Introduction Recently, the generic formulation of FP/SAL FDC has been approved in COPD. Although FP/SAL FDC has been the first long acting FDC approved in COPD, no systematic review assessed the effect of this combination for the treatment of COPD by considering specifically Phase IV studies. The aim of this review was to systematically assess the effect of FP/SAL FDC in COPD patients enrolled in Phase IV studies.Areas covered The question of this systematic review was to examine the evidence regarding the impact of FP/SAL FDC for the treatment of COPD by searching for Phase IV studies in the ClinicalTrials.gov database.Expert opinion Generic drugs represent an effective cost-saving step for health care budgets in the treatment of COPD and should be used in agreement with current recommendations and prescription accuracy. FP/SAL is recommended for the initiation therapy just in a small percentage of symptomatic patients that are at high risk of exacerbation with blood eosinophil counts ≥300 cells per μl. At follow-up FP/SAL FDC can be escalated to triple ICS/LABA/LAMA combination or switched to LABA/LAMA combination by considering symptoms, exacerbations, lack of response to ICS, inappropriate original indication, and ICS-related adverse events such as pneumonia.Introduction Acute respiratory distress syndrome (ARDS) is a severe form of acute lung injury common in critically ill patients and characterized by significant morbidity and mortality. It frequently manifests long-lasting effects beyond hospitalization, from cognitive impairment to physical weakness.Areas covered Several complications of ARDS have been identified in patients after hospital discharge. The authors conducted literature searches to identify observational studies, randomized clinical trials, systematic reviews, and guidelines. A summary of is presented here to outline the sequelae of ARDS and their risk factors with a focus on the limited but growing research into possible therapies. Long term sequelae of ARDS commonly identified in the literature include long-term cognitive impairment, psychological morbidities, neuromuscular weakness, pulmonary dysfunction, and ongoing healthcare utilization with reduced quality of life.Expert opinion Given the public health significance of long-term complications following ARDS, the development of new therapies for prevention and treatment is of vital importance. Furthering knowledge of the pathophysiology of these impairments will provide a framework to develop new therapeutic targets to fuel future clinical trials in this area of critical care medicine.BACKGROUND Atypical knee joint biomechanics after anterior cruciate ligament reconstruction (ACLR) are common. It is, however, unclear whether knee robustness (ability to tolerate perturbation and maintain joint configuration) and whole body movement strategies are compromised after ACLR. PURPOSE To investigate landing control after ACLR with regard to dynamic knee robustness and whole body movement strategies during sports-mimicking side hops, and to evaluate functional performance of hop tests and knee strength. STUDY DESIGN Controlled laboratory study. METHODS An 8-camera motion capture system and 2 synchronized force plates were used to calculate joint angles and moments during standardized rebound side-hop landings performed by 32 individuals with an ACL-reconstructed knee (ACLR group; median, 16.0 months after reconstruction with hamstring tendon graft [interquartile range, 35.2 months]) and 32 matched asymptomatic controls (CTRL). Dynamic knee robustness was quantified using a finite helical axis approexion strength (12% deficit). CONCLUSION Knee robustness was not affected by ACLR during side-hop landings, but alterations in movement strategies were seen for the trunk, hip, and knee, as well as long-term deficits in knee flexion strength. CLINICAL RELEVANCE Knee robustness is lowest immediately after landing for both the ACLR group and the CTRL and should be targeted in training to reduce knee injury risk. Therefore, a noninvasive assay is needed to classify and guide the treatment of glomerular diseases. Here, we develop and apply a high-throughput and quantitative microarray platform to characterize the immunoglobulin proteome in the serum from 419 healthy and diseased patients. The immunoglobulin proteome-clinical variable correlation network revealed novel pathological mechanisms of glomerular diseases. Furthermore, an immunoglobulin proteome-multivariate normal distribution (IP-MiND) mathematical model based on the correlation network classified healthy volunteers and patients with idiopathic membranous nephropathy with an average recall of 48% (23-80%) in the discovery cohort and 64% (63-65%) in an independent validation cohort. Our results demonstrate the translational utility of our microarray platform to glomerular diseases as well as its clinical potential in characterizing other human diseases.
