INTRODUCTION Radiologically inserted gastrostomy (RIG) placement in patients with amyotrophic lateral sclerosis (ALS) carries risks related to periprocedural sedation and analgesia. To minimize these risks, we used a paravertebral block (PVB) technique for RIG placement. METHODS We retrospectively reviewed patients with ALS undergoing RIG placement under PVB between 2013 and 2017. RESULTS Ninety-nine patients with ALS underwent RIG placement under PVB. Median (range) age was 66 (28 to 86) years, ALS Functional Rating Scale-Revised score was 27 (6 to 45), and forced vital capacity was 47% (8%-79%) at time of RIG placement. Eighty-five (85.9%) patients underwent RIG placement as outpatients, with a mean postanesthesia care unit stay of 2.3 hours. The readmission rate was 4% at both 1 and 30 days postprocedure. DISCUSSION PVB for RIG placement has a low rate of adverse events and provides effective periprocedural analgesia in patients with ALS, the majority of whom can be treated as outpatients. © 2020 Wiley Periodicals, Inc.In the biopharmaceutical industry, capillary gel electrophoresis using sodium dodecyl sulfate (CE-SDS) assesses the purity, heterogeneity and stability of therapeutic proteins. However, for mAb-1 and mAb-2, typical CE-SDS under reducing conditions produced atypical protein peak profiles which led to biased purity results thus were not acceptable for biologics manufacturing. This bias was caused by the formation of method-induced higher molecular weight (HMW) artifacts, the levels of which correlated with protein concentration. Here we show that adding sodium tetradecyl and hexadecyl sulfates (STS and SHS respectively) to the sample and the sieving gel buffer solutions was required to prevent formation of aggregate artifacts and to maintain detergentprotein uniformity, suggesting their importance during the sample preparation steps of heat denaturation and subsequent cooling as well as during capillary migration. For these proteins, we show that this uniformity was likely due to the ability of these detergents to bind proteins with markedly higher affinities compared to SDS. "CE-SCX S" methods, with CX representing the chain length of the various detergents utilized, were developed with an STS sample buffer and an SHS-containing sieving gel buffer that minimized artifacts and provided robust characterization and release results for mAb-1 and mAb-2. There are plenty of cultivars that possess variability in their productivity and quality among the different cuts. Therefore, accurate agronomic and qualitative characterization is crucial for selecting the most promising cultivars for breeding and feeding purposes. In the present study, the agronomic characteristics, ruminal degradability and fermentation measures of five cuts of the five most prominent Egyptian beseem clover cultivars (Helaly, Serw, Giza6, Gemmeza1 and Sakha4) were evaluated. RESULTS The Giza6 cultivar produced a significantly higher fresh yield and high whole plant dry matter content on the 3rd cut. Giza6 was among the superior cultivars in crude protein content and had the highest values of gas production at the 3rd cut, and ruminal degraded acid detergent fiber and propionate concentrations at the 2nd cut, at the same time as presenting the lowest ammonia concentrations at the 3rd and 5th cuts. Overall, the 3rd cut produced significantly higher fresh yield across all cultivars, except for Sakha4. The 3rd cut also produced a higher leaf to stem ratio compared to the 4th and 5th cuts. The 1st three cuts had higher degraded neutral detergent fiber than the 4th and 5th cuts. The 1st cut was characterized by low propionate concentration in the Helaly and Gemmeza1 cultivars compared to the other cuts. CONCLUSION Expanded production of the high-yielding, high-quality cultivar Giza6 is recommended. We examined the peripheral blood films of 32 patients and found reactive lymphocytes as shown in the top images in 23 cases (72%).
