Approximately 60% of the UK's leisure horses are kept at livery yards under the management and oversight of a livery yard owner or manager (LYO/M), yet their role has received little research attention. This study used the COVID-19 pandemic as a lens through which to view LYO/Ms' decisions around equine care and management at a time when changes to usual practice were necessary. Qualitative research methods were used. Up to 3 interviews were conducted with 24 different LYO/Ms over nine months (n = 48). Discussion threads from open-access UK discussion fora were also analysed. All data were anonymised and analysed using a Grounded Theory methodology. Prior to the pandemic, equine care and management practices varied greatly across yards, and yard cultures were a product of LYO/Ms' construction of good equine care, their business model, and the need to balance human and equine contentment. The role of the LYO/M was to maintain an equilibrium between those interlinked factors. During the pandemic, LYO/Ms adopted new measures designed to influence the movement of horse owners and other people on yards to minimise the risk of COVID-19 transmission. During this time, LYO/Ms reported prioritising equine wellbeing by limiting change to equine routines and management wherever possible. Instead of altering equine management, there was an expectation that the lives of humans would be moulded and re-shaped to fit with the government COVID-19 guidelines. These results highlight the importance of routines, traditions and cultures in each individual yard. Maintaining the standard of care for the horse was prioritised regardless of who provided that care.Cognitive impairment (CI) is a remarkable feature in pediatric-onset multiple sclerosis (POMS). The Symbol Digit Modalities Test (SDMT) is increasingly used to explore CI in MS. Recently, a four-point worsening on the SDMT score has been demonstrated to correlate with a clinically meaningful cognitive worsening in adult MS. We conducted a post hoc analysis of a randomized computer-assisted rehabilitation trial for attention impairment in POMS to test the clinical meaningfulness of the changes in SDMT scores at the end of the trial (delta SDMT). A four-point SDMT cut-off was applied. POMS patients exposed to specific computer training (ST) and non-specific training (nST) were compared. Data of 16 POMS (9 females, age 15.75 ± 1.74 years) patients were analyzed. At the end of the trial, 25% of patients reported no clinically significant changes (-3 to 3), 12.5% a clinically significant worsening (≤-4) and 62.5% a clinically significant improvement (≥4) in the delta SDMT. The proportion of patients reporting a clinically meaningful improvement was significantly (p = 0.008) higher (100%) in patients exposed to ST in comparison to those (25%) exposed to nST. Such abuses lead to long-term psychophysical and relational consequences and victims are often afraid of asking for support from both parents and professionals. This case report shows the story of a 17-year-old adolescent, Sara, involved by her mother in a strategic counseling process, to solve BDSM-type sexual addiction (slavery and discipline, domination and submission, sadism and masochism), self-aggressive behavior, and alcohol abuse issues. The strategic counseling process is structured in 15 sessions and was based on problem-solving techniques and corrective behavioral strategies. During the sessions, it emerged that Sara had been a victim of sexual violence at the age of 6 and that she had never talked about the rape with anyone. At the age of 12, she began to experience social anxiety and shame, feelings that led her to use alcohol and seek violent sexual partners and bondage relationships. During the counseling sessions, Sara elaborated on her trauma, becoming more aware of her resources and her desires, and she learned to manage the sense of guilt and shame associated with the violence suffered, through alternative strategies. In this research, a novel systemic inflammation response score (SIRS) was calculated, and its prognostic value for postoperative stage I-III breast cancer (BC) patients was analyzed. Methods 1583 BC patients were included in this research. Patients were randomly divided into a training cohort (n = 1187) and validation cohort (n = 396). SIRS was established in the training cohort based on independent prognostic hematological indicator, its relationship between prognosis and clinical features was analyzed. Then, a nomogram consisted of SIRS and clinical features was established, its performance was examined by calibration plots and receiver operating characteristic curve analysis. Results The SIRS was an independent prognostic indicator for BC patients, and a high-SIRS was related to multifocality, advanced N stage, and worse prognosis. Incorporating SIRS into a nomogram could accurately predict the prognosis of BC patients, the results of receiver operating characteristic (ROC) curve analysis showed that the area under the curve (AUC) of nomogram was up to 0.
