Human sensory and motor systems deteriorate with age. When walking, older adults may therefore find it more difficult to adjust their steps to new visual information, especially considering that such adjustments require control of balance as well as of foot trajectory. Our study investigates the effects of ageing on lower limb responses to unpredictable target shifts.

Participants walked on a treadmill with projected stepping targets that occasionally shifted in the medial or lateral direction. The shifts occurred at a random moment during the early half of the swing phase of either leg. Kinematic, kinetic and muscle activity data were collected.

Older adults responded later and corrected for a smaller proportion of the shift than young adults. The order in which muscle activation changed was similar in both groups, with responses of gluteus medius and semitendinosus from about 120 to 140ms after the shift. Most muscles responded slightly later to lateral target shifts in the older adults than in the young adults, but this difference was not observed for medial target shifts. Ageing delayed the behavioural responses more than it did the electromyographic (EMG) responses.

Our study suggests that older adults can adjust their walking to small target shifts during the swing phase, but not as well as young adults. Furthermore, muscle strength probably plays a substantial role in the changes in online adjustments during ageing.

The underlying physiology of post-reflux swallow-induced peristaltic wave (PSPW) is unclear. We aimed to 1) calculate the probability of a random association between reflux and PSPW; 2) characterize factors that could underlie triggering of PSPW and 3) assess the chemical clearance effect of PSPW in healthy asymptomatic subjects.

A total of 251 impedance-pH tracings from healthy asymptomatic subjects were analysed. Twenty consecutive tracings from this pool with 20-40 reflux episodes/24h and a PSPW index higher than 50% were separately analyzed to evaluate the probability of a random association between reflux and PSPW. The characteristics of reflux episodes followed by a PSPW were compared with those not associated with PSPW.

A mean time interval of 29.3s between a reflux episode and the first swallow captured 71% of total reflux episodes, and 67% of accompanying swallows were non-random. Compared to reflux without PSPW, reflux episodes with PSPW were more frequently acidic (P = 0.048), mixed with gas (P < 0.0001), of high proximal extent (P < 0.0001), while awake (P < 0.0001), and with shorter chemical clearance time (P = 0.040). High proximal extent, gas presence and occurring while awake were independent factors associated with PSPW (P < 0.0001).

However, phage translocation has not been investigated in leukemia patients yet. We recruited 44 leukemic patients and 52 healthy adults and quantified the levels of 3 phages in peripheral blood, which were the most positive phages screened from fecal samples. The content of 16S rRNA in plasma was detected by qPCR to assess the intestinal mucosa of these patients. Spearman's rank correlation was used to analyze the relationship between phage load and the relevant clinical data. We found the most prevalent phages in fecal samples were λ phage, Wphi phage, and P22 phage, and λ phage had the highest detection rate in plasma (68%). Phage content was affected by chemotherapy and course of disease and correlated with the levels of CRP (r = 0.43, p = 0.003), sCD14 (r = 0.37, p = 0.014), and sCD163 (r = 0.44, p = 0.003). Our data indicate that plasma phage load is a promising marker for gut barrier damage and that gut phage translocation correlates with monocyte/macrophage activation and systemic inflammatory response in leukemic patients.
