This is a retrospective cohort study conducted on febrile neutropenic adult cancer patients presenting to the emergency department of the American University of Beirut Medical Center.

11.4% of 263 patients had pneumonia although 27.7% had respiratory signs/symptoms. 17.1% of those who were symptomatic and did a CXR were found to have pneumonia. 41.7% of those who were symptomatic and did a CT were found to have pneumonia. 30% had negative findings on CXR but pneumonia on CT.

Patients with positive findings of pneumonia on chest imaging mainly had solid tumors, profound neutropenia, a higher CCI and a longer LOS. Vaccination is an essential means for prevention of tuberculosis infection, but the effects of various vaccines on the intestinal flora of mice and their response to Mycobacterium tuberculosis (Mtb) infection remain poorly understood.

In this study, two different vaccinations - ESAT6 and ESAT6 + TLR8 agonists - were administered to mice transgenic for human TLR8 to investigate gut microbiota characteristics following vaccination. Gut microbiota was investigated by next generation sequencing in the MiSeq Sequencing System. Adonis analysis was used to evaluate the effect of variables on gut bacterial community stucture. Chao1, Shannon index, and phylogenetic diversity index were used to explore the gut bacterial diversity.

The results showed that different vaccines have significant influence on mice intestinal bacteria (adonis analysis, p < 0.01), with gut bacterial diversity within the ESAT6 + TLR8 agonists group being significantly decreased compared to the ESAT6 treatment group (p < 0.01). Following infection with Mtb via tail vein injection, the bacterial community structure within the control versus vaccinated groups altered significantly (adonis analysis, p < 0.01), and the altered changed genera were markedly different between the groups. Following infection, Bifidobacteria differed between the groups, indicated that they play a vital role in the response to infection.

Most children with serious infection diseases suffer from malnutrition. Vitamin D participates in the immune response through endogenous antimicrobial peptides (AMPs) regulation. The aim of this study is to investigate the expression of 25-hydroxyvitamin D3 [25(OH)D3], AMPs [LL-37 and human β-defensin 2 (HBD-2)] in the children with pertussis.

Serum levels of 25(OH)D3, LL-37, and HBD-2 were detected in 116 children with pertussis aged at 1-12 months (67 males and 49 females). Fifty healthy infants at similar age were employed as normal controls.

The serum 25(OH)D3 levels in the children with mild (27.30 ± 5.98 ng/ml) and severe (24.40 ± 6.27 ng/ml) pertussis were significantly lower than that in the healthy group (30.16 ± 5.13 ng/ml; p <0.01). The vitamin D deficiency rates in children with mild (55.9%) and severe (78.12%) pertussis were significantly higher than that in the control group (34%; p < 0.01). The serum levels of LL-37 and HBD-2 were significantly higher in pertussis patients. Spearman rank correlation analysis did not show any correlation of 25-(OH)D3 with LL-37 or HBD-2.

Rickettsioses are zoonotic diseases caused by pathogenic bacteria of the genus Rickettsia and transmitted to man by means of arthropod vectors such as ticks, fleas, mites and lice. Historically, Caldas Department has reported a significant number of cases of murine typhus to the Colombian national health surveillance system, and consequent studies of flea-borne rickettsiosis identified the circulation of Rickettsia typhi and Rickettsia felis in multiple municipalities. Our aim was to genotype species of Rickettsia detected in fleas collected from domestic and wild mammals in Caldas.

Flea samples were taken by convenience sampling from dogs, cats and wild mammals (rodents and marsupials) in 26 municipalities. Specimens were classified by current taxonomic keys and pooled for DNA extraction and molecular screening for Rickettsia spp. by PCR amplification of gltA, htrA and sca5 genes. Positive samples were genotyped by enzyme digestion (htrA) and sequencing.

A total of 1388 flea samples were collected. Rickettsia DNA was amplified in 818 (gltA), 883 (htrA) and 424 (sca5) flea pools.
