Exercise intolerance, evaluated by O2 consumption, predicts mortality in cystic fibrosis (CF). People with CF exhibit skeletal muscle dysfunctions that may contribute to an imbalance between O2 delivery and utilization. Sildenafil, a phosphodiesterase type 5 inhibitor, increases blood flow and improves O2 consumption, although the exact mechanisms in CF have yet to be elucidated. Thus, we hypothesized that exercise intolerance in CF is limited primarily by an impaired skeletal muscle O2 utilization, and sildenafil improves exercise tolerance in CF by addressing this mismatch between O2 demand and extraction.
Fifteen individuals with mild to moderate CF and 18 healthy controls completed an incremental exercise test and measurements of gaseous exchange, chronotropic response, hemodynamics, and O2 extraction and utilization. This study examined one-year reliability and construct validity of survey items relating to time spent on muscle-strengthening exercise (MSE) in a subset of a large prospective cohort.
Participants (N = 293 men, 433 women; age 32-73 years) were selected from the Cancer Prevention Study-3. Information was collected using a one-year pre- and post-survey and four 7-day diaries throughout the year. The pre- and post-surveys collected time spent on MSE in two ways one question captured MSE activities performed during a typical 24-hour period (24-hour survey), and another question captured leisure-time physical activities performed in hours/week and months/year (LTPA survey). Time spent on MSE using the LTPA survey was calculated for individual MSE items and summed for total MSE time. To date, there is limited research showing whether HRV is practical in youth female athletes and rowers during short periods of overload training. The purpose of this study was to evaluate the practicality of HRV in documenting training responses during a period of overload training in youth female rowers. Time-domain (SD of N-N intervals, SDNN; root mean square of successive differences, RMSSD) and nonlinear (SD1) indices of HRV were recorded during baseline training, daily during the 6-day training camp, and 1 week after the camp in 5 athletes from an elite, high-school, rowing team. Training duration and rate of perceived exertion were recorded to document training load. Training load during the camp was 76% above the athlete's normal workload (2,258 ± 459 vs. 1,280 ± 356 arbitrary units (a.u.)). Using progressive statistics, cardiac vagal activity (RMSSD and SD1) was very likely reduced during each day of the camp when compared with baseline training, although returned to baseline within a week of the training camp. Interestingly, SDNN was reduced throughout the training camp and remained reduced up to a week after the training camp (78% likely; effect size = -0.32). These insights add value to HRV's use in youth sport and provides coaches with an easy, cost-effective means to monitor the physiological response to training, allowing fine-tuning of training, potentially enhancing performance.
