0001, and median ± SD) with 100% symptomatic relief at one year. To achieve this, 13 patients required further dilation(s), one required laparoscopic Heller myotomy, and two required repeat POEM. CONCLUSIONS POEM is a viable and safe treatment for pediatric patients with achalasia. We demonstrate improvement in symptoms and procedure proficiency with minimal intra- and postoperative complications. TYPE OF STUDY Prospective cohort study. LEVEL OF EVIDENCE Level II. Virtual surgical planning (VSP) promises higher accuracy, efficiency, and superior patient outcomes, helping normalize outcomes from surgeons of different experience levels. A systematic review was conducted in agreement with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The objective was to evaluate the accuracy and secondarily efficiency of VSP compared with free-hand surgery, for mandibular reconstruction with free flaps. Six studies met inclusion criteria and had quantitative data suitable for meta-analysis. Intercondylar distance and gonion angle were used to assess accuracy, evaluated by mean change from preoperative VSP and postoperative imaging. ACY-738 clinical trial The mean weighted difference in VSP intercondylar distance was 2.0 mm, compared with 3.9 mm for free hand (P=0.101) and mean change in gonion angle for VSP was 3.6°, compared with 7.7° for free hand (P less then 0.05). Efficiency assessed by mean ischemia time, was 73.8min and 109.9min, for VSP and free hand, respectively (P=0.203), and by total operative time, which was 391.8 min and 457.6 min in the VSP and free hand, respectively (P=0.340). VSP is consistently proven to be more accurate and efficient than traditional free-hand surgery; however, a standardized method for accuracy and efficiency measurements is still missing, causing heterogeneity among the scientific reports. STATEMENT OF PROBLEM How the properties of the implant-abutment unit may affect the peri-implant soft-tissue seal, whose stability is considered key to safeguarding the implant from bacterial contamination and preserve peri-implant health conditions, is unclear. PURPOSE The purpose of this systematic review and meta-analysis of animal studies was to investigate whether material and surface properties of transmucosal implant components can influence the peri-implant soft-tissue adhesion at a histological level. MATERIAL AND METHODS An electronic and hand search was conducted until August 2019. Histological animal studies comparing soft-tissue response to abutment or transmucosal collar with different materials and/or surface characteristics were selected by 2 independent reviewers. Risk of bias in individual studies was evaluated. Histomorphometric data on the dimension of the peri-implant attachment were recorded, and a quantitative synthesis by a meta-analysis was performed. Risk of bias in individual studiele data on this topic. STATEMENT OF PROBLEM The polymerization of acrylic resin materials used for the direct fabrication of anterior deprogramming devices results in a temperature increase because of the exothermic reaction. This heat release might cause thermal trauma to the pulp, compromising the tooth's vitality. PURPOSE The purpose of this in vitro study was to compare the temperature increase and the timing of the maximum temperature recording obtained inside the pulp chamber of a maxillary central incisor during the polymerization of different resins materials used for the direct fabrication of anterior deprogramming devices. MATERIAL AND METHODS The exothermic reaction was examined during the polymerization of 4 different polymethyl methacrylate resin products Pattern Resin LS; GC America, DuraLay inlay pattern resin; Reliance Dental, Kallocryl CPGM red; Speiko, and mega-Model Resin NF; Megadental. A polymeric matrix was fabricated to simulate the anterior deprogramming device. A freshly extracted intact maxillary central ihe DuraLay inlay pattern resin material. The mean time recordings ranged from 133 seconds for the Kallocryl CPGM red material to 266 seconds for the DuraLay inlay pattern resin material. CONCLUSIONS The Pattern Resin LS revealed the lowest exothermic reaction (P less then .05) when compared with the other materials tested in this study, whereas Kallocryl CPGM red had the shortest time increase without being statistically significantly different. STATEMENT OF PROBLEM Endocrowns are a monoblock type of restoration that use the pulp chamber and remaining coronal tooth structure as a means of retention. However, data on their long-term survival and success rates as compared with conventional crowns are lacking. PURPOSE The purpose of this systematic review and meta-analysis was to collate published work on endocrowns to assist clinicians in making decisions on when and whether they are an appropriate restorative option with a predictable outcome for extensively damaged endodontically treated teeth. MATERIAL AND METHODS Databases such as PubMed (MEDLINE), Scopus, EMBASE, Cochrane library, and Google Scholar were searched up to June 2019 for clinical and in vitro studies on endocrown survival and success rates. For the meta-analysis, endocrown and conventional crown survival and success rates were compared, and the pooled effects were presented as relative risks and 95% confidence intervals using a random effects model. RESULTS Ten studies fulfilled the inclusion criteria (3 clinical and 7 in vitro) and were included in the systematic review. The meta-analysis of the clinical studies showed an estimated overall 5-year survival rate of 91.4% for endocrowns and 98.3% for conventional crowns. The estimated overall 5-year success rates were 77.7% for endocrowns and 94% for conventional crowns. There were no significant differences in overall survival or success estimates between the assessed restorations (P>.05). CONCLUSIONS Additional well-designed clinical studies with long-term assessment are needed; however, endocrowns appear to be a promising conservative restorative option with acceptable long-term survival for endodontically treated posterior teeth in selected patients.
