Pancreaticobiliary diseases are the important causes of morbidity and mortality worldwide. Among the imaging modalities, Transabdominal ultrasound (TUS) is cheap, available, and noninvasive but it has some limitations. Endoscopic ultrasound (EUS) is invasive but it has some diagnostic and therapeutic advantages over TUS. This study was aimed to see the diagnostic yields of EUS and TUS in the pancreatobiliary diseases. This cross sectional study was conducted in Sir Salimullah Medical College Mitford Hospital (SSMC&MH), Dhaka, Bangladesh from March 2017 to February 2019. All (n=222) patients were evaluated clinically and with relevant investigations. TUS and EUS were done in all patients. Endoscopic retrograde cholangiopancreatography (ERCP) was done in 60 patients. Among 222 patients 56.8% were males; mean age was 46±16 years; the main presenting symptoms were abdominal pain and jaundice. In diseases of biliary tree, EUS showed dilated CBD alone or in combination with stone in 50 and 67 cases and TUS showed 3%, 40%, and 37% respectively. EUS is more sensitive than TUS in diagnosing pancreaticobiliary disorders. It is of paramount importance in patients in diagnosing CBD dilatation, choledocholithiasis, biliary microlithiasis and pancreaticobiliary neoplasm. EUS has important role before proceeding to further management by more invasive techniques like ERCP or surgery.Ventral hernia after abdominal surgery is a common complication. Several techniques for the repair of ventral hernia have been described from time to time and it is a great challenge for a surgeon. The mesh placement by sublay technique authorized by Rives and Stoppa in Europe has been reported to be effective with low recurrence rate. The objective of this study was to evaluate the efficacy of sublay technique of mesh placement in ventral hernia. This prospective study was carried out in the Department of Surgery, Dhaka Dental College, Bangladesh from January 2017 to December 2018. A total of 21 patients with ventral hernia were included in this study. Data collected in data collection sheet regarding demographic data, severity of symptoms and post operative complains of patients which were then analyzed. Age ranged from 21-60 years. Male were 5(23.80%) and female were 16(76.20%). Dragging pain were 7(33.33%), irreducibility were 4(19.05%) but swelling were 100%. Incisional hernia was 18(85.71%) and para-umbilical hernia was 3(14.29%). Post-operative complication were seroma1 (4.76%), major wound infection 1(4.76%), minor infection 1(4.76%) but no recurrence. Sublay mesh repair in ventral hernia was found to be a better and effective technique with minimal complication rate.Type 2 diabetic mellitus (T2DM) patients with sub-clinical cardiac autonomic neuropathy (CAN), which precedes clinically apparent CAN, may have reduced heart rate variability (HRV); yet may remain undetected by conventional autonomic test. The objective of this study was to explore the short-term HRV by power spectral analysis as a tool to detect sub-clinical diabetic cardiac autonomic neuropathy (CAN). This cross -sectional study included 30 recently diagnosed and 54 long-term T2DM male patients (aged 45 to 55 years and without any symptoms of CAN) and 30 age and body mass index (BMI) matched healthy male subjects as controls. This study was done from January 2015 to January 2016 in Endocrinology OPD, BSMMU, who were previously or then diagnosed by the physician at this very OPD following the diagnostic criteria set by the American Diabetes Association Ewing battery of cardiovascular reflex tests (CARTs) was performed to rule out signs of autonomic neuropathy in enrolled patients. Power spectral analysis of frequency domain signals of short term HRV were done by digital data acquisition device RMS Polyrite-D. For statistical analysis, One way ANOVA and independent sample 't' test were used. LF power, HF power and HF normalized units (nu) were significantly lower, and LF nu and LF/HF were significantly higher in long-term T2DM compared to recently diagnosed T2DM (p less then 0.05) and control (p less then 0.001). LF power (p less then 0.01) and HF power (p less then 0.001) were significantly lower in recently diagnosed T2DM compared to control. Results from this study demonstrate the presence of autonomic dysfunction in CAN asymptomatic long-term T2DM patients. Short-term HRV, thus, seems to be an invaluable tool to detect sub-clinical diabetic CAN.Pilonidal sinus disease is a common anorectal condition usually seen in young adult patients. Various methods have been described over the years and there is ongoing debate regarding the ideal method. A-674563 nmr This study was conducted to evaluate the advantages, results of rhomboid excision and Limberg flap reconstruction in the management of sacrococcygeal pilonidal sinus disease. This cross-sectional study was conducted in Surgery Unit-I of Department of Surgery, Shaheed Suhrawardy Medical College Hospital, Dhaka, Bangladesh a tertiary care centre from July 2016 to November 2017. It includes 19 patients who were treated for sacrococcygeal pilonidal sinus disease by Limberg (Rhomboid) flap. All patients that underwent the procedure had good postoperative outcome with minimal postoperative discomfort and were discharged in 3-4 days. There were 3 cases with complications. Limberg flap coverage is very effective for pilonidal disease with low complication rates, reduced hospital stay, low recurrence rates, earlier healing and shorter time off-work. This technique can be easily mastered and used as an indispensable tool for treating sacrococcygeal pilonidal sinus disease.Diabetes Mellitus (DM) represents one of the biggest challenges in our country affecting hundred millions of people worldwide, both in developed countries and in developing ones. The objectives of the study were to assess the proportion of diagnosed and undiagnosed cases of diabetes mellitus among above 40 years old population in a selected rural area in Jamalpur, Bangladesh. A descriptive type of cross sectional study was conducted from September 2018 to December 2018. The respondents of the study were taken purposively. Using semi-structured questionnaire data were derived from face to face interview. The data were analyzed by using statistical for social science package (SPSS) 25.0 version for data entry and analysis. 53.4% of the respondents were aged between 40-50 years and 60.6% were female, 98.3% were Muslim, 57.2% were housewife. The study revealed that nearly half of the respondents (44.9%) had no formal education, 27.1% had primary, 14.8% had secondary, 8.9% had graduate and above and 4.2% had higher secondary education.
