An EUS/ERCP approach equates to shorter time interval to initiation of PDAC therapies.Rheumatic mitral stenosis is still a pathological condition that affects young patients and is an important cause of mortality. 2017-European Guidelines for the management of valvular heart disease suggest a percutaneous approach with a mitral commissurotomy for the treatment of symptomatic pregnant women. Mitral commissurotomy procedure involves radiation exposure that is incompatible with the pregnancy condition. In our case, we present percutaneous mitral commissurotomy (PMC) to a 28-week pregnant woman with a low-radiation dose and the use of transesophageal echocardiography. The woman presented with a mitral transvalvular mean gradient of 21.6 mmHg and with symptoms non-responsive to medical treatment. PMC was driven by a transesophageal echocardiographic probe. This case demonstrates the feasibility and safety of PMC in a pregnant woman with severe rheumatic mitral stenosis.In Western society, there has been a history of perceiving adolescence as a time of "storm and stress," during which youth may go through heightened negative experiences such as conflict with parents, disengagement from school, and emotional dysfunction. Despite increasing attention to positive youth development, such negative stereotypes of adolescence may be held by youth themselves, which undermine their behavioral and neural development. However, youth's stereotypes of teen emotionality in particular (i.e., beliefs that teens typically experience greater emotionality than younger children) and the role of such stereotypes in youth's emotional functioning have not been examined. This longitudinal study investigated the reciprocal relations between youth's negative stereotypes of teen emotionality and their emotional functioning (i.e., emotional expressivity and emotion regulation) in Hong Kong and Mainland China, two regions in China sharing Chinese cultural traditions but differing in the extent of exposure to Western influence (N = 1269; 55% girls; M age = 12.86 years). Although youth in Hong Kong saw the teen years as a time of heightened emotionality more than did their counterparts in Mainland China, such stereotypes predicted youth's greater emotional expressivity and less emotion regulation over the 7th grade in both regions. Moreover, in both regions, youth's negative expressivity reciprocally predicted their stronger stereotypes of teen emotionality over time. Taken together, these findings demonstrate the role of youth's stereotypes of teen emotionality in shaping their emotional expressivity and emotion regulation during early adolescence, and also the role of youth's negative expressivity in reinforcing their stereotypes. Moreover, these findings highlight the relevance of the Western-popularized perception of adolescence as a time of "storm and stress" in non-Western regions in a world of increasing globalization and societal change.The purpose of this study was to assess the incidence of fatal hemorrhage complicated with methamphetamine (MA) poisoning and to examine the postmortem computed tomography (PMCT) features of fatal intracerebral hemorrhage (ICH) with and without MA poisoning. The study also attempted to determine the differences in PMCT between those two groups. Consecutive medicolegal autopsy data from November 2011 to February 2018 were searched for 3044 cases. First, the incidence and distribution of all cases of nontraumatic fatal hemorrhage with various causes were examined. Second, cases of ICH on the basal ganglia and brain stem were extracted. The PMCT findings were compared with respect to nine parameters volume of hematoma, ventricular perforation, midline shift distance, aortic calcification, calcification of aortic valve, calcification of coronary artery, cardiothoracic ratio, circumference of ascending aorta, and volume of bladder contents. Of the 3044 cases, 97 were nontraumatic fatal hemorrhage; of these 97 cases, 20 were classified as MA poisoning with 9 ICH cases, and 60 cases were classified as non-MA poisoning with 14 ICH cases. A statistically significant difference in ages was observed between the two groups. On PMCT comparison of ICH, statistically significant differences were evident in the midline shift distance and calcification of the aortic valve. Forensic radiologists should be aware of the possibility of ICH with MA poisoning if fatal hemorrhage is detected on PMCT. Younger age, less calcification of the aortic valve, and a remarkable midline shift may be the keys to recognition.

Hypertensive disorders of pregnancy (HDP)-gestational hypertension, preeclampsia, and eclampsia-are a leading cause of adverse maternal and perinatal outcomes internationally. Prevention, timely diagnosis, and prompt management can reduce associated morbidity. The purpose of this review is to compare international guidelines pertaining to HDP.

Fourteen HDP guidelines were compared relative to guidelines for the United States (US) where the authors practice. Aspirin is universally recommended for high-risk women to reduce preeclampsia risk. Diagnoses of chronic hypertension, gestational hypertension, and preeclampsia in pregnant women are similar, although blood pressure (BP) thresholds for antihypertensive medication initiation and treatment targets vary due to the limitations in high-quality evidence. There are differences among international HDP guidelines related to dose and timing of aspirin initiation, thresholds for antihypertensive medication initiation, and BP targets.
