To determine whether cfDNA parameters are altered in preeclampsia, we conducted a case-control study using prospectively collected maternal plasma (n=20 preeclampsia, n=22 normal) using our in-house validated prenatal screening assay. Methods and Results Isolated cfDNA was quantified, sequenced using Illumina NextSeq 500, and the placental-derived fraction was determined. Clinical and test characteristics were compared between preeclampsia and controls, followed by comparisons within the preeclampsia cohort dichotomized by cfDNA concentration. Lastly, cfDNA parameters in preeclampsia were correlated with markers of disease severity. Maternal age, body mass index, gestational age at delivery, cesarean rate, and neonatal birthweight were expectedly different between groups (P≤0.05). The placental-derived cfDNA fraction did not differ between groups (21.4% versus 16.9%, P=0.06); however, total cfDNA was more than 10 times higher in preeclampsia (1235 versus 106.5 pg/µL, P less then 0.001). This relationship persisted when controlling for important confounders (OR 1.22, 95% CI 1.04-1.43, P=0.01). The dichotomized preeclampsia group with the highest cfDNA concentration delivered earlier (33.2 versus 36.6 weeks, P=0.02) and had lower placental-derived fractions (9.1% versus 21.4%, P=0.04). Among preeclampsia cases, higher total cfDNA correlated with earlier gestational age at delivery (P=0.01) and higher maximum systolic blood pressure (P=0.04). Conclusions At diagnosis, total cfDNA is notably higher in preeclampsia, whereas the placental derived fraction remains similar to healthy pregnancies. In preeclampsia, higher total cfDNA correlates with earlier gestational age at delivery and higher systolic blood pressure. However, the US has no national paid sick leave policy, and even unpaid leave via the Family and Medical Leave Act (FMLA) of 1993-often viewed as a foundation for new paid leave legislation-is often inaccessible to workers. We analyzed data from a nationally representative survey to determine the extent to which specific FMLA features produce gaps and disparities in leave access. We then used comparative policy data from 193 countries to analyze whether these policy features are necessary or prevalent globally, or whether there are common alternatives. We found that the FMLA's minimum hours requirement disproportionately excludes women, whereas its tenure requirement disproportionately excludes Black, Indigenous, and multiracial workers. Latinx workers also face greater exclusion because of employer size requirements. Of the 94 percent of countries that provide permanent paid sick leave, none broadly restrict leave based on employer size, and 93 percent cover part-time workers without a minimum hours requirement. The aim of this study was to examine adaptation in response to temporal perturbation using real-time perturbation of ongoing speech. Method Twenty native English speakers with no reported history of speech or hearing disorders participated in this study. The study consisted of four word blocks, using the phrases "a capper," "a gapper," "a sapper," and "a zapper" (due to issues with the implementation of perturbation, "gapper" was excluded from analysis). In each block, participants completed a baseline phase (30 trials of veridical feedback), a ramp phase (feedback perturbation increasing to maximum over 30 trials), a hold phase (60 trials with perturbation held at maximum), and a washout phase (30 trials, feedback abruptly returned to veridical feedback). Word-initial consonant targets (voice onset time for /k, g/ and fricative duration for /s, z/) were lengthened, and the following stressed vowel (/æ/) was shortened. Results Overall, speakers did not adapt the production of their consonants but did lengthen their vowel production in response to shortening. Vowel lengthening showed continued aftereffects during the early portion of the washout phase. Although speakers did not adapt absolute consonant durations, consonant duration was reduced as a proportion of the total syllable duration. This is consistent with previous research that suggests that speakers attend to proportional durations rather than absolute durations. This risk can be minimized with reduction of contrast volume via preprocedural risk assessment. We aimed to identify quality gaps for implementing the available risk scores introduced to facilitate more judicious use of contrast volume. Methods and Results We grouped 14 702 patients who underwent percutaneous coronary intervention according to the calculated NCDR (National Cardiovascular Data Registry) AKI risk score quartiles (Q1 [lowest]-Q4 [highest]). We compared the used contrast volume by the baseline renal function and NCDR AKI risk score quartiles. Factors associated with increased contrast volume usage were determined using multivariable linear regression analysis. The overall incidence of AKI was 8.9%. The used contrast volume decreased in relation to the stages of chronic kidney disease (168 mL [SD, 73.8 mL], 161 mL [SD, 75.0 mL], 140 mL [SD, 70.0 mL], and 120 mL [SD, 73.7 mL] for no, mild, moderate, and severe chronic kidney disease, respectively; P less then 0.001), albeit no significant correlation was observed with the calculated NCDR AKI risk quartiles. Of the variables included in the NCDR AKI risk score, anemia (7.31 mL [1.76-12.9 mL], P=0.01), heart failure on admission (10.2 mL [6.05-14.3 mL], P less then 0.001), acute coronary syndrome presentation (10.3 mL [7.87-12.7 mL], P less then 0.001), and use of an intra-aortic balloon pump (17.7 mL [3.9-31.5 mL], P=0.012) were associated with increased contrast volume. Conclusions The contrast volume was largely determined according to the baseline renal function, not the patients' overall AKI risk. These findings highlight the importance of comprehensive risk assessment to minimize the contrast volume used in susceptible patients.Purpose The purpose of this study is to use variability on tests of basic auditory processing to allow identification of those tests that could be used clinically to describe functional hearing ability beyond the pure-tone audiogram and clinical speech-in-noise tests. Method Psychoacoustic tests implemented using the Portable Automated Rapid Testing system on a calibrated iPad were evaluated for nine young normal-hearing participants (M age = 21.3, SD = 2.5) and seven hearing-impaired participants (M age = 64.9, SD = 13.5). Participants completed 10 psychoacoustic subtests in a quiet room. Correlational analyses were used to compare performance on the psychoacoustic test battery with performance on a clinical speech-in-noise test and with the 4-frequency pure-tone average (4FreqPTA). Results Spectral processing ability was highly correlated with 4FreqPTA, and temporal processing ability showed minimal variability across the hearing-impaired group. Tests involving binaural processing captured variability across hearing-impaired listeners not associated with 4FreqPTA or speech-in-noise performance. Conclusions Tests that capture the ability to use binaural cues may add information to what current clinical protocols reveal about patients with auditory complaints. Further testing with a larger sample size is needed to confirm the need for binaural measurements and to develop normative data for clinical settings.

Palliative care (PC) can help patients with cancer manage symptoms and achieve a greater quality of life. However, there are many barriers to patients with cancer receiving referrals to PC, including the stigmatizing association of PC with end of life. This study explores factors that obscure or clarify the stigma around PC referrals and its associations with end of life in cancer care.

A qualitative descriptive design using grounded theory components was designed to investigate barriers to PC referrals for patients receiving treatment at an outpatient cancer center. Interviews with patients, caregivers, and oncology professionals were audio-recorded, transcribed, and independently coded by three investigators to ensure rigor. Participants were asked about their perceptions of PC and PC referral experiences.

Interviews with 44 participants revealed both obscuring and clarifying factors surrounding the association of PC as end of life. Prognostic uncertainty, confusion about PC's role, and social network influence all perpetuated an inaccurate and stigmatizing association of PC with end of life. Contrarily, familiarity with PC, prognostic confidence, and clear referral communication helped delineate PC as distinct from end of life.
