Brügger et al. (Science Advances, 7, eabc8790, 2021) recently introduced the use of remote thermography to investigate how monkeys process socially significant vocalizations. This is a welcome addition to the arsenal of comparative psychologists. Interpretative issues abound, but the ability to non-invasively measure physiological responses that do not immediately result in overt behavior opens interesting lines of investigation.
People with type2 diabetes mellitus (T2DM) and diabetic kidney disease (DKD) have increased morbidity and mortality risk. Angiotensin-converting enzyme inhibitors (ACEi) or angiotensinII receptor blockers (ARB) are recommended to slow kidney function decline in DKD. This representative, real-world data analysis of patients with T2DM was performed to detect onset of DKD and determine methods and timing of DKD diagnosis and time to initiation of ACEi/ARB therapy.
Patients diagnosed with T2DM before January1, 2016 who developed DKD between January1, 2017 and June30, 2019 were identified from a longitudinal ambulatory electronic health record (EHR) dataset (Veradigm Inc). In a contemporary, large national cohort of patients with T2DM, progression to DKD was common but likely underrepresented. The low rate of DKD-screening labs, along with sizable delays in diagnosis of DKD and initiation of ACEi/ARB therapy, indicates that many patients who progress to DKD are not being properly treated.
Anemia is one of the most common complications of chronic kidney disease (CKD). As a result of the side effects of high doses of recombinant human erythropoietin (rhEPO) and the differences in the standard dose of the injectable iron, this study aimed to evaluate the effect of high and low intravenous iron supplementation on hematinic parameters and EPO requirements in patients under hemodialysis.
This multicenter, randomized, double-blind clinical trial was conducted on 60 patients with CKD admitted to Sina and 29 Bahman hospitals in Tabriz, Iran in 2019-2020 to undergo hemodialysis. Diabetes screening is key to early disease detection and prompt treatment, but it is not known whether US adults with a disability receive similar levels of diabetes screening as individuals without a disability. We compared diabetes screening levels in US adults with a disability to those without one. Using national 2017 Behavioral Risk Factor Surveillance System surveys, we determined the prevalence of diabetes screening by disability status in US adults who fall under the American Diabetes Association's recommended screening guidelines those younger than 45 years old with a body mass index (BMI) ≥ 25 kg/m2 and those aged 45 years and older. We used logistic regression modelling to examine the impact of disability status on diabetes screening while adjusting for diabetes associated sociodemographic and clinical factors. In people with a disability, around 50% of those younger than 45 years old with a BMI ≥ 25 kg/m2 and 33% of those 45 years or older did not receive screening. In the under 45 years with a BMI ≥ 25 kg/m2 screening group, individuals with a disability had a slightly higher but non-significant prevalence, but a lower adjusted odds of diabetes screening compared to those without a disability. People with a disability under age 45 had a slightly lower but again non-significant prevalence but a higher adjusted odds of diabetes screening than did those without a disability who were age 45 or older. Additional interventions are needed to improve diabetes screening levels among US adults with a disability at high risk of developing diabetes as screening is a critical initial step in the diabetes management process.
Measurement of the pituitary stalk (PS) diameter does not always solve the issue of minimal PS thickening. A previously undescribed image is found at the infundibular level on high resolution thin section T2W MRI in a large number of normal individuals. We speculate that this image-whose exact origin is still unknown-may serve as a marker of the normal infundibulum.
In the last 6months, 350 consecutive adult patients suspected of sellar pathology or controlled after medical or surgical treatment prospectively underwent a pituitary MRI including a sagittal T2W high resolution sequence. One hundred twelwe patients presenting a pituitary mass with suprasellar extension or those whose PS was not entirely visible were excluded.
A short focal annular T2 hypointense thickening of the wall of the infundibular recess of the third ventricle, more pronounced anteriorly was found in 151/238 patients. Additionally, a more or less tiny ventral extension was demonstrated on sagittal T2W sequence in 105/151 patients. These images were not identified on T1W or on T1W gadolinium enhanced sequences.
