The objective of this study was to determine the frequency and predictors of biliary tract cancer (BTC) with deficient DNA mismatch repair (dMMR) in Japan.
Immunostaining and microsatellite instability analysis were performed for mismatch repair-related proteins in tissue specimens from 662 patients who underwent surgery for BTC between 2001 and 2017 to identify dMMR-BTC. We compared dMMR-BTC and proficient MMR (pMMR)-BTC based on patient demographics, pathological features, and host immune responses characterized by the percentage of stromal tumor infiltrating lymphocytes (sTIL percentage) and tertiary lymphoid structures (TLS).
The incidence of dMMR-BTC was 2.3%. Significant predictors of dMMR-BTC were its primary lesion being intrahepatic cholangiocarcinoma (odds ratio [OR] 6.34, P=.004), presence of signet ring cell component (OR 35.62, P<.001), sTIL percentage ≥40% (OR 3.43, P=.038), and presence of TLS (OR 22.22, P<.001). The sensitivity, specificity, and negative likelihood ratio for any one or more of these four variables to be positive were 93.3%, 57.8%, and 0.12, respectively.

Secondary analysis of data from three clinical studies, following deliberate self-harm (n= 460), cognitive behaviour therapy for health anxiety (n= 444) and a 30-year follow-up of 200 anxious/depressed patients. Social function and personality were assessed using the Social Functioning Questionnaire (SFQ) and the Personality Assessment Schedule, with its ICD-11 modification. A 5-item short version of the SFQ, the Short Social Functioning Questionnaire (SSFQ), was also developed. The SFQ score in the first two studies (area under curve [AUC] 0.64 and 0.65) partly predicted personality status; in the third study, this achieved close agreement (AUC SFQ 0.85 [95% CI 0.8-0.9]; AUC SSFQ 0.84 [95% CI 0.78-0.89]). In all studies, social function deteriorated linearly with increasing personality pathology. Cut-off points of 4 on the SSFQ and 7 on the SFQ had high sensitivity (SSFQ 82%-90%; SFQ 82%-83%) and acceptable specificity (SSFQ 66%-75%; SFQ 69%-75%) in identifying personality disorder in the third study.

In this study, twenty Egyptian cases with neck-AN were enrolled, where each case was exposed to four sessions with 2weeks apart of both fractional CO
laser on the right half of the neck and retinoic acid peel on the left half of the neck. Cases were assessed by a scoring system Acanthosis Nigricans Area and Severity Index (ANASI) score, two blinded dermatologists, and dermoscopically before and one month after treatment.
We found a highly statistically significant improvement among both treated groups regarding (ANASI) score and dermatologist's assessments. Bedside, the degree of sulci cutis, cristae cutis, brown-to-dark brown dots, and milia-like cysts, dermoscopic sign improvement was evident in both treated groups. However, fractional CO
laser shows the superior result to retinoic acid peel in the treatment.
