carcinoma than at neoplasm with neoplasia.

The aim of this study is to evaluate how speed affects non-linear measures of variability. Fixed and self-selected speeds were compared to an anatomically scaled speed calculated based on leg length to evaluate which provided a more reproducible result between subjects.

Sixteen subjects ran on a treadmill at a fixed, scaled and self-selected speed and at ±10% in each case. Kinematic data were collected for two minutes at 250Hz for each trial. Sample entropy (SaEn) and maximum Lyapunov exponents (LyE) were calculated from the sagittal knee and hip joint angles to evaluate regularity of gait and local stability. These nonlinear measures were compared to evaluate the dynamic similarity of the movement in each case, and to evaluate speed as a confounding variable in non-linear analysis.

An anatomically scaled speed shows more dynamic similarity than a fixed or self-selected speed with the lowest observed coefficient of variation for each measure. This was found to be statistically significant for both nonlinear measures of the hip (SaEn p=0.038; LyE p=0.040). A poststructural discourse analysis, employing the theoretical ideas of truth, power, knowledge, subjectivity, and normalization, was conducted to explore the mass media's constructions of nurse as hero in the contexts of COVID-19. Media electronic databases were searched between March 1, 2020 to August 1, 2020 to locate newspaper and magazine articles, corporate advertisements, videos, social media postings, and institutional/coal, ethical, and practice supports of nurses as the pandemic continues.Social-emotional information processing (SEIP) is critical for appropriate human interaction. It is composed of processes that underlie how we behave towards others, especially in response to adverse social threat. We conducted a study in 26 healthy participants who completed a validated Video-SEIP (V-SEIP) task in the fMRI scanning environment. The V-SEIP phases studied included encoding (ENC) of socially relevant information, hostile attribution (HA) of motive, and the negative emotional response (NER) the participant would have in the context of the video vignettes. The ENC phase was associated with activation of amygdala, left ventrolateral prefrontal cortex, right middle temporal gyrus, and visual cortex, the HA phase associated with activation of several brain regions including frontal and temporal cortex as well as basal ganglia and cerebellum, while the NER phase was associated with activation in the midbrain with regions involving the periaqueductal gray, basal ganglia, and the cerebellum. We suggest that this V-SEIP task represents a novel neuro-biomarker for the study of SEIP and that it can be extended for use in a number of psychiatric conditions in which anger, irritability, and impulsive aggressive are prominent features.
