Alfaxalone has been used increasingly in biomedical research and veterinary medicine of large animals in recent years. However, its effects on the cerebral blood flow (CBF) physiology and intrinsic neuronal activity of anesthetized brains remain poorly understood.

Four healthy adult rhesus monkeys were anesthetized initially with alfaxalone (0.125mg/kg/min) or ketamine (1.6mg/kg/min) for 50min, then administrated with 0.8% isoflurane for 60min. The results showed increased activities of caspases 8, 9 and 3, between 72-120 hours after administration of MPP+, both in striatum and midbrain. After this study, we tested the effect of CuSO4 on MPP+ neurotoxicity, showing a diminution of the apoptotic damage induced by MPP+, decreased levels of enzymatic activity of caspases 8 (-34 and -25 %), 9 (-25 and -42 %) and 3 (-40 and -29 %) in striatum and midbrain, respectively. Finally, we performed an immunohistochemical analysis, evidencing a decreased number of apoptotic cells in the groups pretreated with copper sulfate pretreatment compared to the control group. Simulation-based training is a valuable component of cardiothoracic surgical education. Effective curriculum development requires consensus on procedural components and focused attention on specific learning objectives. Through use of a Delphi process, we established consensus on the steps of video-assisted thoracoscopic surgery (VATS) left upper lobectomy and identified targets for simulation.

Experienced thoracic surgeons were randomly selected for participation. Surgeons voted and commented on the necessity of individual steps comprising VATS left upper lobectomy. Steps with greater than 80% of participants in agreement of their necessity were determined to have established "consensus." Participants voted on the physical or cognitive complexity of each, or both, and chose steps most amenable to focused simulation.

Thirty thoracic surgeons responded and joined in the voting process. Twenty operative steps were identified, with surgeons reaching consensus on the necessity of 19. Components deemed most difficult and amenable to simulation included those related to dissection and division of the bronchus, artery, and vein.

Through a Delphi process, surgeons with a variety of practice patterns can achieve consensus on the operative steps of left upper lobectomy and agreement on those most appropriate for simulation. This information can be implemented in the development of targeted simulation for VATS lobectomy.

Surgical myectomy eliminates symptoms in the majority of patients with obstructive hypertrophic cardiomyopathy (HCM), but dyspnea and/or angina can recur in some. This study investigates clinical features associated with a lack of clinical improvement postoperatively.

Between March 2007 and December 2012, 963 patients underwent transaortic septal myectomy at our Clinic. 601 patients received standardized follow-up questionnaires, which were answered by 409 (68.1%). We compared clinical characteristics of patients with and without improvement, and identified predictors of worsening health using a multivariable proportional odds ordinal logistic model.

Of 409 patients, 329 (80.4%) indicated better health status at follow-up, and 80 (19.6%) responded that their health had stayed the same or worsened. The median (IQR) age of patients with perceived better health was 56.8 (47.4, 65.7) years, and those without improvement was 63.0 (50.0, 70.2) years. New York Heart Association class III/ IV dyspnea was present in 90.
