Several studies have described the clinical features of COVID-19 in solid organ transplant recipients. However, many have been retrospective or limited to more severe cases (hospitalized) and have not routinely included serial virologic sampling (especially in outpatients) and immunologic assessment.

The aims of the study were to determine the prevalence of HCP-associated ICD/ACD and to characterize relevant allergens.
This study is a retrospective analysis of North American Contact Dermatitis Group (NACDG) patch test data, 2001-2016.
Of 38,775 patients tested, 3481 (9.0%) had positive patch test reactions associated with HCPs. The HCP-positive patients were significantly more likely to be female (79.9% vs 66.0%) and/or have primary sites of dermatitis on the face (32.0% vs 27.8%) or scalp (15.4% vs 2.2%) compared with the HCP-negative patients (P < 0.0001). Of 4908 HCP-associated positive patch test reactions, 86.9% (n = 4263) were due to allergens on the NACDG screening series; p-phenylenediamine (35.8%), methylisothiazolinone (9.7%), methylchloroisothiazolinone/methylisothiazolinone (8.7%), and cocamidopropyl betaine (5.9%) were the most frequent. Most reactions (87.7%, 3736/4263) were currently clinically relevant. The most common job associated with 366 occupationally related NACDG HCP-associated allergens was hairdresser/cosmetologist (71.9%). Two hundred eighty-two patients (0.7%) had ICD associated with HCPs. Shampoo/conditioners were the most frequent source of NACDG HCP-associated reactions (47.3%) and HCP-associated ICD (45.0%).

Approximately 795,000 Americans experience a new or recurrent stroke every year, and it remains one of the leading causes of disability worldwide. Patient engagement in rehabilitation and physical activity has been associated with improvements in functional outcomes during inpatient rehabilitation. There is limited knowledge of the barriers and facilitators to patients' engagement in stroke rehabilitation.
An integrative review with a systematic approach was conducted to explore the current available literature on barriers and facilitators to patient engagement in stroke inpatient rehabilitation.
Twenty-five studies were included in the final review. Four major themes described the identified barriers and facilitators (1) stroke impact, (2) self-efficacy, (3) therapeutic relationship, and (4) motivating factors. Individualized patient-centered goal setting was a commonly studied intervention found to successfully increase engagement.
Engagement in stroke rehabilitation is associated with improved patient outcomes. Using the identified themes from this review, nurses can develop focused interventions aimed at reducing barriers and increasing stroke patient engagement.

The role of Mohs micrographic surgery (MMS) in the management of melanoma of the head and neck (HNM) has been controversial. The authors systematically reviewed the local recurrence rate of melanoma in situ (MIS) and T1a melanomas using MMS compared with conventional wide local excision (WLE) and staged excision (SE).
To systematically review the local recurrence rate of early-stage melanomas of the HNM treated with MMS versus WLE or SE.
A search of English medical literature was conducted through the common databases until November 26, 2019. Using PRISMA guidelines for the treatment of MIS and T1a melanoma with MMS, WLE, or SE, our search yielded a total of 32 articles.
Mohs micrographic surgery has a lower local recurrence rate for early-stage melanomas over both SE and WLE pooled recurrence risk 0.8% (95% confidence interval [CI] 0.4-1.1) versus 2.5% (95% CI 1.5-3.4) versus 8.7% (95% CI 5.1-12.2) (p < .001), respectively.
Mohs micrographic surgery may offer a lower recurrence rate than SE or WLE in the management of early-stage melanomas of the face or HNM. Further clinical validation in a randomized controlled trial is required.
